Resection of about 50% of the small intestine was performed in rats diabetised with streptozotocin. The first results show that this operation leads to normalisation of sugar metabolism that is partly referable to partial malabsorption, but above all to loss of weight. Current knowledge with regard to the relation between diabetes and obesity is discussed and an account is given of the modern surgical approaches to the treatment of major obesity. Jejuno-ileal bypass operations have been used for more than 20 years for the treatment of massive obesity. Of 101 patients originally operated on, the status of 98 is known. Given the mortality and reanastomosis rates, the operation must be considered an absolute failure in 28 percent of the patients. Visceral infiltration by mast cells (systemic mastocytosis) is an unusual manifestation of cutaneous mastocytosis. Symptomatic gastrointestinal disease occurs in approximately one-fourth of patients having systemic mastocytosis. An unusual gastrointestinal complication is malabsorption, described in sporadic case reports. A patient with gastric acid hypersecretion and malabsorption is described. Treatment using the histamine H2 antagonist, cimetidine, ameliorated the patient's gastrointestinal symptoms; however, the malabsorption was not resolved.

A 33-year-old man had a sudden onset of night blindness 3 1/2 years after intestinal bypass surgery for morbid obesity. Ocular examination disclosed a probable Bitot's spot and electrophysiologic testing showed markedly impaired dark adaptation and subnormal electroretinography. After therapy with orally administered vitamin A, the night blindness disappeared and the dark adaptation curve and electroretinogram returned to normal.

Twenty-eight patients, thought to have coeliac disease and on gluten free diets, were put on a normal diet to confirm their diagnoses. Eighty-one patients with dermatitis herpetiformis were treated with a gluten-free diet (GFD) for periods varying from 6 to 36 months. A case of severe "non-responsive" coeliac disease has been shown to be sensitive to gluten whilst under prednisone therapy, but not without prednisone. This adds an additional dimension to the criteria for diagnosis of coeliac disease. Cases of non-responsive coeliac disease may require both prednisone and gluten exclusion to induce a remission. In this patient, azathioprine induced and sustained a remission when unacceptably high doses of prednisone had failed, and may prove to be a valuable immunosuppressive in non-responsive coeliac disease.

The existence of tropical sprue in Africa is controversial. In this paper we present 31 cases seen in Rhodesia over a 15 month period. They have the clinical features, small intestinal morphology, malabsorption pattern, and treatment response of tropical sprue. A 39-year-old man with severe bullous pemphigoid developed overwhelming diarrhea after 5 weeks' treatment with 150 mg of cyclophosphamide and 60 mg of prednisolone daily.
