Efficacy of two reduction methods in conjunction with 3-D-printed patient-specific pin guides for aligning simulated comminuted tibial fractures in cadaveric dogs.

OBJECTIVE: To assess the feasibility and accuracy of using 2 methods for reduction and alignment of simulated comminuted diaphyseal tibial fractures in conjunction with 3-D-printed patient-specific pin guides. SAMPLE: Paired pelvic limbs from 8 skeletally mature dogs weighing 20 to 35 kg. METHODS: CT images of both tibiae were obtained, and 3-D reconstructions of the tibiae were used to create proximal and distal patient-specific pin guides. These guides were printed and used to facilitate fracture reduction and alignment in conjunction with either a 3-D-printed reduction guide or a linear t

Therapeutic effect of an external fixator in the treatment of unstable pelvic fractures in children.

BACKGROUND: Unstable pelvic fractures in children are serious and complex injuries, and the optimal method to manage these injuries is controversial. HYPOTHESIS: We hypothesized that an external fixator would be a satisfactory method of treating unstable pelvic fractures in children. PATIENTS AND METHODS: We retrospectively reviewed 40 pediatric patients with unstable pelvic fractures who were treated with an external fixator in Beijing Children's Hospital from June 2006 to June 2016. Postoperative follow-up was 12 to 108 months, with an average of 26 months. One year after the operation, fr

Prediction of Conservative Treatment Failure for Isolated Unilateral Mandibular Condylar Fractures Using Quantitative Measures: Suggestion of Indications for Intervention.

BACKGROUND: Interest in the treatment of mandibular condyle fracture in cases of maxillofacial trauma has been a huge concern for decades because of a diversity of opinions on the topic and the relatively few studies conducted on individuals treated by physiotherapy or rehabilitation with or without intermaxillary fixation (IMF). METHODS: We measured fracture gaps between bone ends over an 8-year period from 2012 to 2019. The study cohort was composed of only conservatively treated patients. This retrospective study was conducted to identify factors that influence treatment failure by compari

[Injuries of the lower cervical vertebrae--the monocortical stabilization technique].

PURPOSE OF THE STUDY: In contrast to the thoracolumbal spine, the cervical spine bears a lower biomechanical load and, therefore, anterior stabilization of a fracture is a definitive procedure in the majority of cases. What remains the matter of choice is screw fixation in the body of the vertebra involved. This may be either monocortical or bicortical. In this study, we evaluate a group of patients in whom fractures of the lower cervical spine were treated using the CSLP monocortical system (Synthes). MATERIAL: We included 68 patients in whom complete radiographic data were available and the

[Intramedullary osteosynthesis of distal metacarpal fractures with curved wires].

When intramedullary pinning is used to treat metacarpal fractures, as recently described by Förstner (1994) and Foucher (1995), the closed reduction technique developed by Jahss (1938) is applied in the same way as for conservative fracture treatment. It is not always possible to achieve complete anatomical reduction using this closed technique. The intramedullary pinning technique, that we have applied since 1989, involves a Kirschner wire which is bent at one end. Apart from reducing the fracture, the pre-set Kirschner wire serves as a butressing internal fixator. The elastic clamping of the

Contoured Harrington instrumentation in the treatment of unstable spinal fractures. The effect of supplementary sublaminar wires.

Twenty-nine patients with major fractures and fracture-dislocations of the thoracic and lumbar spine were treated by spinal fusion and contoured Harrington distraction instrumentation. Two groups of patients were identified. Group I (16 patients) had sublaminar wires. Group II (13 patients) had no wiring. Satisfactory alignment was achieved in all patients. There were several complications in both groups. None of the patients in Group I had postoperative cast immobilization. The addition of sublaminar wiring appears to eliminate the need for rigid external immobilization and is beneficial only

Neglected acetabular fracture with hip dislocation in a 34-year-old subject treated with total hip arthroplasty: our experience.

Neglected hip fracture-dislocations are rare but still possible clinical situations, especially in developing countries. Some authors described skeletal traction and open reduction with internal fixation as a treatment for the abovementioned conditions. Despite the poor literature about this topic, total hip arthroplasty (THA) may be a feasible option in the treatment of neglected hip fracture-dislocations. We present a case of a 34-year-old Moroccan man reporting a 1-year neglected acetabular fracture with hip dislocation successfully treated with THA. The patient showed a significant improve

Management of hangman's fractures and a subaxial compression fracture in two children with osteogenesis imperfecta.

Cervical spine fractures in osteogenesis imperfecta are rare. The purpose of this article is to describe the successful outcomes of the nonoperative and operative treatment of C2 pars fracture and operative treatment of C7 compression fracture in two children with osteogenesis imperfecta. Patient 1, a 22-month-old female, had a C2 pars fracture managed nonoperatively with a cervical orthosis. Patient 2, a 15-year-old male, had concurrent C2 pars and C7 compression fractures; the C2 pars fractures were treated operatively via a posterior approach and open reduction and internal fixation, and th

Allograft bone transplantation for neuropathic arthropathy of the knee associated with type 1 diabetes mellitus.

