Effect of early analgesia with erector spinae plane block to reduce ventilation after severe chest trauma: the multicentre randomised controlled TARGET study protocol.

INTRODUCTION: Blunt chest trauma is frequently associated with rib fractures, and inadequate pain control may lead to respiratory complications and prolonged ventilatory support. In spontaneously breathing patients, pain impairs coughing and secretion clearance, promoting atelectasis and respiratory failure, while in mechanically ventilated patients, it may delay weaning. Current French guidelines recommend intravenous multimodal analgesia as first-line therapy followed by thoracic epidural or paravertebral block if pain remains uncontrolled. However, these techniques have limited applicabilit

Phase III LUNA clinical programme: design of four randomised, double-blind, placebo-controlled studies assessing the efficacy and safety of tozorakimab in patients with symptomatic COPD and a high risk of exacerbations.

INTRODUCTION: The dysregulation of both reduced (IL-33red) and oxidised (IL-33ox) interleukin (IL)-33 has been implicated in chronic obstructive pulmonary disease (COPD) inflammation and remodelling processes. Tozorakimab, an anti-IL-33 monoclonal antibody, inhibits both IL-33red and IL-33ox activity. METHODS AND ANALYSIS: The tozorakimab LUNA programme comprises four ongoing, multicentre, randomised, double-blind, parallel-group, placebo-controlled, phase III studies evaluating the efficacy and safety of tozorakimab in participants with symptomatic COPD and a history of exacerbations receivi

Obesity and arthritis of the knee Study (OAKS): study protocol for a prospective cohort study examining the treatments and outcomes of people with obesity and severe knee arthritis.

INTRODUCTION: People with body mass index (BMI) ≥35 kg/m2 have an approximately 19-fold increased risk of undergoing total knee replacement (TKR); however, many UK integrated care boards (37%) have restrictive policies which limit access to TKR for people based on BMI. Therefore, access to both surgical and non-surgical treatments varies widely, exacerbating existing health inequalities. It remains unclear how decisions about offering TKR are made in people with severe knee osteoarthritis, which weight-loss interventions are provided in practice and how different management pathways relate to

Non-inferiority of saline versus corticosteroid injections for trapeziometacarpal osteoarthritis: a protocol for a pilot and feasibility randomised controlled trial.

INTRODUCTION: Trapeziometacarpal osteoarthritis (TMO) is among the most prevalent forms of upper extremity osteoarthritis. It is frequently associated with significant levels of pain and disability, particularly among ageing women. The typical care pathway for TMO relies on non-surgical approaches for up to 2 years, before surgery is considered. One of the most common non-surgical approaches is an intra-articular cortisone injection. However, these are not universally recommended due to their safety profile and unclear efficacy compared with saline injections. Recent evidence suggests that sal

Spondyloarthritis Inception Cohort of Southern Denmark (SPINCODE): a protocol for a single centre prospective observational cohort study in patients suspected of axial spondyloarthritis.

INTRODUCTION: Spondyloarthritis (SpA) comprises a heterogeneous group of chronic inflammatory diseases. Axial SpA (axSpA) predominantly involves the sacroiliac joints and the spine and typically presents with back pain and stiffness. Modern imaging techniques, especially MRI, allow earlier detection of axSpA compared to conventional radiography, but imaging findings still need cautious interpretation to facilitate a clinical diagnosis. This cohort study of patients with suspected SpA is designed to (1) compare demographic, clinical, laboratory, imaging data and patient-reported outcomes (PROs)

Exploring the mechanism of reducing the risk of falls in knee osteoarthritis patients by using Baduanjin based on the functional connection of vestibular network: a study protocol for a randomised controlled trial.

INTRODUCTION: Falls are a major global public health challenge and a leading cause of severe consequences, such as fractures, traumatic brain injuries and even death among older adults. Older adults with knee osteoarthritis (KOA) are at a higher risk of falling and tend to experience more severe injuries due to impaired balance. Baduanjin, a mind-body exercise, has been shown to improve lower limb muscle strength and balance in older adults and to reduce fall risk in those with KOA; however, its specific mechanisms remain unclear. Therefore, this study aims to examine the effectiveness of Badu

Adjuvant mecobalamin for anti-VEGF therapy in retinal vein occlusion: protocol of a single-centre, randomised, double-blinded controlled trial.

INTRODUCTION: Retinal vein occlusion (RVO), a common retinal vascular disease, is frequently treated with anti-vascular endothelial growth factor (anti-VEGF) agents as first-line therapy. However, anti-VEGF monotherapy lacks neuroprotective effects, primarily targets vascular leakage and neovascularisation, and requires frequent long-term injections that impose substantial economic burdens. Therefore, combined therapeutic strategies that address both vascular pathology and neural damage are being explored. This article describes the protocol for evaluating mecobalamin (a widely used neuroprote

FinnDiane LifeOne Study: influence of ageing on people with type 1 diabetes - a prospective observational cohort study protocol.

INTRODUCTION: Life expectancy for people with type 1 diabetes has increased due to improved treatment of diabetes and its comorbidities, allowing many to reach old age. Still, we lack knowledge of how individuals with type 1 diabetes age. On one hand, those who reach older age can be considered survivors, but on the other hand their long-standing diabetes might still exhibit negative impacts on their health and functional ability. Healthy ageing is the WHO's priority for this decade. The focus has shifted from chronological age to functional ability, which reflects the ability of individuals t

CARE study: prospective cohort study on supportive care among paediatric oncology patients in western Kenya-a study protocol.

