Microbiome and atherosclerosis: hype or future therapeutic target?

From 2010 to 2025, evidence increasingly identifies the gut microbiome, microbial dysbiosis, trimethylamine-N-oxide (TMAO), and short-chain fatty acids (SCFAs) as modulators of atherosclerosis and cardiovascular disease. We conducted a narrative review of literature retrieved from PubMed, Scopus, and the Clinical Trials Registry-India (CTRI), covering studies published between January 2010 and March 2025. A total of 103 primary studies (animal, n=15; human cohort, n=28; mechanistic, n=22; randomized controlled trials, n=38) and 10 ongoing clinical trials were reviewed. Germ-free and antibiotic

Gut microbiome in type 2 diabetes mellitus: A literature review.

Type 2 diabetes mellitus (T2DM) is a complex metabolic disorder influenced by genetic, environmental, and lifestyle factors. Emerging evidence highlights the crucial role of the gut microbiota (GM) in the pathophysiology of T2DM, with alterations in microbial diversity and composition contributing to insulin resistance, systemic inflammation, and metabolic dysregulation. This review explores the current literature on the role of the GM, its metabolites, including short-chain fatty acids (SCFAs), bile acids (BAs), and branched-chain amino acids (BCAAs), as well as the impact of diet and body ma

From gut microbiota to synaptic plasticity: Mechanisms shaping cognitive function and brain disorders.

The gut microbiota has emerged as a key regulator of central nervous system development and function. Accumulating experimental and clinical evidence demonstrates that microbial signals influence brain activity through integrated neural, immune, neuroendocrine, and metabolic pathways. These pathways converge on core mechanisms of synaptic plasticity, including neurotrophic signaling, glutamatergic transmission, dendritic remodeling, and adult neurogenesis, thereby shaping learning, memory, executive function, and emotional regulation. Disruption of microbiota-brain communication is implicated

Combatting antibiotic resistance: challenges and emerging therapeutic strategies.

The discovery of antibiotics marked a crucial milestone in medical history, widely credited for its pivotal role in saving countless lives. However, the escalating resistance of microbes to traditional antimicrobial drugs (antibiotics) now jeopardizes the effectiveness of these life-saving drugs. The World Health Organization adopted a Global Action Plan (GAP) in 2015, recognizing the looming threat to human health posed by antimicrobial resistance (AMR). As of 2019, 4.95 million global deaths are attributed to AMR, with 1.27 million deaths recorded in that year alone. In this review, a thorou

The role of gut microbiome in antimicrobial resistance transmission between companion animals and livestock: mechanisms, drivers, and One Health implications.

Antimicrobial resistance (AMR) poses a critical global public health challenge, with animal gut microbiomes serving as significant reservoirs and transmission hubs for antimicrobial resistance genes (ARGs). This review synthesizes current knowledge on the central role of gut microbiomes in companion animals and livestock in facilitating AMR dissemination. It examines key mechanisms that enable horizontal gene transfer within intestinal ecosystems: conjugation, transduction, and transformation. It also highlights how co-selection by heavy metals, disinfectants, and other non-antibiotic agents s

The role of the gut microbiota-uric acid metabolism axis in high-altitude hyperuricemia: dysregulation mechanisms, pathway associations and therapeutic perspectives.

High-altitude areas (≥2500 m) are characterized by low oxygen concentrations, which leads more people affected by high uronic acid in the blood. This review systematically investigates the "gut microbiota-uric acid metabolism axis" as a potential target for inhibiting Hyperuricemia (HUA). This review introduces the four functions of the axis: direct reduction of uric acid, intestinal excretion, regulation of uricase expression, and the intestinal-renal axis signal; then investigates how hypoxia alters all of these paths. In addition, this review illustrates how this axis is related to the clas

Chronic kidney disease and cognitive impairment: The role of diet and gut microbiota.

Several important milestones have been achieved in recent years in the area of slowing the progression of chronic kidney disease (CKD). Consequently, additional facets of reducing quality of life (QoL) in CKD patients have emerged, including progressive cognitive impairment (CI) in up to 50% of patients. Accumulating evidence suggests that gut microbiome dysregulation may promote CI in a paradigm of kidney-brain-gut axis. Systemic inflammation, oxidative stress, uremic toxins and increased intestinal permeability ("leaky gut") are key components of the kidney-brain-gut axis dysfunction. Correc

Lung-Brain Axis Mechanisms of Cognitive Dysfunction in Long COVID.

Cognitive dysfunction, characterized by memory impairment, attentional deficits, and executive dysfunction, represents a critical clinical manifestation in post-acute sequelae of COVID-19 that significantly compromises patients' quality of life. The lung-brain axis, as a bidirectional regulatory network connecting the respiratory system to the central nervous system, interacts through neural circuits, humoral pathways, and microbial pathways, and may play a central role in the cognitive impairments occurring in long COVID (LC). This paper systematically reviews the multidimensional pathways of

Beyond the Lungs: ICU Oral Care and Gut Resilience-A Hypothesis-Generating Narrative Review.

