Cadaveric Simulation of Flatfoot and Surgical Corrective Techniques: The Evans Osteotomy vs the Z-Osteotomy.

BACKGROUND: Progressive collapsing flatfoot deformity (PCFD) describes the pathologic form of a flatfoot (pes planus). Pes planus is often treated with lateral column-lengthening (LCL) procedures such as the Evans LCL. The Z-osteotomy has been explored as an alternative to the Evans LCL. We aim to compare bone position using radiographs and dynamic movement using a robotic gait simulator (RGS). METHODS: Flatfoot was produced in cadaveric specimens by attenuating medial arch supporting ligaments and cyclically loading through the tibia. Clinical diagnostic measures from radiographs were used t

Clinical features and ultrasound findings of a rare musculoskeletal system disease-neuromuscular choristoma.

BACKGROUND: Neuromuscular choristomas (NMCs), are extremely rare developmental lesions that, have been previously established associated with recurrent fibromatosis after surgery, leading to several operations or even amputation. However, reports on the ultrasound imaging features and clinical conditions of NMCs are rare. The purpose of this study is to describe the ultrasound features and clinical analysis of NMCs to provide suggestions to identify the optimal management strategy. METHODS: From September 2020 to September 2021, 7 patients with a confirmed diagnosis of NMC who underwent ultra

Tarsal tunnel syndrome caused by posterior facet talocalcaneal coalition: A case report.

RATIONALE: Tarsal tunnel syndrome (TTS) is a compressive neuropathy of the posterior tibial nerve and its branches. Tarsal coalition is defined as a fibrous, cartilaginous, or osseous bridging of 2 or more tarsal bones. TTS with tarsal coalition is uncommon. Here, we present a rare example of successful surgical management of TTS with posterior facet talocalcaneal coalition. PATIENT CONCERNS: A 74-year-old woman presented with hypoesthesia, numbness, and an intermittent tingling sensation on the plantar area over the right forefoot to the middle foot area. The hypoesthesia and paresthesia of

The congenital clubfoot - immunohistological analysis of the extracellular matrix.

PURPOSE: Congenital clubfoot is one of the most common limb disorders in humans and its etiology is still unclear. In order to better understand the pathogenesis of patients with primary clubfoot, we examined whether there are quantitative changes in the extracellular matrix (ECM; based on common interstitial collagens [C] like CI and CIII, microfilamentous collagens like CVI, noncollagenous proteins like undulin, and enzymes like matrixmetalloproteinase [MMP]-2 and tissue inhibitor of matrixmetalloproteinase [TIMP]-2 that are known to play a role in fibrogenesis and fibrolysis) of muscles inv

Plantar forces in flexor hallucis longus versus flexor digitorum longus transfer in adult acquired flatfoot deformity.

BACKGROUND: Flexor hallucis longus (FHL) and flexor digitorum longus (FDL) tendon transfers are frequently used to restore the function of a deficient tibialis posterior tendon in stage II adult acquired flatfoot deformity (AAFD). Either transfer causes some loss in toe flexion force, although the decision to tenodese the cut tendon to restore associated function remains controversial. This study quantified changes in plantar force before and after tendon transfer and with or without distal tenodesis in a cadaveric model. METHODS: The plantar force distribution of 10 matched pairs of statical

[The Bosworth ankle fracture (case report)].

The authors describe an atypical case of the so called Bosworth fracture of ankle. A woman, 26 years suffered a fracture of fibula in the proximal third of the shaft, the distal fibular fragment was displaced from the fibular insicure of distal tibia posteromedially where it remained locked behind posterior tibial tubercle, and a fracture of medial malleolus and fracture of the posterolateral fragment of distal tibia. Thus it was a fracture-dislocation of ankle of Weber C type. The primary examination showed an evident external rotation deformity of the foot with regard to tibia. Conservative

All-suture anchor-enhanced capsulorrhaphy versus simple longitudinal capsulorrhaphy combined with Chevron-Akin osteotomy in the treatment of moderate-to-severe hallux valgus: a retrospective cohort study.

BACKGROUND: Hallux valgus (HV) is a common forefoot deformity. The ideal surgical technique for treating moderate-to-severe hallux valgus (HV) is unclear. Surgical methods that effectively restore the balance of soft tissues around the first metatarsophalangeal (MTP1) joint are necessary. This study compared osteotomy combined with either all-suture anchor-enhanced capsulorrhaphy (AEC) or simple longitudinal capsulorrhaphy (SLC). METHODS: Due to the different surgical methods from October 2018 to November 2024, ninety-five patients with moderate-to-severe HV were assigned into the AEC group o

Minimally Invasive vs Open Lapidus Bunionectomy: 1-Year Patient-Reported Outcomes.

BACKGROUND: Hallux valgus is a common and potentially debilitating foot deformity, and the modified Lapidus is a prominent corrective procedure in the United States. The clinical distinctions between the minimally invasive surgery (MIS) and the established open techniques for modified Lapidus have not yet been well-described, particularly in the context of patient-reported outcomes. This study aims to systematically compare patient-reported outcomes between the 2 surgical approaches. METHODS: One hundred forty moderate to severe hallux valgus patients who received modified Lapidus (66 MIS, 74

Novel classification of tibial hemimelia and reconstruction-guiding principles.

BACKGROUND: Tibial hemimelia is an extremely rare congenital limb deficiency characterized by variable tibial absence, knee instability, and ankle deformity. Existing classification systems rely primarily on static radiographic morphology and provide limited guidance for surgical decision-making, particularly regarding limb reconstruction versus amputation. Determinants of limb salvage especially the integrity of the quadriceps extensor mechanism and knee stability are not systematically incorporated into current classifications. This study proposes a novel classification designed to integrate

Glaucoma and degenerative vitreoretinopathy in a girl with Nicolaides-Baraitser syndrome.

We report the case of a 12-year-old girl diagnosed with Nicolaides-Baraitser syndrome with novel ocular features. Diagnosis was based on clinical features, including developmental delay, sparse hair, and craniofacial features along with de novo mutation in SMARCA2. Eye findings included bilateral glaucoma, cataracts, and degenerative vitreoretinopathy. Given the absence of an associated recognizable disorder and the low prevalence of these ocular findings in the general population, we suggest that these ocular features may not be chance association.

Perioperative posterior heel pain caused by multiple etiologies including a neuroma in continuity of the posterior branch of the sural nerve: a case report.

Posterior heel pain after a prior Haglund's deformity surgical correction can be resultant to multiple etiologies: osseous, tendinous, and neural. In this case report, all three potential etiologies were found to be contributing to the postoperative status of the patient. This case report illustrates identification and treatment of a neuroma in continuity of the posterior branch of the sural nerve with preservation of the sural nerve itself via microdissection, which we believe has not been described previously in the literature.
