Secondary sclerosing cholangitis in critically ill patients with febrile infection-related epilepsy syndrome (FIRES): a case series.

OBJECTIVES: To describe the occurrence of secondary sclerosing cholangitis in critically ill patients (SC-CIP) with febrile infection-related epilepsy syndrome (FIRES). METHODS: Monocentric retrospective analysis of all adult patients with FIRES admitted from January 2020 to December 2024. RESULTS: Four patients (3 males) with a mean age of 24 years (range: 18-40 years) and no significant medical history presented with cryptogenic FIRES. They required treatment with antiseizure medications (mean: 9; range: 8-10), anesthetics (propofol, midazolam and ketamine in all cases), and immunotherapie

How to treat patients with splanchnic vein thrombosis: recent advances.

Splanchnic vein thrombosis (SVT) is an unusual-site venous thromboembolism that includes portal, mesenteric, and splenic vein thrombosis as well as the Budd-Chiari syndrome. SVT is a relatively rare disease (portal vein thrombosis and Budd-Chiari syndrome are, respectively, the most and the least common presentations); roughly one‑third of the cases are detected incidentally, and liver cirrhosis and solid cancer represent the main risk factors. Once SVT is diagnosed, careful patient evaluation should be performed to assess the stage, grade, and extension of the thrombosis, as well as the risks

Spontaneous Cervical Epidural Hematoma Following COVID-19 Illness Presenting to a Chiropractor: A Case Report.

Cervical epidural hematoma (CEH) is a rare and potentially fatal condition in which blood accumulates in the epidural space of the cervical spine. A 64-year-old man presented to a chiropractor with a two-week history of sudden-onset neck pain, shoulder pain, occipital headache, and numbness in the shoulders and upper extremities. He had recovered from a mild course of coronavirus disease 2019 (COVID-19) illness one month prior. The patient's primary care provider had previously prescribed a nonsteroidal anti-inflammatory drug for his neck pain. However, his symptoms worsened, and he visited th

Fasciocutaneous Flaps for Refractory Intermediate Stage Osteoradionecrosis of the Mandible-Is It Time for a Shift in Management?

PURPOSE: Historically, free tissue transfer has been reserved for advanced stage osteoradionecrosis (ORN) of the jaw with pathologic fracture or cutaneous fistula. The purpose of this study was to evaluate if in patients with recalcitrant intermediate stage ORN, would mandibular debridement in combination with vascularized soft tissue coverage result in durable symptom and disease resolution. PATIENTS AND METHODS: A retrospective cohort study of patients with mandibular ORN was performed comparing a mandibular preserving approach with a fasciocutaneous forearm flap for progressive treatment-r

A hypothesis: Role of physical factors in pathophysiology of rhinophyma - Focus on habitual mechanical trauma.

BACKGROUND: Rhinophyma is a skin disorder which causes nose enlargement and deformation due to proliferation of sebaceous glands and connective tissue. It is not only an aesthetic problem but may also lead to impaired nasal breathing and problems with liquids intake. HYPOTHESIS: Although rhinophyma is considered to be a subtype of rosacea, here we hypothesise whether it is a separate disease with mechanical trauma as a main reason of the disease progress. METHODS: 22 patients with diagnosed rhinophyma were qualified for the study. All patients were physically examined and detailed patients'

Optimizing treatment of HIV-associated lymphoma.

Cancer is the leading cause of death for HIV-infected persons in economically developed countries, even in the era of antiretroviral therapy (ART). Lymphomas remain a leading cause of cancer morbidity and mortality for HIV-infected patients and have increased incidence even in patients optimally treated with ART. Even limited interruptions of ART can lead to CD4 cell nadirs and HIV viremia, and increase the risk of lymphoma. The treatment of lymphoma is now similar for HIV-infected patients and the general population: patients with good HIV control can withstand intensive therapies appropriate

[Management of closed head injury].

