INTRODUCTION: The breath-actuated nebulizer (BAN) and the handheld nebulizer (HHN) are 2 nebulizers used in the ED of Cooper University Hospital. The purpose of this study was to compare the nebulizers to identify which device resulted in a resolution of symptoms with fewer treatments. The primary hypothesis was that adult ED patients with a chief complaint of wheezing and dyspnea who were given nebulized treatments via the BAN would require less nebulizer treatments than those patients given nebulized treatments via HHN. In addition, the secondary purposes of the study was to determine if the

BACKGROUND: Rapid access chest pain clinics have facilitated the early diagnosis and treatment of patients with coronary heart disease and angina. Despite this important service provision, coronary heart disease continues to be under-diagnosed and many patients are left untreated and at risk. Recent advances in imaging technology have now led to the widespread use of noninvasive computed tomography, which can be used to measure coronary artery calcium scores and perform coronary angiography in one examination. However, this technology has not been robustly evaluated in its application to the c

OBJECTIVES: To describe patterns of service use and to predict risk factors for re-presentation to a metropolitan emergency department (ED) among people who are homeless.
METHODS: A retrospective cohort analysis was undertaken over a 24-month period from a principal referral hospital in Melbourne, Australia. All ED visits relating to people classified as homeless were included. A predictive model for risk of re-presentation was developed using logistic regression with random effects. Rates of re-presentation, defined as the total number of visits to the same ED within 28 days of discharge, we

The primary aim of this study was to evaluate the ability of a nurse practitioner in geriatrics, working in the emergency department of a tertiary referral hospital, to assess high-risk elderly patients comprehensively. A secondary aim was to explore patient characteristics associated with referral to community aged care services. Of 469 patients assessed by the nurse, 327 (70%) were admitted to the hospital. A comprehensive set of data was obtained for 334 (71%) patients. For 142 patients not admitted, 163 new referrals were made, mostly to the Aged Care Assessment Team. Those referred were m

AIM: The objective of this study is to examine the association between moral courage, psychological resilience, and the occurrence of workplace violence among emergency department nurses.
BACKGROUND: Workplace violence represents a major occupational hazard in healthcare institutions worldwide. Moral courage refers to nurses' ability to make principled decisions when facing ethical conflicts, thereby avoiding moral distress. Psychological resilience is defined as an individual's positive internal capacity to cope with adversity and stress. Such violent incidents severely undermine nurses' phy

BACKGROUND: Culture shapes care delivery, professional communication, and the development of trust between patients and providers. Culture encompasses identifiable elements, shared values and beliefs, and underlying assumptions that shape behaviour. The Emergency Department (ED) subculture arises from its complex, high-pressure focus on acute illness. ED professionals increasingly encounter older patients with frailty, whose complex but often non-urgent needs require more holistic, individualised care. However, the ED subculture may hinder the provision of appropriate geriatric emergency care.

Posterior circulation strokes are frequently underrecognized in emergency departments, especially when dizziness is the presenting complaint without overt motor deficits. Diagnostic delays often occur when providers anchor on benign causes such as benign paroxysmal positional vertigo. Early computed tomography and magnetic resonance imaging may miss acute infarcts in the posterior fossa, whereas the bedside head-impulse, nystagmus, and test of skew plus hearing examination has demonstrated greater sensitivity for detecting stroke in acute vestibular syndrome. A 76-year-old male with vascular r

OBJECTIVES: Falls are a leading cause of morbidity and mortality among older adults, with residents of skilled nursing facilities (SNFs) at a particularly high risk. Although emergency department (ED) transfers after falls are common, they carry risks of delirium, infection, and care fragmentation. The objective of this study is to compare ED transfer rates between witnessed and unwitnessed falls in SNFs.
DESIGN: Single-site, observational study.
SETTING AND PARTICIPANTS: A 120-bed SNF in the greater Roanoke area, serving both long-term and short-term rehabilitation residents. Data were coll

BACKGROUND: Accurate assessment of the patient’s condition in the emergency department (ED) is critical for individuals with acute myocardial infarction (AMI). In this study, point-of-care ultrasound (PoCUS) was utilized to evaluate ventricular wall motion in AMI patients in the ED, aiming to determine its clinical value in early risk stratification and condition assessment. METHODS: A retrospective cohort study was conducted by systematically collecting clinical data from patients who presented to the ED, were ultimately diagnosed with AMI, and underwent cardiac PoCUS examinations. RESULTS: A

INTRODUCTION: The screening, brief intervention, and referral to treatment (SBIRT) approach is an evidence-based tool that combines standardized screening for unhealthy or risky alcohol and drug use with principles of motivational interviewing to promote behavior change and connect patients with the appropriate treatment and recovery support services. There is an increased demand for health care students and providers to be trained in SBIRT. We developed a curriculum to improve medical students' attitudes toward and proficiency in administering SBIRT.
METHODS: The curriculum was deployed as p

BACKGROUND AND IMPORTANCE: Hypotension and advanced-age are significant risk factors for increased sepsis-related mortality. However, the relationship between the timing of hypotension in the emergency department (ED) and the prognosis of elderly patients with septic shock is little understood.
OBJECTIVE: To determine the effect of hypotension on arrival at the ED with the prognosis of elderly patients with septic shock.
DESIGN, SETTINGS, AND PARTICIPANTS: A retrospective analysis of a multicenter registry that was prospectively collected from 12 EDs. Patients aged older than 65 years who we

BACKGROUND: Ionised hypocalcaemia can worsen haemorrhage due to its role in clot formation, vascular tone and cardiac contractility. Current guidelines recommend maintaining normal ionised calcium levels in the bleeding trauma patient. However, guidance does not specify when to replace calcium, and for paediatric trauma, it is unclear when clinicians should measure or decide to replace calcium.
OBJECTIVES: This study aimed to survey the practices, protocols and opinions regarding the measurement and replacement of calcium in paediatric major trauma.
METHODS: A cross-sectional survey with sin
