Fractures in Paget's disease.

After analyzing the various types of anatomical and radiographic fractures in pagetic bone, the authors explain the reason for their preference for surgical treatment if the lesion is localized in the lower limbs. This choice allows for a reduction in the duration of immobilization and a lowered incidence of local (rapid and intense osteolysis of the pagetic bone) and general complications (hypercalciuria, at times, hypercalcemia). It is best to resort to internal osteosynthesis for diaphyseal fractures and prosthetic substitution for fractures of the femoral neck where treatment with screws a

Fracture propagation propensity of ceramic liners during impingement-subluxation: a finite element exploration.

Although improvements in materials engineering have greatly reduced fracture rates in ceramic femoral heads, concerns still exist for liners. Ceramics are vulnerable to fracture due to impact and from stress concentrations (point and line loading) such as those associated with impingement-subluxation. Thus, ceramic cup fracture propensity is presumably very sensitive to surgical cup positioning. A novel fracture mechanics finite element formulation was developed to identify cup orientations most susceptible to liner fracture propagation for several impingement-prone patient maneuvers. A new device to treat intra-capsular fracture neck of femur non-union.

In adolescents and young adults, femoral neck fractures often result from high-velocity trauma. These fractures are usually of vertical shear pattern. There is an increased incidence of avascular necrosis and non-union, which is difficult to treat. Non-union of fractured neck of femur in young adults is a serious problem. There is growing evidence that these fractures should be treated with an angle-stable device to improve biomechanics at the fracture site. An ideal implant should prevent varus deformation and retroversion of the fracture in order to prevent failure of the osteosynthesis and

A novel biomechanical model for reproducing and analysing the causal mechanisms of PFNA "cut-in" phenomenon.

INTRODUCTION: "Cut-in" is a rare but serious complication associated with cephalomedullary nail fixation for intertrochanteric hip fractures, particularly with Proximal Femoral Nail Antirotation (PFNA). This phenomenon involves paradoxical superomedial migration of the helical blade through the femoral head. Although increasingly recognised, the underlying mechanisms remain incompletely defined. This study aims to describe a new superolateral tensile loading set up in a bidirectional loading model, with the aim of incorporating possible mechanical deviations introduced by abductor weakness. Magnetically Controlled Intramedullary Compression Nailing for Femoral Nonunion in Osteogenesis Imperfecta: A Case Report.

CASE: A 51-year-old nonambulatory woman with type III osteogenesis imperfecta sustained a diaphyseal femur fracture during a transfer, distal to a childhood-placed intramedullary pin. At the time of presentation, the fracture was a painful atrophic nonunion, preventing effective transfer. Retrograde nailing with a motorized magnetic lengthening nail, inserted with preset distraction, allowed gradual compression, solid union, and pain relief. CONCLUSION: This report describes the method of off-label lengthening nail modification used for gradual compression of a femoral nonunion site, achievin

Biomechanics of internal fixation in Hoffa fractures - A comparison of four different constructs.

OBJECTIVE: Compare the biomechanical effectiveness of four different bone-implant constructs in preventing fracture displacement under axial loading. METHODS: Twenty artificial femora had a standardized coronally oriented fracture of the lateral femoral condyle, representing a Hoffa fracture classified as a Letenneur type I. Four different fixation constructs were applied to the synthetic bones for biomechanical testing. The constructs consisted of a posterolateral (PL) buttressing locking plate in conjunction with two cannulated lag screws inserted from posterior to anterior (PA) - Group 1;

Finite element analysis of 3 internal fixations for distal type C3 femur fractures with medial wall bone defects.

OBJECTIVES: Managing 33-C3 femur fractures with medial wall bone defects poses a significant challenge for orthopedic surgeons. The gold standard treatment for arbeitsgemeinschaftfür osteosynthesefragen (AO)/orthopedic trauma association (OTA) 33-C3 distal femur fractures with medial wall bone defects remains elusive. This study employs finite element analysis to compare the stability and mechanical behavior of 3 internal fixation patterns (single lateral distal locking plate, retrograde intramedullary nail, and dual plates) for 33-C3 femur fractures with medial wall bone defects. The aim is t

Mechanical outcomes of the TFNA, InterTAN and IMHS intramedullary nailing systems for the fixation of proximal femur fractures.