The oral and gut microbiomes form an interconnected ecosystem that may influence host defense. In the intensive care unit (ICU), endotracheal intubation, broad-spectrum antibiotics, and proton pump inhibitors can disrupt physiological barriers and may facilitate ectopic gut colonization by oral pathobionts. Such translocation could contribute to intestinal inflammation, barrier dysfunction, and systemic immune dysregulation; however, direct clinical evidence in critically ill patients remains limited and largely associative, and much of the mechanistic evidence derives from animal models or no

The microbiota-gut-brain axis in postpartum depression: a review of microbial mechanisms and therapeutic potential.

Postpartum depression (PPD), a common but mechanistically uncertain mood disorder, occurs in 10-15% of mothers and seriously affects maternal and infant health. Recent evidence suggests that alteration of gut microbiota could dysregulate gut-brain axis functioning, which is potentially linked to the pathogenesis of PPD. This review innovatively synthesizes up-to-date preclinical and clinical evidence to illustrate relevant mechanisms, including microbial metabolite signaling, immune-inflammatory pathways, and neuroendocrine disruptors. Furthermore, the therapeutic potential of microbial-target

Maternal Tuberculosis and Infant Gut and Immune Development: Implications for Maternal-Child Health.

BACKGROUND: Maternal tuberculosis (TB) during pregnancy involves chronic infection, immune activation, and antimicrobial therapy, which may alter the maternal gut, vaginal, and breast milk microbiomes and influence neonatal immune development. METHODS: We conducted a narrative review of studies identified through PubMed and GoogleScholar addressing the maternal TB-microbiome-infant immunity axis. Where evidence from pregnant women with TB was limited, findings were extrapolated from non-pregnant TB populations, antibiotic exposure studies, and broader maternal-infant microbiome research. RES

Targeting the Microbiota-Gut-Brain Axis: Bacillus coagulans Protects Against Rotenone-Induced Parkinson's Disease in Rats.

Gut microbiota and its derived metabolites affect brain physiology through several pathways. Dysfunction of gut-microbiota is involved in the pathogenesis of Parkinson's disease (PD). Introduced the pleiotropic effect of probiotics (PBT) in the function of the central nervous system, can delay the disease progression through microbiota-gut-brain axis (MGBA). PD is characterized by aggregated alpha-synuclein (α-syn), oxidative stress and neuroinflammation leading to depletion of dopaminergic neurons in the midbrain region. Our study designed to assess the neuroprotective effect of PBT Bacillus

Thermoresponsive Amifostine-Loaded Chitosan Biogel for Prevention of Radiation-Induced Rectal Injury.

Radiation-induced rectal injury (RRI) limits radiotherapy and results in a poor clinical prognosis. In this study, we report a radioprotective strategy for preventing RRI using a thermoresponsive amifostine-loaded chitosan biogel. Specifically, the biogel is synthesized via the reaction of crocetin with cysteine-modified chitosan and sodium alginate, followed by incorporating amifostine-sodium glycerophosphate solution. The storage modulus increases markedly at the gel point of ∼30 °C. The biogel possesses favorable biocompatibility in vitro and maintains the ability of cells to migrate and pr

The skin microbiome: the overlooked axis in modern dermatology.

The skin microbiome plays a critical role in maintaining cutaneous barrier integrity, modulating immune responses, and influencing the expression of dermatologic disease, yet its integration into clinical practice remains limited. Microbial alterations have been described in conditions such as atopic dermatitis, acne vulgaris, psoriasis, and chronic wounds; however, uncertainty regarding causality, interindividual variability, and lack of methodological standardization have hindered clinical translation. Current diagnostic frameworks rarely incorporate microbial metrics, while antimicrobial th

Gut dysbiosis, metabolic signals, and pulmonary immune reprogramming: decoding the gut microbiota -immune axis in stroke-associated pneumonia.

Stroke-associated pneumonia (SAP) is the most common infectious complication following acute stroke. The limited efficacy of conventional antimicrobial therapy suggests that SAP may be fundamentally a syndrome driven by dysregulated cross-system interactions. This review proposes the "gut microbiota-immune axis" (GMIA) as a comprehensive framework for the development of SAP and systematically discusses the potential mechanisms by which post-stroke microbial-derived metabolic signals-including short-chain fatty acids (SCFAs), bile acids, tryptophan metabolites, and endotoxins-drive systemic imm

Bacterial specificity of the gut microbiome predicts bone density in primary hyperparathyroidism.

Primary hyperparathyroidism causes mild-to-severe bone loss, but the reason for this heterogeneity is unclear. We investigated the role of the microbiome in 50 primary hyperparathyroidism patients. Microbiome transfers from primary hyperparathyroidism patients with and without osteoporosis to germ-free mice replicated the human bone phenotype and regulated TNF+ T cells and Th17 cells in mice. Accordingly, circulating TNF+ T cells and Th17 cells and TNF/IL17 production predicted bone density in primary hyperparathyroidism patients. Bifidobacterium longum, TNF+ T cells, and Th17 cells were media
