Provision of paediatric and adolescent gynaecology in Europe today: A joint review by the European Association of Paediatric and Adolescent Gynaecology (EURAPAG) and European Board and College of Obstetrics and Gynaecology (EBCOG).

Paediatric and Adolescent Gynaecology (PAG) is a multidisciplinary field combining aspects of gynaecology but also includes paediatrics, endocrinology, genetics, radiology, psychology and urology. Specialist knowledge is warranted for the care of these youngsters, and it is important that doctors attending to the gynaecological needs of children must understand that they are not just "little women". Their needs and accompanying clinical approaches required are very different from those of adults in this sensitive area, as is the spectrum of diseases and problems. A multidisciplinary collaborat

The treatment of bacterial vaginosis in pregnancy with clindamycin to reduce the risk of infection-related preterm birth: a response to the Danish Society of Obstetrics and Gynecology guideline group's clinical recommendations.

Preterm birth is the major cause of perinatal mortality and morbidity worldwide. Infection/inflammation is responsible for a significant percentage of preterm birth, particularly at early gestations. A recent clinical recommendation by a guidelines group of the Danish Society of Obstetrics and Gynecology advised against the use of clindamycin for the treatment of bacterial vaginosis in pregnancy to reduce the risk of spontaneous preterm birth based on lack of evidence of efficacy. We believe that the evidence for the use of clindamycin for this indication is robust and that this recommendation…

Association Between Pre-pregnancy Obesity and Adverse Obstetric Outcomes: A Retrospective Cohort Study at a Secondary Care Center in Tbilisi, Georgia.

Background Maternal obesity is an increasingly important public health concern and has been consistently associated with adverse pregnancy outcomes, including hypertensive disorders of pregnancy, cesarean delivery, fetal overgrowth, and preterm birth (PTB). Local institutional data remain valuable because patient characteristics and obstetric practice patterns may vary across healthcare settings. This study aimed to evaluate the unadjusted and adjusted associations between pre-pregnancy obesity and selected adverse obstetric outcomes - hypertensive disorders of pregnancy, postpartum hemorrhage

Prenatal Diagnosis and Obstetrical Challenges of a Massive Fetal Ascites Affecting One Fetus in a Dichorionic Diamniotic Twin Pregnancy: A Case Report.

Fetal ascites is a rare prenatal condition; it may result from a wide range of underlying etiologies, including structural anomalies, genetic disorders, congenital infections, and metabolic diseases. Despite considerable advances in prenatal investigations, etiology remains unidentified in some cases. Massive fetal ascites may also complicate obstetric management by distorting fetal biometric measurements and influencing delivery planning. We report the case of a 34-year-old woman with a spontaneously conceived dichorionic diamniotic twin pregnancy. At 24 weeks of gestation, massive fetal asci

A pilot study to implement artificial intelligence-enabled, point-of-care obstetric ultrasound for gestational age estimation in Zambia: An evaluation protocol.

BACKGROUND: Ultrasound is essential for accurate pregnancy dating, but its implementation in low- and middle-income countries is hindered by cost, infrastructure, and training barriers. The implementation of point-of-care ultrasound (POCUS) with artificial intelligence (AI) technology to accurately estimate gestational age can potentially address these barriers. We describe a protocol to evaluate the acceptability, feasibility, and fidelity of integrating AI-enabled POCUS for gestational age dating into routine antenatal care (ANC) in Zambia's Lusaka Province. METHODS: PIKABU (Piloting Integr

Living donor liver transplantation from a donor with definite obstetric antiphospholipid syndrome: a case report with perioperative risk mitigation strategy.

Antiphospholipid syndrome (APS) is an acquired autoimmune thrombophilia associated with venous, arterial, and microvascular thrombosis. Living donor liver transplantation (LDLT) from a donor with definite APS is exceptionally uncommon and raises a difficult balance between recipient benefit and donor safety. We report LDLT using a right lobe graft from a 47-year-old female spouse donor with recurrent spontaneous abortions and persistent lupus anticoagulant positivity documented on repeat testing more than 12 weeks apart, fulfilling criteria for definite obstetric APS. Anticardiolipin immunoglo

The Association Between Intimate Partner Violence and Prior-to-Pregnancy Fear of Childbirth Among Nulligravid Women: Implications for Preconception and Obstetric Care.