OBJECTIVE: To review current concepts regarding the management of severe closed head injury and determine the possible causes of brain damage in these patients. DEVELOPMENT: Severe closed head injury causes a high rate of mortality and disability in developed countries. The diagnosis and management of these patients has progressed in recent years, from the availability of brain scanning to varied therapeutic methods based on brain metabolism and intracranial dynamics. We have reviewed the different neuropathological processes in relation to closed head injury and emphasized the importance of

Valvular disease in the elderly.

As patients survive to advanced age, they commonly develop degenerative valvular heart disease as well as degenerative diseases of other organ systems. In addition, a reservoir of patients with other forms of valvular heart disease develop progressive symptomatology with advancing age. These patients often present a challenge to the cardiologist in both diagnosis and management. Inasmuch as these patients tolerate cardiovascular surgery less well than their younger counterparts, criteria for surgical intervention may often need modification. Chronologic age must be recognized as but one of man

Nilotinib in locally advanced pigmented villonodular synovitis: a multicentre, open-label, single-arm, phase 2 trial.

BACKGROUND: Pigmented villonodular synovitis (alternatively known as diffuse-type giant cell tumour) is a rare, locally aggressive tumour driven by a specific translocation resulting in the overexpression of colony-stimulating factor 1 (CSF1). CSF1 receptor (CSF1R) inhibitors (ie, tyrosine kinase inhibitors and antibodies) induce a response in patients with pigmented villonodular synovitis. We investigated the safety and efficacy of a CSF1R tyrosine kinase inhibitor, nilotinib, in patients with locally advanced non-resectable pigmented villonodular synovitis. METHODS: In this phase 2, open-la

Research Progress on Alzheimer's Disease with Classical Traditional Chinese Medicine Formulas.

Alzheimer's disease (AD) is caused by complex pathological changes and is a problem that cannot be ignored by the global public health system. Its main clinical manifestations include progressive cognitive impairment, mental and psychological changes, and, in advanced stages, paralysis and loss of self-care ability. The economic and psychological burdens imposed on AD patients and their caregivers have been progressively exacerbating, while the annual expenditures on AD treatment by nations worldwide and by the United Nations have demonstrated a consistent upward trend rather than a decline. Surgical Management of Progressive Scoliosis in Crisponi Syndrome: Multidisciplinary Perioperative Strategy and Technical Considerations.

INTRODUCTION: Crisponi syndrome is a rare autosomal recessive disorder caused by pathogenic variants in the CRLF1 gene and characterized by neonatal autonomic instability, feeding difficulties, recurrent hyperthermia, and persistent thermoregulatory dysfunction. Although survival has improved with advances in supportive care, musculoskeletal manifestations, particularly progressive spinal deformities, remain insufficiently described. CASE REPORT: We report a 16-year-old male with genetically confirmed Crisponi syndrome who developed severe progressive scoliosis. Recurrent febrile episodes lim

Androgen receptor inhibition suppresses anti-tumor neutrophil response against bone metastatic prostate cancer via regulation of TβRI expression.

Bone metastatic disease of prostate cancer (PCa) is incurable and progression in bone is largely dictated by tumor-stromal interactions in the bone microenvironment. We showed previously that bone neutrophils initially inhibit bone metastatic PCa growth yet metastatic PCa becomes resistant to neutrophil response. Further, neutrophils isolated from tumor-bone lost their ability to suppress tumor growth through unknown mechanisms. With this study, our goal was to define the impact of metastatic PCa on neutrophil function throughout tumor progression and to determine the potential of neutrophils

Confounder factors masking a Leydig-cell ovarian tumor in a post-menopausal woman treated for androgen-positive receptor breast cancer.

Post-menopause hyperandrogenism is a condition that needs careful evaluation. Aromatase inhibitors (AI), which are important in the management of positive estrogen breast cancer, and chronic kidney disease (CKD) can puzzle the evaluation of this condition. A postmenopause female with type-2 diabetes and advanced CKD was attended due to progressive virilization, which has started after the introduction of an AI for breast cancer 5 years earlier. Clinical and radiological investigation has confirmed a pure Leydig cell tumor as source of hyperandrogenism. Re-evaluation of the breast tumor immunoh

Usability Evaluation of a Clinical Decision Support System for Geriatric ED Pain Treatment.

