Aerobic minutes and step number remain low in inpatient stroke rehabilitation.

OBJECTIVE: Rehabilitation is important for regaining mobility poststroke. Clinical practice guidelines suggest a high number of repetitive stepping activities to optimize subacute recovery especially when undertaken at intensities that challenge cardiovascular fitness. However, adherence to these guidelines is unclear. The objective of this study was to quantify aerobic minutes and step number in usual care inpatient stroke rehabilitation unit physical therapy sessions across Canada and identify characteristics of participants who met guideline aerobic intensity minutes at a session midpoint i

Clinical guideline on the open arthrolysis for post-traumatic elbow stiffness in adult patients.

BACKGROUND: Post-traumatic elbow stiffness poses significant clinical challenges for upper limb surgeons and severely impairs patients' ability to perform essential daily activities, including eating, dressing, and personal hygiene, thereby imposing substantial socioeconomic burdens. Open arthrolysis is widely employed when conservative therapies fail, yet current literatures demonstrate considerable heterogeneity in treatment concepts, surgical techniques, and perioperative management. This evidence-based clinical practice guideline aims to standardize treatment profiles and improve surgical

Decreased fetal movements - An audit of predictors and an evaluation of management based on a locally developed flow chart.

OBJECTIVES: Decreased fetal movements (DFM) is associated with adverse pregnancy outcomes. We aimed to look at the risk factors associated with DFM and outcomes of women who presented with and without DFM and outcomes before and after the implementation of a locally developed flow chart based on an evidence-based guideline. STUDY DESIGN: This was a retrospective audit of 1165 women ≥ 28 weeks' gestation with a singleton pregnancy who presented with concerns regarding DFM. We compared labor and neonatal outcomes to 4706 in a control group who did not present with concerns regarding FM. We also

Nonepithelial ovarian cancer - the current clinical practice in the Nordic countries. Survey from the surgical subcommittee of the Nordic society of gynecological oncology (NSGO).

BACKGROUND: Nonepithelial ovarian cancer (NEOC) represents a wide variety of rare tumors. They are often diagnosed at an early stage and have a good prognosis compared to epithelial ovarian cancer. In the Nordic countries, the total annual number of patients diagnosed with ovarian cancer, Fallopian tube cancer or primary peritoneal carcinoma (hereafter ovarian cancer) was 2281 in 2014-2018, of which 3-10% were NEOC. International guidelines for diagnosis, treatment and follow-up have been developed. We present the results of a survey, aiming at clarifying current clinical practice in the Nordi

The Burden of Analgesics Overprescription for Chronic Headache: Impacts on Patients, Healthcare System and Countries.

Chronic headache (CH) is a leading cause of debilitating disorders, with many headache types remaining undiagnosed due to improper medication use. This misuse not only worsens the underlying condition but also leads to a secondary problem: medication overuse headache (MOH). This creates a cycle of increasingly complex headache cases, posing significant challenges for patients, healthcare providers, and the healthcare system as a whole. Physicians often contribute to this issue by prescribing analgesics without proper assessment or adequately educating patients on their correct use, thereby inc

COPD and the burden of multimorbidity: navigating the complexity.

INTRODUCTION: Chronic obstructive pulmonary disease (COPD) is a chronic condition that affects millions of people worldwide. The majority of patients with COPD have multiple coexisting chronic diseases, such as cardiovascular diseases, osteoporosis, lung cancer, and metabolic syndrome, a phenomenon that is known as multimorbidity. The coexistence of these diseases with COPD complicates diagnosis, treatment, and prognosis. AREAS COVERED: This review explores the underlining mechanisms that connect COPD and multimorbidity, such as shared risk factors and pathophysiological pathways. It also hig

Why and when are activity scores and advanced ultrasound techniques needed in inflammatory bowel disease (IBD)?

In addition to avoidance of radiation exposure, the main advantages of ultrasound diagnostics in inflammatory bowel disease (IBD) patients are its' immediate availability, also as a component of the physical examination (extended point of care), its' repeatability and its' high patient comfort. The fact that the examination is carried out directly by the treating physician and is not delegated to the radiologist increases compliance and therapy adherence of the patients. In the current series of papers a synopsis of recommendations for ultrasound in IBD guidelines, ultrasound parameters in infl

Angiotensin-converting enzyme inhibitors, statins and the polypill in cardiovascular diseases prevention: ignorance is bliss or not?

The polypill strategy, which combines several medicines that simultaneously control different risk factors/diseases in a single pill, is one of the approaches used in cardiovascular therapy. In different guidelines, this one-pill combination therapy is suggested as first-line step in disease management. Because the cardiovascular diseases (CVD) pandemia, prevention is essential. The approaches that could improve adherence are of great importance to achieve health, social and economical benefits. However, direct or indirect experience of adverse drug reaction is often the reason for discontinua

Reducing the Risk: An Evidence-Based Approach to the Febrile Infant Less Than 60 Days of Age in the Emergency Department.

Fever is the most common complaint for children of all ages who present to an emergency department (ED) for evaluation and treatment. Although most children with fever have a self-limiting viral illness, the infant less than 60 days of age with fever poses a higher risk for an underlying serious bacterial illness (SBI) including urinary tract infections, bacteremia and bacterial meningitis, or an invasive bacterial infection (IBI) including bacteremia. This case is a discussion of a high-risk febrile infant that was evaluated in the emergency department and was ultimately diagnosed with Strept

Improving access to gut-directed hypnotherapy for irritable bowel syndrome in the digital therapeutics' era: Are mobile applications a "smart" solution?

Gut-directed hypnotherapy (GDH) is a highly effective brain-gut behavioral therapy which is recommended in international guidelines for the treatment of irritable bowel syndrome (IBS). There is increasing recognition of the value of GDH as part of integrated care alongside medical and dietary approaches. This has led to recent innovations to widen access to GDH to meet the increasing demand. Recent advances include streamlined courses of individualized GDH, group therapy, and remote delivery. In this issue of Neurogastroenterology and Motility, Peters et al. retrospectively report outcomes of

The optimization of peritoneal dialysis training in long-term.

Peritoneal dialysis is a home based therapy for patients with advanced chronic kidney disease. This method provides adequate clearance of uremic toxins and removal of excess fluid when a proper dialysis prescription is combined with patient adherence. Peritonitis is the most frequent infectious complication among these patients and may render the continuity of the treatment. Training patients and their caregivers have prime importance to provide proper treatment and prevent complications including infectious ones. The training methods before the onset of treatment are relatively well establish
