Vestibular evoked myogenic potentials are intact in cervical dystonia.

Vestibular dysfunction has been reported in patients with cervical dystonia (CD), but it is still unclear whether the abnormalities occur as part of the CD syndrome or whether they arise from the abnormal posture and movement of the head. We compared vestibular-evoked myogenic potentials (VEMPs) recorded from the affected neck muscles (i.e., cervical VEMPs) with those recorded from muscles unaffected by the dystonia, the extraocular muscles (i.e., ocular VEMPs). We compared 21 patients and age-matched normal controls to investigate whether these short-latency reflexes are altered in CD. We als

Descending influence from the nucleus locus coeruleus/subcoeruleus on visceral nociceptive transmission in the rat spinal cord.

Visceral nociceptive signals are the subject of descending modulation from the locus coeruleus/subcoeruleus (LC/SC). We have recently found dorsal horn neurons whose visceral nociceptive responses are not inhibited by the descending LC/SC system (LC/SC-unaffected neurons) in the rat. The aim of the present study was to estimate a possible role of LC/SC-unaffected neurons for pain processing and pain-related responses. We focused on the fact that nociceptive signals from a visceral organ produce not only visceral pain but also visceromotor reflexes (muscular defense). Different effects of LC/SC

Stimulus-induced bilateral central periodic discharges, cortical myoclonus and arousal responses in mild reversible coma.

OBJECTIVE: To study the association between stimulus-induced periodic epileptiform discharges (PEDs), arousal EEG responses and limb jerking in a comatose patient with reversible viral encephalitis. METHODS: We recorded video EEG in Intensive Care Unit (ICU) for strictly clinical purposes. Back averaging was performed off-line using Neuroscan 4.3. RESULTS: We recorded spontaneous and stimulus-induced bilateral central PEDs (bi-central PEDs) that were followed by phasic vertex potentials, customarily considered as EEG arousal responses. Bi-central PEDs were associated with myoclonus when prov

Evidence for increased activation of persistent inward currents in individuals with chronic hemiparetic stroke.

Despite the prevalence of hyperactive stretch reflexes in the paretic limbs of individuals with chronic hemiparetic stroke, the fundamental pathophysiological mechanisms responsible for their expression remain poorly understood. This study tests whether the manifestation of hyperactive stretch reflexes following stroke is related to the development of persistent inward currents (PICs) leading to hyperexcitability of motoneurons innervating the paretic limbs. Because repetitive volleys of 1a afferent feedback can elicit PICs, this investigation assessed motoneuronal excitability by evoking the

Electrophysiological evaluation of cranial neuropathies.

BACKGROUND: Cranial nerve abnormalities occur frequently in both focal and diffuse neurologic disorders and can be evaluated by electrophysiological techniques available in most clinical neurophysiology laboratories. REVIEW SUMMARY: The optic nerve is evaluated by visual evoked potentials. Measurements of latency, amplitude, and waveform morphology are especially useful in detecting demyelinating lesions. Brain stem auditory evoked potentials evaluate the auditory portion of the eighth cranial nerve. Using an auditory stimulus, a number of waveforms are generated, and changes in the normal pa

Effect of repetitive transcranial magnetic stimulation on saccades in depression: a pilot study.

Therapeutic repetitive transcranial magnetic stimulation (rTMS) in depression is applied over the prefrontal cortex. This brain region is known to play an important role in the control of saccades. We wanted to investigate whether the fast rTMS procedure affected saccadic activity in depression. Reflexive saccades (RS) and voluntary saccades were studied in 11 patients before and after therapeutic rTMS for depression. Two types of voluntary saccade tasks were used: a voluntary prosaccade (VpS) task and an antisaccade (AS) task. Eye movements were registered by infrared oculography. Latency and

Neuronal control of the pyloric sphincter of the guinea-pig.

The pyloric sphincter (PS) controls gastric emptying and prevents the reflux of duodenal content into the stomach. Neuronal pathways and reflexes controlling the guinea-pig PS were physiologically investigated in isolated preparations. Simultaneous intracellular or extracellular and tension recordings from PS circular muscle with electrical and stretch stimulation were used. Electrical stimulation evoked an initial small contraction followed by a relaxation with a corresponding inhibitory junction potential (IJP) then a second large contraction with a corresponding excitatory junction potentia

[Propofol: ED50 and ED90 induction doses in pediatric anesthesia].