INTRODUCTION: Childhood cancer presents significant challenges in low- and middle-income countries (LMICs), as survival rates remain substantially low. Supportive care, including nutritional support and infection prevention plus management, is crucial in improving outcomes of childhood cancer patients. To develop evidence-based interventions improving supportive care and survival, insight is needed into local prevalences of malnutrition, colonisation and infections, their association with clinical outcomes and the attitude of parents or legal guardians towards nutritional care and infection pr

Maternal, Infant, Reproductive and Child Health in Cystic Fibrosis (MATRIARCH_CF): a prospective, observational study to evaluate pregnancy and parenthood in females with cystic fibrosis and health of the offspring in the CFTR-modulator era.

INTRODUCTION: Cystic fibrosis transmembrane conductance regulator (CFTR) modulators (CFTRm) have altered the landscape of pregnancy and parenthood in cystic fibrosis (CF). As pregnant and breastfeeding females with CF (FwCF) were excluded from pivotal CFTRm trials, evidence to guide management and counsel families on maternal and child outcomes is limited. While CFTRm are unlicensed for use in pregnancy, substantial clinical benefit means most continue therapy. Early studies indicate that CFTRm cross the placenta and are detectable in breastmilk; reported associations include infant liver dysf

Sensorimotor integration, nutrition and gut microbiota in Ecuadorian autistic children - "Proyecto Wiñay": a research protocol for a comparative cross-sectional study.

BACKGROUND: Autistic children experience disproportionately high rates of malnutrition compared to non-autistic (NA) peers. Sensorimotor integration differences (SMD), prevalent in 69-95% of autistic individuals, are hypothesized to drive this disparity. SMD contributes to core autistic traits that exacerbate feeding difficulties (e.g., food selectivity), potentially leading to malnutrition and gut dysbiosis. Current caregiver-reported SMD assessments lack validity in autistic populations and are prone to cultural and socioeconomic biases, limiting insight into nutritional outcomes. OBJECTIVE

Targeting Vascular Inflammation In Patients with Community-Acquired Pneumonia (TIN-CAP): protocol for a multicentre, randomised, double-blind, placebo-controlled trial.

INTRODUCTION: Inflammation plays a central role in atherosclerosis development and subsequent cardiovascular complications, including heart attack and stroke. Patients with inflammatory conditions such as community-acquired pneumonia (CAP) present with an elevated risk of cardiovascular events, which is likely driven by unresolved systemic inflammation. Targeting this heightened inflammatory burden may present a novel therapeutic strategy to attenuate heart attack risk in CAP survivors. Icosapent ethyl (IPE), an omega-3 fatty acid, demonstrates both pro-resolving and cardioprotective propertie

Prospective Study of Non-Autoimmune Insulin-Deficient Diabetes Subtype in Young Individuals in sub-Saharan Africa (PANDA study): study protocol.

INTRODUCTION: Diabetes affects over 24 million people in Africa, with type 1 and type 2 diabetes being the most recognised forms. Recent studies in sub-Saharan Africa have identified a substantial proportion of children and young adults with clinically diagnosed type 1 diabetes (T1D) who lack evidence of islet autoimmunity despite severe insulin deficiency, suggesting a distinct non-autoimmune insulin-deficient diabetes subtype (NAID). This study aims to characterise the clinical phenotype, assess the underlying pathophysiology and longitudinal progression of this diabetes subtype. METHODS AN

Study on blueberries, protein and exercise for improving frailty and cardiovascular disease (STRONG): protocol for a parallel-group randomised controlled superiority trial in older Nova Scotians.

INTRODUCTION: As frailty and cardiovascular disease are closely linked, lifestyle interventions to reduce frailty could improve cardiovascular health in at-risk individuals. Previous clinical trials have shown that exercise reduces frailty, both individually and in combination with dietary protein. Blueberries can improve vascular health and protect against cellular senescence. The objective of the study on blueberries, protein and exercise for improving frailty and cardiovascular disease (STRONG) is to determine whether a year-long treatment including blueberries, protein and exercise will re

Longitudinal evaluation of minimally invasive protocols for the prevention and control of dental caries in Brazilian schoolchildren: a multicentre study protocol.

INTRODUCTION: Dental caries remains one of the most prevalent chronic diseases in childhood worldwide. In Brazil, untreated carious lesions continue to affect oral health and quality of life in school-aged children. Minimally invasive approaches for caries management have demonstrated feasibility in community settings and may expand access to preventive and restorative care in socially vulnerable populations. METHODS: This multicentre longitudinal intervention study will recruit 50 000 children aged 6-12 years from public schools in five Brazilian cities (Santos, Ponta Grossa, Brasília, Forta

Clinical study of adebrelimab in combination with apatinib and irinotecan for PD-1 inhibitor-ineffective advanced-stage gastric cancer: study protocol for a single-arm, single-centre, exploratory trial.

INTRODUCTION: Immunotherapy has revolutionised cancer treatment. Immune checkpoint inhibitors have demonstrated significant efficacy across multiple tumour types, including gastric cancer, where several approved programmed cell death 1/programmed cell death-ligand 1 (PD-1/PD-L1) inhibitors show promising antitumour activity. While PD-L1 expression serves as a predictive biomarker for PD-1 inhibitor response, and PD-L1-positive patients generally show better outcomes, therapeutic resistance remains a challenge. Many initial responders eventually develop resistance, and surprisingly, some PD-L1-
