[The role of MRI in dislocations of the patella and other knee pathologies].

In addition to ultrasound and conventional x-ray, magnetic resonance imaging (MRI) plays an important role in the visualization of lesions of the knee. It allows the orthopaedic surgeon to safely detect ruptures of the cruciate ligaments, the meniscus, osteochondral lesions and other ligamentary structures such as the collateral ligaments and the medial patellofemoral ligament (MPFL). In patellar dislocations, risk factors for recurrent dislocations can be determined and the therapy can be adjusted accordingly. This articles aims to give the reader a comprehensive overview on current concepts a

Open anterior-inferior hip dislocation.

Traumatic hip dislocations result from high-energy trauma. These dislocations are usually posterior in direction and have severe associated injuries. The less common anterior dislocation is usually of the inferior type. We report a case of an open anterior-inferior hip dislocation secondary to a high-speed motor vehicle collision. The wound was in the medial upper ipsilateral thigh. To our knowledge, this is the first reported case of an adult with an open inferior-type anterior hip dislocation.

Spontaneous fusion of 'os acetabuli' after triple pelvic osteotomy.

In acetabular dysplasia, an overloading of the acetabular rim can cause a stress fracture, creating an 'os acetabuli', or a lesion of the acetabular labrum. At puberty, the os acetabuli seems to be the epiphysis of the os pubis. We present the case of a 14-year-old girl with acetabular dysplasia and spontaneous fusion of an 'os acetabuli' after biomechanical correction by triple pelvic osteotomy. Our report supports the correctness of the biomechanical principle: reorientation of the acetabulum results in a better coverage of the femoral head, reduces the stress at the acetabular rim, shifts t

Lateral distraction injury to the lumbar spine. Report of spontaneous reduction.

STUDY DESIGN: A patient with a lateral distraction injury of the lumbar spine that reduced spontaneously and not associated with any thoracic or abdominal injury is reported. SUMMARY OF BACKGROUND DATA: A brief summary of the clinical presentation is given as is the surgical technique employed. Lateral distraction injuries of the thoracolumbar spine associate a distraction injury with lateral bending in the frontal plane, causing unilateral disruption of the ligamentous and osseous restraints. The have been associated with life-threatening thoracic and abdominal injuries. Closed reduction of

Common hand and wrist injuries in basketball players.

Multiple injuries to the hand are relatively common in basketball. This article explains the importance of an early and thorough examination, preferably by a hand specialist. It is important to obtain a history of prior injury during the assessment because treatment and prognosis may be very different. Delayed treatment often yields an inferior result and lost time. If an injury is not treated initially, it may require operative treatment or even be untreatable.

[Experimental carpal displacement induced by bony lesions].

Bony lesions were created experimentally in 48 fresh cadaver wrists. They consisted of radial and scaphoid resections along the extent of the proximal carpal row, representative of clinically-induced (usually traumatic) lesions. The displacements obtained experimentally to mirror each clinical situation modify the physiologic equilibrium established between the trans-osseous forces and the ligaments. The comparison between the experimentally-induced lesions and clinical displacements shows the etiology of the clinical conditions which indicates the proper treatment.

[Fractures and luxations of the mid- and forefoot].

Fractures of the mid- and forefoot, as well as luxations of the Chopart and Lisfranc joints, have in the past often been treated conservatively if not with therapeutic nihilism. In light of the modern operative techniques and claims regarding anatomic and functional restitution, this management from should be reconsidered. Shortening and malposition can be corrected by internal fixation and joint surfaces are being reconstructed, thus restoring the load-bearing capacity of the plantar arch. Post-traumatic impairment can be completely or at least partially avoided by early specific measures. Th

Irreducible dorsal dislocation of the interphalangeal joint of the great toe: report of two cases.

In this paper, we report two cases of dislocation of the hallux interphalangeal joint with interposition of the plantar plate including the sesamoid bone. These dislocations were treated with open reduction: one through lateral approach and the other through plantar approach. Plantar approach afforded repair of the plantar plate with preservation of the sesamoid bone and satisfactory result.

Dorsal dislocation of the trapezoid.

A case of dorsal dislocation of the trapezoid associated with dorsal subluxation of the carpometacarpal joints of the index and long fingers is described. The initial trapezoid subluxation was overlooked and it progressed quickly to a complete dislocation. Open reduction and capsular repair without internal fixation was followed by early recurrent trapezoid subluxation. Repeat open reduction combined with a limited intercarpal arthrodesis was used as a salvage procedure and produced a good result.

Lateral displacement of the intact mandibular condyle. Review of literature and report of case with associated facial nerve palsy.

A rare injury is described, in which symphyseal fracture of the mandible was associated with lateral displacement of the intact mandibular condyle into the temporal fossa and damage to the ipsilateral facial nerve with unilateral facial paralysis. Management of the injury was associated with an episode of acute laryngeal obstruction, necessitating emergency tracheostomy.

Diagnostic imaging of trauma to the ankle and foot. Part V: Midfoot injuries.

The authors have provided a series of dialogues dealing with the subject of trauma to the foot and ankle. Previous information was presented on ankle fractures, ligament injuries, talar and calcaneal maladies. The present discussion reviews pathology of the midfoot region.

Subtrochanteric osteotomy with intramedullary fixation for arthroplasty of the dysplastic hip. A case report.

Hip dysplasia in its end stage may require arthroplasty, and often extreme femoral anteversion is present. To correct rotational alignment of knee and hip joints yet preserve part of the femoral neck during total hip arthroplasty, a subtrochanteric derotational osteotomy was performed in a 37-year-old woman. Intramedullary fixation of the osteotomy was secured using a custom-fitted femoral component alone.

[Acute management of spinal injuries. The thoracic and lumbar spine, conservative and functional therapy].

Fractures of the thoracic spine are commonly treated functionally. In the thoracolumbar and lumbar region compression fractures with angulation can be treated by closed reduction and casting according to the Boehler method. Yet, stable fractures with minor deformities may be treated with advantage by early weight bearing. Primarily unstable thoracic and lumbar vertebra fractures gradually become stable with increasing fracture healing and may also be treated functionally but with prolonged bed rest of 6-12 weeks. Thus open reduction and internal fixation achieve better anatomical alignment and