BACKGROUND: Older adults are at risk for inadequate emergency department (ED) pain care. Unrelieved acute pain is associated with poor outcomes. Clinical decision support systems (CDSS) hold promise to improve patient care, but CDSS quality varies widely, particularly when usability evaluation is not employed. OBJECTIVE: To conduct an iterative usability and redesign process of a novel geriatric abdominal pain care CDSS. We hypothesized this process would result in the creation of more usable and favorable pain care interventions. METHODS: Thirteen emergency physicians familiar with the Elec

[Ethical foundations of institutional psychotherapy].

HISTORICAL ASPECTS: The idea behind this work is to have an ethical examination of the institutional psychotherapy movement which has long influenced French public psychiatry and which has progressively, since the 80s, been subject to growing doubts. In the first part, institutional psychotherapy is presented. It is a model for theoretical development and practice in psychiatric care. It came into being just following the end of the Second World War at the same time as modern medical ethics. Its principles come on the one hand, from recognition of asylums' pathogenic effects--which led to the

Inhaled General Anesthetics in Alzheimer's Disease Progression: Divergent Effects, Underlying Mechanisms, and Future Perspectives.

Alzheimer's disease (AD) is a devastating, age-related neurodegenerative disorder characterized by progressive cognitive decline, neuronal loss, and hallmark pathologies like β-amyloid (Aβ) deposition and tau hyperphosphorylation. With global aging, more AD patients or high-risk individuals undergo perioperative care, exposing them to inhaled anesthetics such as isoflurane and sevoflurane. Preclinical and clinical evidence on these agents' effects on AD pathogenesis remains conflicting, ranging from exacerbating pathology to neuroprotection, highlighting the critical need to clarify the contex

Clinical impact of the Alzheimer's Disease Neuroimaging Initiative: A review of studies using ADNI data (2023 to June 2025).

Alzheimer's disease (AD) continues to pose a major public health challenge. Since its launch in 2004, the Alzheimer's Disease Neuroimaging Initiative (ADNI) has played a pivotal role in advancing the field by providing a comprehensive, open-access, longitudinal dataset that integrates neuroimaging, biomarker, genetic, and clinical data related to AD. We used standard literature search methods to identify ∼1830 publications from 2023 to mid-2025 that used ADNI data or samples. This review highlights key ADNI studies with direct clinical applications. We describe how these have impacted the deve

A predictable cascade of junctional failure following progressive long-segment fusion: an 11-year case analysis.

Instrumented spinal fusion is a common treatment for degenerative spinal disease but may lead to progressive mechanical and biological complications, especially in long multilevel constructs. We report the case of a 62-year-old woman who developed serial adjacent segment degeneration, pseudarthrosis, implant failure, vertebral fractures, and progressive deformity over 11 years following posterior lumbar fusion. Multiple revision surgeries with stepwise cranial extension to the upper thoracic spine were required. Despite advanced fixation techniques-including multi-rod constructs, iliac fixatio

Sequential Transformation of Polycythemia Vera to Myelofibrosis and KMT2A-Rearranged Acute Myeloid Leukemia Treated With Revumenib: A Rare Case of Clonal Evolution.

Transformation of polycythemia vera (PV) into post-polycythemic myelofibrosis (MF) and subsequently acute myeloid leukemia (AML) represents one of the most aggressive trajectories among myeloproliferative neoplasms (MPNs). Post-MPN AML carries a median survival of approximately six months, particularly among older adults with adverse cytogenetics. We report the case of a 73-year-old man with JAK2-positive PV diagnosed in 2020 who progressed to MF in 2024 and developed AML later that year. Cytogenetic analysis revealed a KMT2A (11q23) rearrangement with KMT2A::ELL fusion, a finding rarely descr
