Endovascular treatment of tiny ruptured anterior communicating artery aneurysms.

INTRODUCTION: Because of its high complication rate, the endovascular treatment (EVT) of anterior communicating artery (ACoA) aneurysms less than 3 mm in maximum diameter remains controversial. We evaluated EVT of tiny ruptured ACoA aneurysms with Guglielmi detachable coils (GDCs). METHODS: We treated 19 ruptured ACoA aneurysms with a maximum diameter of <or=3 mm with GDCs. The pretreatment Hunt and Hess score was grade 1 in four patients, grade 2 in six, grade 3 in six, and grade 4 in three. The patients were clinically assessed before and after treatment and with multiple angiographic follo…

[Choriocarcinoma presenting as a spontaneous rupture of an adrenal metastasis: a case report].

A 20-year-old woman, who had a history of childbirth, was admitted to the emergency room with an acute abdomen. An abdominal enhanced computed tomography (CT) revealed retroperitoneal hemorrhage, leading to life-threatening, and multiple tumors in the liver. Additionally, chest CT also revealed multiple coin lesions in the lung. These findings strongly indicated a spontaneous rupture of adrenal tumor. We performed exploratory laparotomy. An examination during surgery showed a rupture of adrenal tumor. En bloc resection of adrenal gland, tumor, and the kidney was performed. Pathological finding…

Outcome of achilles tendon ruptures treated by a limited open technique.

BACKGROUND: The optimal surgical management of Achilles tendon ruptures remains a topic of active debate. Recently, many authors have preferred the limited open method because it afforded sufficient visualization to ensure anatomic apposition of disrupted tendon fibers, minimized local blood supply disturbances, guaranteed free tendon movement, and produced excellent cosmesis. We report our initial experience with this technique and review the literature. MATERIALS AND METHODS: The outcomes of 30 consecutive patients that underwent limited open repair for Achilles tendon rupture using Achillo…

Ruptured sinus of Valsalva aneurysm associated with noncompaction of the ventricular myocardium.

Rupture of a sinus of Valsalva aneurysm is a rare, but life-threatening cardiac abnormality that requires surgical correction when diagnosed, and is frequently associated with other congenital defects, particularly with ventricular septal defect, aortic valve regurgitation, and bicuspid aortic valve. We present the case of a 21-year-old man who had a ruptured aneurysm of the noncoronary sinus into the right atrium, a ventricular septal defect, a persistent left superior vena cava and a noncompaction of the ventricular myocardium diagnosed by two-dimensional echocardiography. Surgical repair wa…

Female gender and the risk of rupture of congenital aneurysmal fistula in adults.

AIMS: To delineate the risk factors for rupture of congenital aneurysmal fistulas in adult patients. METHODS: We conducted a literature search of the Medline database using Pubmed search interface to identify reports dealing with rupture of congenital aneurysmal fistulas in an adult population. The search included the English and non-English languages between 1963 and 2005. RESULTS: Fourteen adult patients (12 females) with serious and life-threatening complications secondary to aneurysmal fistulas were reported. Mean age was 62.9 years. The ethnic origins of these 14 patients were 9 Asian a…

[Ruptured pseudoaneurysm of the renal artery associated with segmental arterial mediolysis].

We present a 71-year-old woman with spontaneous perinephric hematoma due to a rupture of pseudoaneurysm of the right renal artery on the fourth day after radical cystectomy and bilateral ureterocutaneostomy for bladder cancer. This patient received steroid therapy for chronic rheumatoid arthritis for several years. The digital subtraction angiography of the right renal artery showed two pseudoaneurysms in the anterior inferior segmental branch and the posterior inferior segmental branch. Transarterial coil embolization of the right renal artery proximally and distally to the two aneurysms was …

MRSA-associated bacterial myocarditis causing ruptured ventricle and tamponade.

We report a case of an 81-year-old man with bacterial myocarditis presenting with elevated troponins and sepsis, who succumbed due to a ruptured ventricle. The infecting organism was found to be methicillin-resistant Staphylococcus aureus. Bacterial myocarditis is a rare occurrence when independent of infective endocarditis. Generally, this is a complication of bacteremia that is discovered post-mortem. Rarely, as in our patient, it causes significant necrosis of the myocardium leading to rupture of a ventricle. As with viral myocarditis, this disease can present with signs and symptoms of acu…

Failure in the nonoperative management of pediatric ruptured appendicitis: predictors and consequences.

