Disruption of local cortical blood flow (CBF) autoregulation and CO2 reactivity, or vasoparalysis, has been documented in humans after aneurysmal subarachnoid hemorrhage (SAH). Generally, the degree of vasoparalysis is related to the patient's clinical grade. The therapeutic efficacy of oral acetazolamide has been evaluated in a small pilot study of 14 patients with Menière's disease (23 hydropic ears). Symptomatic improvement was found in 4 cases. In 2 of these patients the improvement was not sustained, whilst another had to stop the drug due to the development of bilateral renal calculi. A deterioration in symptoms was seen in 3 cases. The parameters of the general and cerebral hemodynamics were studied in 36 patients with chronic insufficiency of the cerebral circulation following papaverine administration. Of a group of 79 patients (45 males, 34 females) with myelomeningocele (MMC), 52 had associated hydrocephalus. Facial trauma often results in minor and infrequently results in major bleeding in the structures of the face. We have recently observed two patients who suffered fatal hemorrhages which could have been controlled using relatively simple measures. Treating physicians often overlook this serious and potentially life-threatening source of hemorrhage until the patient has been in shock for long periods of time and irreversible ischemic brain damage and renal failure have occurred. The etiology to the empty sella syndrome (ESS) is not known. Increased intracranial pressure (ICP) has been suggested to be one of the possible causes. In the present study the CSF circulation was analyzed in 48 subjects with ESS with gamma cisternography, pneumoencephalography (PEG) and computed tomography (CT). The cases of six patients with fronto-ethmoidal osteomas which have produced intracranial complications are reported. The patients have been divided in two groups depending on their clinical aspects, from cranio-facial deformities to exophtalmus, rinorrhea, pneumocephalus, meningitis and mucocele. A review of the literature is made and the importance of such lesions is stressed together with a critical analysis of the methods proposed for their surgical treatment. An interesting case of systemic lupus erythematosus (SLE) is presented in which the clinical onset of myelopathy strongly suggested demyelinating disease. Later, intracranial hypertension developed on two occasions. Postmortem examination showed the systemic lesions of SLE. In addition, demyelinating lesions were found in the spinal cord and the optic nerve, the neuropathologic picutre of Devic's syndrome, a variant of multiple sclerosis. We question whether some of the cases showing necrotic myelopathy might have an autoimmune pathogenesis.
