Predictors of chronic pain and disability in patients treated conservatively after distal radius fracture: a prospective cohort study.

PURPOSE: To determine predictors of chronic pain and disability among patients with distal radius fractures (DRF) treated conservatively with closed reduction and cast immobilization. METHODS: This was a prospective cohort study. Information on patient characteristics, post-reduction radiographic parameters, finger and wrist range of motion, psychological status (Hospital Anxiety and Depression Scale or HADS), pain (Numeric Rating Scale or NRS), and self-perceived disability (Disabilities of the Arm, Should, and Hand or DASH) were taken at baseline, cast removal, and 24 weeks. Differences in

The influence of the COVID-19 pandemic on traumatic brain injuries in Tyrol: experiences from a state under lockdown.

PURPOSE: In February 2020, the federal state of Tyrol in Austria has become one of the epicenters of the COVID-19 pandemic. Tyrol is known for numerous skiing areas. Thus, winter sport resorts became a starting point for COVID-19 infections spreading towards the rest of the state, Austria and other countries, leading to a mandatory quarantine for almost a million people, who were placed under a curfew and restrictions in daily life. Additionally, all ski resorts and hotels were closed. We aimed to analyze the influence of the COVID-19 quarantine on traumatic brain injury (TBI) cases in Tyrol.

Displacement Patterns of Blunt Rib Fractures and Their Relationship to Thoracic Coinjuries: Minimal Displacements Count.

Displacement patterns of rib fractures (RF) and their association with thoracic coinjuries and outcomes are unknown. This is a retrospective review of adult patients with blunt closed RF who underwent chest CT at a Level I trauma center (2007-2012). Displacement patterns of RF were compared among the three-dimensional planes using CT images. An analysis of receiver operating characteristic (ROC) curves was performed to identify displacements in each plane most strongly associated with chest coinjuries. Univariate analysis was used to find association of displaced RF with hospital course and ou

Acute compartment syndrome.

BACKGROUND: acute compartment syndrome (ACS) is one of the few true emergencies in orthopedics and traumatology. It is a painful condition caused by the increase interstitial pressure (intracompart-mental pressure - ICP) within a closed osteofascial compartment which impair local circulation. It occurs most often in the legs, but it can affects also the arms, hands, feet, and buttocks. It usually develops after a severe injury such as fractures or crush injury, but it can also occurs after a relatively minor injury and it may be iatrogenic. Uncommon causes of ACS have been also described, that

Unusual delayed presentation of post-traumatic intrapericardial hernia associated with intestinal occlusion.

AIM: We report a case of a 64-year-old man, admitted to our department following the onset a few months earlier of canalization disorders and a sensation of retrosternal tension. MATERIAL OF STUDY: Patient's history revealed blunt thoraco-abdominal trauma with multiple costal fractures 15 years earlier as a result of a road accident and a cholecystectomy at the age of 57. A barium meal revealed an intrapericardial displacement of some intestinal loops; as the patient suffered acute intestinal occlusion with severe abdominal pain associated with nausea and vomiting, we performed an emergency m

Lateral closed wedge osteotomy for cubitus varus deformity.

BACKGROUND: Lateral closed wedge (LCW) osteotomy is a commonly accepted method for the correction of the cubitus varus deformity. The fixation of osteotomy is required to prevent loss of correction achieved. The fixation of the osteotomy by the two screw and figure of eight wire is not stable enough to maintain the correction achieved during surgery. In this prospective study we supplemented the fixation by Kirschner's (K-) wires for stable fixation and evaluated the results. MATERIALS AND METHODS: Twenty-one cases of the cubitus varus deformity following supracondylar fractures of the humeru

Trauma and wound healing: role of the route of nutritional support.

Following severe trauma and surgical injury, nutritional support via the enteral route has been shown to lead to increased survival and decreased complications when compared to the parenteral route. We hypothesized that the route of nutrient delivery may affect cutaneous wound healing following severe traumatic insult. Forty-six Sprague Dawley rats underwent bilateral closed femoral fractures, central venous catheterization, gastrostomy placement and dorsal skin incision with placement of polyvinyl alcohol sponges into subcutaneous pockets. Identical nutritional infusates of 25% dextrose, 4.25

Hip pain in athletes.

Hip pain in athletes involves a wide differential diagnosis. Adolescents and young adults are at particular risk for various apophyseal and epiphyseal injuries due to lack of ossification of these cartilaginous growth plates. Older athletes are more likely to present with tendinitis in these areas because their growth plates have closed. Several bursae in the hip area are prone to inflammation. The trochanteric bursa is the most commonly injured, and the lesion is easily identified by palpation of the area. Iliotibial band syndrome presents with similar lateral hip pain and may be identified b

Use of a delayed cortical bone graft to treat diaphyseal defects in the forearm.