OBJECTIVE: To study ED50 and ED90 induction doses of propofol in children. PATIENTS AND METHODS: Seventy-two children from 1 to 110 months of age and premedicated with pentobarbital 4 mg/kg participated in the study. Seven different doses of propofol (1.5, 2, 2.5, 3, 3.5, 4 and 4.5 mg/kg) were injected into the dorsal hand or foot vein in 30 seconds. We measured the response to application of the mask, corneal reflex and response to trapezius muscle compression 30 and 60 s after administration. A log-probit statistical analysis was used and complications were noted. RESULTS: The doses that p

[Possibilities of early diagnosis of autonomic neuropathy in diabetics].

The possibilities of the diagnosis of autonomous neuropathy were investigated by the authors in 71 insulin-treated diabetes mellitus patients with the use of dynamic esophageal scintigraphy and determination of cardiac reflex tests. It has been found that the extent and severity of the lesion can be earlier and more exactly recognized with the help of these two methods. Important information can be obtained by the determination of esophageal transit time about the functional condition of vagal nerves supplying gastrointestinal organs. In patients suffering from autonomous neuropathy the esopha

Characterization of cat nasal afferents and brain stem neurones receiving ethmoidal input.

Stimulation of the nasal mucous membrane can initiate protective reflexes, particularly sneezing and apnea. Very little is known about the receptors in the nasal cavity responsible for initiating these reflexes, which are thought to be mediated by trigeminal rather than olfactory pathways. In the cat, the ethmoidal branch of the ophthalmic division of V carries this afferent information. The objectives of the present study were to determine a set of adequate nasal receptor stimuli capable of initiating these protective reflexes; to characterize the types of information carried in the ethmoidal

Central cardiovascular effects of baclofen in spontaneously hypertensive rats.

This study was conducted to investigate the effects of centrally administered baclofen on blood pressure and heart rate in conscious spontaneously hypertensive (SHR) and normotensive Wistar-Kyoto (WKY) rats. Administration of baclofen (1.0 microgram/kg) into the lateral cerebral ventricle (icv) produced an increase in mean arterial pressure (MAP) in both SHR and WKY rats. The increase in MAP was significantly lower in SHR (13 +/- 3 mmHg) when compared with WKY (27 +/- 5 mmHg). The changes in heart rate (HR) were variable, from no change to a very small increase and did not differ significantly

Chlordiazepoxide facilitates erections and inhibits seminal emission in rats.

Two experiments have been conducted to clarify the effects of the benzodiazepine chlordiazepoxide (CDP) on the different components of male rat sexual behavior. In the first experiment the effects on penile erectile reflexes (PR) of two doses (10 and 30 mg/kg, IP) of CDP were compared with those of vehicle, no-treatment and baseline pre-test in a repeated measures design. In the second experiment the different components of male sexual behavior - PR, mating behavior and spontaneous seminal emission SSE) - were extensively studied after vehicle, 3 and 30 mg/kg CDP treatments. CDP was found to i

[Electronystagmograhy in disseminated sclerosis. Uses and limits (author's transl)].

A review of the literature shows the extreme frequency of nystagmus and changes in vestibular reflexes in disseminated sclerosis. During the course of this disease, it is possible to detect nystagmus in at least two-thirds of cases and in a similar proportion there are changes in vestibular reflexes, usually a hyperreflexia, especially in the early phases of disseminated sclerosis. Subjectively the patients rarely complain of vertigo and such symptoms are in any case part of the early form of the disease. On the other hand, disorders of balance are much more frequent but the factors involved a

Bradycardia and the trigeminal nerve.

A review of the literature suggests that the oculocardiac reflex is part of a wider phenomenon involving any structure supplied by the trigeminal nerve. This can lead to potentially fatal complications in maxillofacial surgery. A review of the literature is discussed and two further cases are reported.

Deep inspiration induced sinus arrest. An unusual manifestation in a patient with the sick sinus syndrome.

In a patient with the sick sinus syndrome and near syncope a prolonged sinus pause was documented and reproduced thereafter during sustained deep inspiration. Administration of intravenous atropine abolished this phenomenon, most probably indicating a hyperresponsiveness of the sinus node and AV junction to a vagotonic reflex.