INTRODUCTION: Hip fractures have become a major public health priority due to their increasing incidence. Intramedullary (IM) nailing has gained popularity as a surgical technique for managing these fractures. However, comparative studies with extramedullary devices indicate potentially increased mechanical complications associated with IM nailing. Specifically, there have been recent reports of early implant fracture associated with the TFNA system. This study compares the rate of mechanical complications between 3 implants. Secondarily, we analysed the factors associated with implant fractur

Impingement and perforation of the anterior femoral cortex in cephalomedullary nailing: Systematic review and surgical techniques.

BACKGROUND: Anterior femoral cortical impingement and perforation are known risks of cephalomedullary nailing. The incidence of and risk factors for these findings have not been fully established in the literature. The purpose of this review was to answer: (1) What is the incidence of anterior femoral cortical impingement and perforation associated with cephalomedullary nailing of proximal femur fractures? (2) How does incidence vary by nail radius of curvature (ROC)? (3) What populations are at increased risk of impingement and perforation? (4) What surgical techniques prevent their occurrenc

Structure-mechanical analysis of various fixation constructs for basicervical fractures of the proximal femur and clinical implications; finite element analysis.

OBJECTIVE: This present study was conducted to determine the structural-mechanical stability of various fixation constructs through finite element (FE) analysis following simulation of a basicervical fracture and to introduce the clinical implications. MATERIALS AND METHODS: We simulated fracture models by using a right synthetic femur (SAWBONES®). We imported the implant models into ANSYS® for placement in an optimal position. Five assembly models were constructed: (1) multiple cancellous screws (MCS), (2) FNS (femoral neck system®), (3) dynamic hip screw (DHS), (4) DHS with anti-rotation 7.

Diagnostic Criteria and Treatment of Atypical Ulnar Fractures Associated With Prolonged Bisphosphonate Therapy: Multicenter Case Analysis.

PURPOSE: Atypical ulnar fracture (AUF) related to prolonged bisphosphonate therapy is a rare complication. We propose diagnostic criteria of AUFs and present a treatment algorithm. METHODS: Twelve AUFs in 10 patients were studied. The diagnosis of AUF was based on the case definition of atypical femoral fracture (AFF). We investigated clinical and radiographic characteristics of AUFs according to major and minor features of AFFs, and modified the case definition of an AFF to fit the characteristics of AUFs. All AUFs were treated surgically. The radiographic union of fractures was investigated

Hip fractures in children and adolescents.

BACKGROUND: Femoral neck fractures account for <1% of the fractures in children, and are produced by high energy trauma. The most commonly accepted treatment for such fractures is gentle manual anatomical reduction and internal fixation, yielding a healing rate between 80% and 90%. SOURCES OF DATA: Electronic search through major web databases (Medline, Cochrane and Google Scholar). All types of article were eligible for inclusion, except for reviews, systematic reviews, meta-analyses and case reports. The methodological quality of the studies was assessed using the Methodological Index for N

Algorithmic treatment of Busch-Hoffa distal femur fractures: A technical note based on a modified Letenneur classification.

Treatment of distal femur fractures in the coronal plane can be challenging. Depending on fracture line orientation, topography and associated comminution, decision-making regarding approach and fixation is not straightforward and can result in complications. Therefore, treatment of coronal plane distal femur fractures (Busch-Hoffa fractures) should be approached in a systematic manner, leading to efficient planning and operative execution. Here, we offer a proposed treatment algorithm, guiding treatment, approach and fixation based on the modified Letenneur classification of coronal plane dis

Pediatric tibia and femur fractures in patients weighing more than 50 kg (110 lb): mini-review on current treatment options and outcome.

The main objective of this paper is to review the current literature on treatment of tibial and femur fractures in children and adolescents guided by body weight in patients weighing 50 kg (110 lb) or more. A secondary aim of this mini-review was to determine, as per literature review, whether weight  > 50 kg (110 lb) is an identifiable factor associated with increased complication rate. A search of the PubMed/MEDLINE, EMBASE and Cochrane Database of Systematic Reviews databases from 1954 to September 2017 was performed to identify papers related to pediatric tibia and femur fractures in child

[Regional anaesthesia in patients with lower limb injury].

Almost all surgical procedures following injury can be provided in peripheral regional anaesthesia or spinal anaesthesia - under consideration of specific contraindications. The majority of injuries at the lower limb are associated with severe pain and immobilization during the postoperative phase. Moreover, opioids are often required which are related to nausea, vomiting and impairment of vigilance. For avoidance of those side effects, regional anaesthesia techniques should be considered as a more effective approach with a better profile of side effects. Hip and proximal femur fractures are m