The technique of delayed autogenous cortical bone grafting was used in 17 patients (6 women, 11 men, with an average age of 22 years) to treat diaphyseal defects resulting mainly from closed or compound fractures complicated by infection and bone tissue loss. Bones affected were the humerus in 1 case, the radius in 7 cases, the ulna in 4 cases, the radius and ulna in 2 cases, the first metacarpal in 1 case, and the femur in 2 cases. The average length of the defect was 5.7 cm and the graft, prepared from the anteromedial aspec of the tibia, was at least 1.5 cm longer than the defect. The graft

[Posterior shoulder joint instability. Classification, pathomechanism, diagnosis, conservative and surgical management].

The posterior instability of the shoulder is a more difficult diagnostic and therapeutic challenge than the anterior instability. There are many etiologies and causes of posterior instability. Most studies in the literature are retrospective and yield a great variation in therapeutic recommendations. Generally it has to be separated in traumatic and atraumatic instabilities. Most of the traumatic dislocations are impaction fractures of the humeral head against the dorsal glenoid. Therapy is depending on the size of the humeral defect, the duration of dislocation and the functional demand of th

Gas gangrene in traumatic hand injuries. Report of three cases.

Gaseous gangrene is an infection with anaerobies germs characterized by gaseous infiltration of soft tissues with quick and serious damage to the general state. Known in war time, it rages also in civilian practice. Two etiologies are dominating: post-trauma gaseous gangrenes, post-surgery gaseous gangrenes. Among post-trauma gaseous gangrenes, the ones on departure of inferior members are the most frequent arising from fractures or from luxations open or closed. Localisation on superior limbs is scarce, specially after soft parts' trauma. We report three cases of superior limbs' gaseous gangr

Prophylactic antibiotics in trauma.

Clinical and laboratory experience has shown that prophylactic antibiotics may be effective in trauma when the period of contamination is brief, defined and predictable. Prevention of infections has been achieved in the treatment of penetrating abdominal wounds, open fractures, and early wounds that can be adequately debrided and closed. Prophylactic antibiotics do not prevent infection in wounds left open, in wounds that are incompletely debrided or treated very late, following tracheal intubation, after urinary tract catheterization, or after vascular cannulation. Continuing contamination is

[Closed extensor tendon injuries of the finger].

The extensor apparatus of the fingers is a complex structure. The diagram of the extensor apparatus as seen in figure 1 is in reality not so clearly structured. Closed lesions are the mallet finger, where either an avulsion of the bony insertion of the tendon occurs or where a fracture of the base of the distal phalanx with dislocation of a bone-fragment together with the tendon insertion has happened. Other closed lesions are the closed Boutonnière deformity and the avulsion of a lumbrical muscle. The mallet finger is mostly treated conservatively with a prefabricated splint holding the DIP-j

Reliability and reproducibility of the STAB classification compared with Neer and AO classifications for proximal humerus fracture-dislocations: a retrospective cohort study of 57 patients.

BACKGROUND: Proximal humerus fracture-dislocations represent a challenging injury pattern with high complication rates. Current classification systems, including Neer and AO, have limitations in guiding treatment decisions for Neer type VI injuries. This study introduces and validates the STAB classification system, comparing its reliability and reproducibility with established classifications. METHODS: A retrospective cohort study of 57 patients with Neer type VI proximal humerus fracture-dislocations treated between January 2018 and December 2024 was conducted. Four observers with different

Redisplacement of reduced distal radius fractures in adults: does the type of casting play a role? The CAST study, a multicentre cluster randomized controlled trial.

AIMS: It is not clear which type of casting provides the best initial treatment in adults with a distal radial fracture. Given that between 32% and 64% of adequately reduced fractures redisplace during immobilization in a cast, preventing redisplacement and a disabling malunion or secondary surgery is an aim of treatment. In this study, we investigated whether circumferential casting leads to fewer the redisplacement of fewer fractures and better one-year outcomes compared with plaster splinting. METHODS: In a pragmatic, open-label, multicentre, two-period cluster-randomized superiority trial

Impaired Consciousness Following Cerebrospinal Fluid Leakage After Halo Vest Fixation for Odontoid Fracture: A Case Report.

CASE: An 82-year-old woman sustained an Anderson Type II odontoid fracture after a fall and underwent halo vest fixation. Eight weeks later, she developed eyelid edema and altered consciousness. Computed tomography revealed intracranial pin penetration, cerebrospinal fluid (CSF) leakage, and pneumocephalus. The halo vest was removed, and subsequent conservative treatment with bed rest and antibiotics led to full neurological recovery. CONCLUSION: This rare case of intracranial pin penetration and CSF leakage following halo vest fixation emphasizes the importance of close monitoring and early
