Pre-exposure prophylaxis to prevent the acquisition of HIV-1 infection (PROUD): effectiveness results from the pilot phase of a pragmatic open-label randomised trial.

BACKGROUND: Randomised placebo-controlled trials have shown that daily oral pre-exposure prophylaxis (PrEP) with tenofovir-emtricitabine reduces the risk of HIV infection. However, this benefit could be counteracted by risk compensation in users of PrEP. We did the PROUD study to assess this effect. METHODS: PROUD is an open-label randomised trial done at 13 sexual health clinics in England. We enrolled HIV-negative gay and other men who have sex with men who had had anal intercourse without a condom in the previous 90 days. Participants were randomly assigned (1:1) to receive daily combined …

Geographical alignment of prehospital critical care services with population need for out-of-hospital cardiac arrest patients.

BACKGROUND: Prehospital critical care is associated with improved outcomes following out-of-hospital cardiac arrest (OHCA), but may not be distributed equitably. AIM: To examine the alignment between prehospital critical care provision and neighbourhood OHCA risk, predicted survival, and deprivation across three neighbouring ambulance regions in England. METHODS: Data on all OHCAs treated by three neighbouring English ambulance services (2018-2023) were aggregated to the neighbourhood level. Prehospital critical care response time predictions were generated from service-supplied operational …

Culicoides biting midges (Diptera: Ceratopogonidae) feeding on donkeys in the United Kingdom, with reference to the risk of transmission and persistence of African horse sickness virus.

African horse sickness virus (AHSV: Sedoreoviridae; Orbivirus) causes a severe and often fatal disease in horses (African horse sickness: AHS) and is transmitted almost exclusively by Culicoides biting midges (Diptera: Ceratopogonidae). In recent years, unprecedented outbreaks of AHSV have occurred in new geographical foci in Thailand and other related Culicoides-borne viruses continue to emerge unexpectedly, causing disease outbreaks in northern Europe. This study investigated Culicoides abundance and diversity at a donkey (Equus asinus) sanctuary in southern England. The incidence and severi…

Economic evaluation of the Liverpool heart failure virtual ward model.

BACKGROUND: A virtual ward (VW) supports patients who would otherwise need hospitalization by providing acute care, remote monitoring, investigations, and treatment at home. By March 2024, the VW programme had treated 10 950 patients across six speciality VWs, including heart failure (HF). This evaluation presents the economic assessment of the Liverpool HF VW. METHOD AND RESULTS: A comprehensive economic cost comparison model was developed by the York Health Economics Consortium (University of York) to compare the costs of the VW to standard hospital inpatient care [standard care (SC)]. Prediction of individualised 6-month mortality risk in opioid use disorder.

BACKGROUND: People with opioid use disorder (OUD) have substantially higher standardised mortality rates compared with the general population. However, lack of individualised prognostic information presents challenges in personalisation of addiction treatment delivery. AIMS: To develop and validate the first prognostic models to estimate 6-month all-cause and drug-related mortality risk for people diagnosed with OUD using indicators recorded at baseline assessment in addiction services in England. METHOD: Thirteen candidate prognostic variables, including sociodemographic, injecting status a…

Little lives-reading between the lines: insights from the Northampton Infirmary Eighteenth Century Child Admission Database.

The presence of children in eighteenth-century English voluntary hospitals is an area of increasing interest and attention. The Northampton Infirmary admission records detail inpatient and outpatient ages from 1744 to 1804, allowing for longitudinal investigations of children in the institution. The most common distempers affecting children were surgical infections, infectious diseases, and skin diseases; fifty-six per cent of the child patients were male and 43.3 per cent were female. Nearly seventy-five per cent of children left the hospital 'cured'. This article outlines the Northampton Inf

Tailoring opioid use prevention content for juvenile diversion programs with adolescents and their caregivers.

INTRODUCTION: Opioid use (OU) skyrockets as youth transition into young adulthood, indicating adolescence is a critical time for prevention. The juvenile legal system (JLS) presents an ideal setting for OU prevention, as it is the single largest referral source for youth outpatient OU treatment, after self-referral. However, no OU prevention programs have been developed for youth in JLS diversion programs or their families. The current formative study established specific OU prevention needs and preferences of families in JLS programs, to guide future tailored prevention efforts. METHODS: We

Association of genetic liability to smoking initiation with e-cigarette use in young adults: A cohort study.

BACKGROUND: Tobacco smoking and e-cigarette use are strongly associated, but it is currently unclear whether this association is causal, or due to shared factors that influence both behaviours such as a shared genetic liability. The aim of this study was to investigate whether polygenic risk scores (PRS) for smoking initiation are associated with ever use of e-cigarettes. METHODS AND FINDINGS: Smoking initiation PRS were calculated for young adults (N = 7,859, mean age = 24 years, 51% male) of European ancestry in the Avon Longitudinal Study of Parents and Children, a prospective birth cohort

A Prospective Study of Rumination and Irritability in Youth.

Although youth irritability is linked with substantial psychiatric morbidity and impairment, little is known about how personal characteristics influence its course. In this study we examined the prospective associations between angry and depressive rumination and irritability. A sample of 165 school pupils aged 12-14 years were assessed at two time points six months apart. They completed measures of irritability at Times 1 and 2 and depressive and angry rumination at Time 1. In line with our hypotheses, we found that angry rumination is significantly associated with irritability six months la
