Role of nitrates in unstable angina pectoris.

Unstable angina pectoris is a clinically heterogeneous process with patient symptoms varying between reduced threshold for exertional angina and the occurrence of multiple episodes of rest pain. The major factors in the pathogenesis of unstable angina appear to be intracoronary platelet aggregation and thrombus formation secondary to fissuring or rupture of atheromatous plaques, with associated coronary vasoconstriction due to release of constrictor materials from aggregating platelets and deficiency of endothelium-related vasodilator activity. The latter factor is of particular interest in vi

Illness perception in patients with Alopecia areata under topical immunotherapy.

Alopecia areata (AA) is a chronic autoimmune-mediated disorder. There is little research on how AA patients conceptualize their disease. The purpose of this study was to investigate the perception and attitude of patients with AA about their illness. The cross-sectional study was performed on 102 patients with AA referred to our alopecia clinic. Patients between 16 and 60 years were enrolled in the study. A questionnaire including demographic and clinical characteristics and Illness Perception Questionnaire-Revised (IPQ-R) was administered to each patient. More than half of patients (55%) expe

Osteonecrosis of the jaw correlated to bisphosphonate therapy in non-oncologic patients: clinicopathological features of 24 patients.

OBJECTIVE: Osteonecrosis of the jaws (ONJ) is a well known side effect of bisphosphonate therapies in patients with multiple myeloma or other malignancies. Its real incidence is still undetermined, and only few cases of ONJ in patients taking bisphosphonates for non-oncologic diseases have been reported. It was postulated that the clinical features, predisposing factors, and treatment outcome of this subset of patients might be different from those of oncologic patients. METHODS: Over a 4 year period, a total of 102 bisphosphonate-treated patients affected by ONJ were identified. Among these,

A review of the pharmacological and psychopharmacological aspects of smoking and smoking cessation in psychiatric patients.

The data reviewed confirm that mentally ill patients smoke twice as many cigarettes as patients without mental illness. The secretion of neurotransmitters such as noradrenaline, serotonin, dopamine, acetylcholine, gamma-amino-butyric acid and glutamate is increased by the binding of nicotine to central nicotine receptors. There are also data showing that serotonin formation and secretion in patients with mental illness are influenced by chronic smoking. Cigarette smoke inhibits the activity of monoamine oxidase B, which is responsible for the catabolism of several brain neurotransmitters. Pati

Post-incident responses (debriefing) after restrictive interventions in mental health inpatient settings: A realist synthesis.

BACKGROUND: Restrictive interventions (restraint, seclusion and rapid tranquilisation) are used widely in adult mental health inpatient settings despite national and international priorities to reduce them. Post-incident responses (sometimes referred to as debriefing), the immediate support and subsequent reflection and learning that should follow restrictive interventions, are recommended by the National Institute for Health and Care Excellence and embedded in restraint reduction frameworks, but their required components and mechanisms of effect remain poorly specified. AIM: To explain how p

Effectiveness and Implementation of Digital Health Interventions on Physiological, Psychological, and Functional Outcomes in Adults With Multimorbidity: Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Multimorbidity involves heterogeneous disease combinations, treatment burden, competing priorities, and complex care pathways. Digital health interventions (DHIs) may support monitoring, self-management, and care coordination, but their effects on health-related outcomes remain uncertain. OBJECTIVE: This systematic review and meta-analysis evaluated the effectiveness of DHIs on physiological, psychological, and functional outcomes in adults with multimorbidity, summarized implementation outcomes, and explored intervention-multimorbidity matching patterns. METHODS: PubMed, Web of

Effect of digital adherence technologies on treatment outcomes in people with drug-susceptible tuberculosis: four pragmatic, cluster-randomised trials.

BACKGROUND: The impact of digital adherence technologies on tuberculosis treatment outcomes remains poorly understood. We investigated whether smart pillboxes and medication labels can reduce poor treatment outcomes in patients with tuberculosis. METHODS: We did independent pragmatic, cluster-randomised trials in the Philippines, South Africa, Tanzania, and Ukraine. 110 clusters were randomly assigned (1:1) to standard of care versus intervention arms, which were further randomly assigned (1:1; except in Ukraine) to a smart pillbox or medication labels. We enrolled adult patients receiving tr

Whole-Exome Sequencing Analysis of Idiopathic Hypogonadotropic Hypogonadism: Comparison of Varicocele and Nonobstructive Azoospermia.

As a rare disease leading to male infertility, idiopathic hypogonadotropic hypogonadism (IHH) has strong heterogeneity of clinical phenotype and gene mutation. At present, there is no effective diagnosis and treatment method for this disease. This study is to explore the possible new pathogenic gene of idiopathic hypogonadotrophic hypogonadism and the pathological mechanism affecting its occurrence. We performed a whole-exome sequencing on 9 patients with normosmic idiopathic hypogonadotropic hypogonadism (nIHH), 19 varicocele patients with asthenospermia, oligospermia, or azoospermia, 5 patie

High complication rate of bile duct stents in patients with chronic alcoholic pancreatitis due to noncompliance.

BACKGROUND AND STUDY AIMS: Biliary obstruction in chronic pancreatitis is frequently treated by endoscopic insertion of a plastic stent into the common bile duct, a therapy regarded as having a low complication rate. The aim of this study is to analyze the frequency and severity of complications caused by biliary stents in patients with chronic alcoholic pancreatitis. PATIENTS AND METHODS: We retrospectively analyzed all our patients with chronic pancreatitis (n = 14) who were provided with a plastic stent for biliary stenosis between June 1993 and December 1997. Stent exchanges were followed

Perspective on patient and non-academic partner engagement for the responsible integration of large language models in health chatbots.

Uses of large language models (LLMs) in health chatbots are expanding into high-stakes clinical contexts, heightening the need for tools that are evidence-based, accountable, accurate, and patient-centred. This conceptual, practice-informed Perspective reflects on engaging patients and non-academic partners for the responsible integration of LLMs, grounded in the co-construction of MARVIN (for people living with HIV) and in an emerging collaboration with MIT Critical Data. Organised by the Software Development Life Cycle, we describe: conception/needs assessment with patient partners to identi