BACKGROUND/OBJECTIVES: Fear of childbirth may begin before pregnancy and may be shaped by earlier traumatic and relational experiences. This study examined the association between intimate partner violence (IPV) and prior-to-pregnancy fear of childbirth among non-pregnant women and considered its implications for preconception and obstetric care. METHODS: This descriptive, correlational, cross-sectional study was conducted online in Türkiye between 1 January and 31 May 2025 after approval from the Non-Interventional Scientific Research Ethics Committee of Istanbul Atlas University (23 Decembe

Combined Obstetric and Thrombotic Complications Are Associated with Features Suggestive of an Immune-Enriched Phenotype Among Women with Thrombotic Antiphospholipid Syndrome (APS).

Background/Objectives: To determine whether obstetric antiphospholipid syndrome (oAPS) among women with primary thrombotic APS (tAPS) defines a distinct immunologic phenotype or influences thrombotic characteristics and recurrence risk. Methods: This predefined subgroup analysis was conducted within a multicenter retrospective cohort study of 663 patients with persistent antiphospholipid antibody (aPL) positivity and objectively confirmed thrombosis. The present analysis included 295 women with primary tAPS and a history of pregnancy. Participants were classified as oAPS or non-oAPS according

Minimal clinically important difference and psychometric properties of the Obstetric Quality of Recovery-10 measure: a systematic review and network meta-analysis.

INTRODUCTION: The Obstetric Quality of Recovery-10 (ObsQoR-10) is a patient-reported outcome measure for evaluating inpatient postpartum recovery. However, the minimal clinically important difference for ObsQoR-10 remains unknown, limiting clinical and research interpretability. The primary aim of this study was to determine the minimal clinically important difference of ObsQoR-10 through systematic review and network meta-analysis of studies and to evaluate its psychometric properties. METHODS: We conducted a systematic review and network meta-analysis of studies and searched relevant databa

The Use of Transperineal and Endoanal Ultrasound in Diagnosis and Management of Obstetric Anal Sphincter Injuries (OASIs): A Narrative Review from a Gynecologic Perspective.

Obstetric anal sphincter injuries (OASIs) are serious complications of vaginal delivery and the most frequent cause of postpartum anal incontinence. Underdiagnosis during delivery persists due to limited availability of accurate imaging. Transperineal ultrasound (TPUS) and endoanal ultrasound (EAUS) enable structural assessment of the anal sphincter and have growing roles in triage, repair planning, and follow-up.The aim of the present review is to integrate current evidence on diagnostic value, clinical usefulness, and potential implementation of TPUS and EAUS in gynecologic practice.Ultrasou

A decision-tree model to evaluate the impacts of workforce investments aimed at strengthening emergency obstetric and neonatal care (EmONC) facilities in Burundi.

INTRODUCTION: Human resources for health (HRH) in low- and middle income countries (LMICs) are often allocated irrationally based on normative policies that ignore service delivery patterns and health outcomes, failing to account for costs and expected benefits. To help Burundi rationalise scarce resources and tackle health system inefficiencies, we developed four workforce investment scenarios aimed at strengthening emergency obstetric and neonatal care (EmONC) facilities and predicted the costs and benefits associated with these investment proposals over the 2025-2030 timeframe. METHODS: We

Magnesium Toxicity in an Obstetric Patient Due to Preeclampsia-Related Renal Dysfunction Despite Administration of a Standard Pritchard Regimen: A Case Report.

Magnesium toxicity could occur in the treatment of preeclampsia/eclampsia, especially when the patient has coexisting renal impairment. If undetected and promptly treated, hypermagnesemia can cause severe fetomaternal complications and death. We present the case of a 22-year-old pregnant woman at 32 weeks of gestation, who was referred to our hospital with eclampsia, treated with intramuscular magnesium sulfate following a standard Pritchard regimen. An emergency cesarean section was carried out under general anesthesia due to multiorgan dysfunction. Following the cesarean delivery, she was su