Diabetes mellitus causes neuromusculoskeletal disorders characterized by abnormalities of nervous tissue, joint and bone. Early diagnosis and prevention of disease progression is difficult in cases of neuropathic arthropathy of the knee in diabetes. We report the case of a patient with type 1 diabetes mellitus who developed an insufficiency fracture of the medial part of the proximal tibia, which was viewed as an early finding of neuropathic arthropathy of the knee. In surgical treatment of the fracture, allograft bone transplantation and internal fixation were performed after curettage of the

Isolated avulsion fracture of the extensor carpi radialis brevis insertion due to a boxer's injury.

Avulsion fractures involving the radial wrist extensor tendons are extremely rare. Only 8 avulsion fractures of the extensor carpi radialis longus and 7 avulsion fractures of the extensor carpi radialis brevis have been previously reported. We know of no case involving an isolated avulsion fracture of the base of the third metacarpal where the extensor carpi radialis brevis attachment to the fracture fragment was still completely intact. This article reports a patient with an isolated avulsion fracture of the extensor carpi radialis brevis insertion treated by open reduction and internal fixat

Acute tibial tubercle avulsion fractures in the sporting adolescent.

INTRODUCTION: The authors report 7 cases of acute tibial tubercle avulsion fractures. The fracture occurred in 6 out of the 7, after an abrupt tension of the patellar tendon in male sporting adolescents (age 13-17 years). Two patients presented symptoms of homolateral Osgood-Schlatter's disease before the lesion. METHOD: According to Ogden's classification, the tibial tubercle avulsion fracture was not displaced in 3 cases (stage IA) and was treated conservatively by immobilization for 6 weeks. In 4 cases, the fracture was displaced and necessitated an internal fixation with plaster for about

Fracture of the posterior process of the talus associated with subtalar dislocation: report of a case.

Although subtalar dislocation is not common in foot trauma, fracture of the entire posterior process of the talus is even rarer. The concomitant injury of a subtalar dislocation associated with a fracture of the posterior process of the talus has not yet been reported in the medical literature. A 52-year-old woman involved in a traffic accident had her right foot twisted into an inversion deformity. An X-ray revealed a medial subtalar dislocation associated with a fracture of the entire posterior process of the talus. Tomography demonstrated that the posterior process was incongruent to the an

Volar fixation for dorsally displaced fractures of the distal radius: a preliminary report.

Using a volar approach to avoid the soft tissue problems associated with dorsal plating, we treated a consecutive series of 29 patients with 31 dorsally displaced, unstable distal radial fractures with a new fixed-angle internal fixation device. At a minimal follow-up time of 12 months the fractures had healed with highly satisfactory radiographic and functional results. The final volar tilt averaged 5 degrees; radial inclination, 21 degrees; radial shortening, 1 mm; and articular incongruity, 0 mm. Wrist motion at final follow-up examination averaged 59 degrees extension, 57 degrees flexion,

The impact of pedicle screw transitional segment and thread distribution on postoperative rod-screw system failure : insights from clinical research and mechanical testing.

AIMS: This study aimed to evaluate the differences in postoperative internal fixation system failure between two distinct pedicle screw designs, and to examine how variations in the transitional segment and thread distribution of these screws influence the bending mechanical properties of the rod-screw system. METHODS: A retrospective analysis was performed on 81 cases of thoracolumbar posterior internal fixation surgery from January 2020 to December 2023, focusing on comparing the postoperative fracture and loosening rates associated with the two pedicle screw designs. Finite element analysi

Surgical treatment of monostotic fibrous dysplasia of the proximal femur in children and adolescents: Observational European Paediatric Orthopaedic Society multicenter study.

PURPOSE: Monostotic fibrous dysplasia is a rare benign fibro-osseous disorder. Proximal femoral monostotic fibrous dysplasia is especially vulnerable to pathological fracture and deformation, requiring specific treatment strategies. Literature on pediatric proximal femoral monostotic fibrous dysplasias is sparse and without consensus. We present the largest observational cohort study on various treatment methods of pediatric proximal femoral monostotic fibrous dysplasia. METHODS: Pediatric patients with proximal femoral monostotic fibrous dysplasia were included, from 10 academic hospitals fo

Shoulder arthroscopy combined to hardware removal in proximal humeral fractures: a series of 58 cases with a mean follow-up of 2 years.

BACKGROUND: Proximal humerus fractures are common injuries. Locking plates and anterograde medullary nails are the two most common fixation devices used when open reduction and internal fixation (ORIF) is indicated. Complications related to fracture and to hardware are numerous, especially shoulder stiffness. The goal of this study is to report the clinical outcomes of gleno-humeral arthroscopic arthrolysis combined with hardware removal. METHODS: A total of 58 patients (25 men, 33 women) with a mean age of 58 years (24-79) were reviewed retrospectively. Forty of them were active workers (5 h
