Congenital adrenal hyperplasia associated with hyperphosphatemic rickets.

The authors describe a boy with precocious puberty due to adrenal hyperplasia associated with rickets, hypocalcemia, hyperphosphatemia, elevated PTH and alkaline phosphatase levels, and concentrations of 25-OH-D and 1,25-(OH)2D at the upper limit or above normal range. Treatment with hydrocortisone for 9 months did not normalize hypocalcemia and hyperphosphatemia. The addition of 1,25-(OH)2D3 (0.5-2 micrograms/day) to the corticoid treatment for 1 year was followed by a progressive normalization of plasma calcium, phosphorus, PTH and alkaline phosphatase concentrations with improvement of the

Highly sensitive two-site immunoradiometric assay of parathyrin, and its clinical utility in evaluating patients with hypercalcemia.

We have developed a highly sensitive, two-site immunoradiometric assay (IRMA) for human parathyrin (PTH) that is specific for the intact, secreted, biologically active 84-amino-acid peptide. This assay has several technical advantages: it does not detect even high concentrations of inactive carboxyl-terminal fragments, results are available within 24 h, and the detection limit for intact hormone is low (1 ng/L). The assay readily measures concentrations of PTH in all healthy subjects and distinguishes these values from low or undetectable PTH values observed in clinical situations in which PTH

[Transitory pseudohypoparathyroidism in the newborn infant].

This is a report on two young infants suffering from transitory pseudohypoparathyroidism. Therapy with vitamin D in high doses was not helpful but metabolic changes were promptly corrected by the administration of 1.25-(OH)2-D3 (Rocaltrol). The typical pretreatment findings were: hypocalcemia, hyperphosphatemia, reduced clearance of phosphate. Parathormone in serum was normal. Treatment with 0.25 and 0.5 micro-Gram 1.25-(OH)2-D3 respectively resulted in prompt correction of serum calcium- and serum-phosphate-changes. The observation of the urinary-cAMP-excretion during the course of the diseas

[Physiology and main implications of magnesium in osteo-articular pathology].

The role of magnesium in the constitution, metabolism and hormonal control of bone tissue can be deducted from what is known of its physiology. Magnesium deficiency may result in vitamin-D resistant hypocalcaemia or even biochemical signs of hypoparathyroidism, which only regress after the deficiency is corrected. Measurements of magnesium levels in blood and urine are justified when such abnormalities occur in patients with digestive disorders, predominantly alcoholism, liver diseases or malabsorption. In all other circumstances, magnesium therapy is very rarely indicated for osteo-articular

Radioimmunoassay of serum parathyrin: comparison of five commercial kits.

We compared serum parathyrin radioimmunoassay data obtained with five commercially available kits for normal subjects, patients on dialysis, and patients with hyperparathyroidism. The Cambridge Nuclear kit gave low results, did not distinguish the dialysis-group sera from the normal sera, and had the greatest CV (56%) both inter- and intra-assay. The Immuno Nuclear PTH-II kit gave more satisfactory clinical data than their PTH-C-terminal kit; however, as with most procedures involving a substantial and variable nonspecific binding correction for each serum, parallelism between samples and the

[Successful gastric ulcer treatment of a patient with primary hyperparathyroidism with cimetidine (author's transl)].

Case report on the successful gastric ulcer treatment of a patient with primary hyperparathyroidism with the H2-receptor antagonist cimetidine. After four weeks of therapy with 0.8 g/d cimetidine stomal ulcerations were seen by x-rays and endoscopy to be healed. The patient was discharged on a regimen of 400 mg daily of oral cimetidine. Peptic ulcer did not recur after 1/2 year of treatment with this H2-receptor antagonist, although the signs of primary hyperparathyroidism were still present.

Evaluating the preoperative diagnostic value of 99mTc-MIBI/99mTcO4 - dual-isotope SPECT/CT fusion imaging and ultrasonography in suspected primary hyperparathyroidism.

BACKGROUND: Primary hyperparathyroidism (PHPT) is a common endocrine disorder that often requires surgical intervention. Preoperative locali … yroid adenoma volume. METHODS: A retrospective analysis was conducted on 75 patients with elevated parathyroid hormone (PTH) levels and suspected primary hyperparathyroidism between September 2023 and November 2024. All patients underwent 99mTc-MIBI/99mTcO4 - dual-isotope SPECT/CT fusion imaging and US. Using surgical histopathology as the gold standard, the sensiti …

Serum calcium, phosphorus, and parathyroid hormone levels and 24-hour urine calcium excretion were similar in both groups. Primary hyperparathyroidism--early diagnosis in patients referred for thyroid surgery.

BACKGROUND AND AIMS: From 1986 to 1998, 190 patients presented for first-time operations for sporadic, non-malignant, non-multiple endocrine … the operation (n=29). The groups were compared regarding clinical manifestations, serum calcium and parathyroid hormone, pathologic-anatomical substrates, operative complications and outcome. RESULTS: Renal, osseous and gastrointestinal manifestations were more frequent in group I than in groups IIa and IIb (P<0.05). However, cardiovascular and neuromuscular symptom …