INTRODUCTION: The initial nonoperative management of perforated appendicitis fails in 15% to 25% of children. These children have complications and increased hospitalization. The purpose of this study was to identify predictors of failure. METHODS: Children with perforated appendicitis treated with antibiotics and intent for nonoperative management over a 4-year period were reviewed. Seventy-five children were identified and included in the study. Failure was defined as undergoing appendectomy before the initially planned interval. RESULTS: Nine (12%) of the patients required appendectomy so

Recurrent anaphylaxis due to non-ruptured hepatic hydatid cysts.

BACKGROUND: Hydatid disease, a parasitic infestation of humans, is endemic in the Mediterranean region, Australia, New Zealand and the Middle East, and mostly involves the liver. Anaphylactic reactions, which sometimes are the first manifestations of the disease, frequently occur due to cyst rupture after a minor/major trauma, though they may also be spontaneously seen on rare occasions. In extremely few studies, anaphylactic shock has been reported in patients without macroscopic rupture of the hydatid cysts. CASE REPORT: Our patient had recurrent anaphylactic episodes without any trauma and…

The CT pattern of a ruptured P3 posterior cerebral artery aneurysm.

A 48-year-old male smoker with a history of significant alcohol use developed thunderclap headache during sexual intercourse. On neurological exam, he was confused and agitated with a Glasgow Coma Score of 11 (E3, M6, V2). Computed tomography (CT) of the brain revealed diffuse subarachnoid hemorrhage (SAH) with blood notably present in the quadrigeminal and supracerebellar cisterns (Figure 1). Cerebral angiogram demonstrated a distal P3 posterior cerebral artery (PCA) aneurysm (Figure 2). The PCA aneurysm was successfully treated with endovascular coil occlusion.

[Spontaneous rupture of the spleen as a rare complication of chronic calcifying pancreatitis].

Spontaneous rupture of the spleen is a rare complication of chronic calcifying pancreatitis. Anaemia and haemorrhagic shock may occur but pain is the first symptom making diagnosis more difficult. We report the case of a 32-year-old man suffering from chronic pancreatic pathology who developed a spontaneous splenic rupture. He complained of abdominal pain without haemorrhagic shock. An abdominal CT-scan revealed a rupture of the spleen with a haemoperitoneum.

Ruptured hepatic artery aneurysm: a novel approach to distal control.

We describe a unique method for obtaining distal control in a ruptured hepatic artery aneurysm. A 34-year-old man with Ehlers-Danlos syndrome type IV presented with a ruptured left hepatic artery aneurysm. The distal aneurysm was within the parenchyma of the liver. Proximal control was obtained with pledgeted sutures. The distal hepatic artery was difficult to control. Injection of 5 mL of thrombin into the distal left hepatic artery resulted in hemorrhage control. This technique should be considered in select patients in whom distal arterial control is challenging.

Spontaneous hemoperitoneum in pregnancy from a ruptured superficial uterine vessel.

A 31-year-old multipara woman pregnant at gestational age 32+ weeks with twins encountered hemoperitoneum resulting from superficial uterine vessel rupture during tocolytic course. The initial presentations were unspecific and sonographic examination was negative. Later the aggravated symptoms led to an impression of abruption placentae and emergent cesarean section was performed. A superficial venous bleeder was located on the posterior uterine wall and the internal bleeding was up to 3 L. Maternal and fetal outcome were good. Hemoperitoneum during pregnancy is rare but life-threatening to bo…

A case of spontaneous rupture perforation of leiomyosarcoma in the urachus of a young boy.

A case of spontaneous rupture of the urachal leiomyosarcoma in a 12-year-old boy is presented. This is of interest because massive hemoperitoneum due to spontaneous rupture of the urachal leiomyosarcoma is remarkably rare. We diagnosed based on histopathological features. Partial cystectomy and adjuvant chemo-radiation therapy was done with no sign of local recurrence or metastasis until 6 months after the operation.

Surgical experience of the ruptured distal anterior cerebral artery aneurysms.

OBJECTIVE: Distal anterior cerebral artery (DACA) aneurysms are fragile and known to have high risks for intraoperative premature rupture and a relatively high associated morbidity. To improve surgical outcomes of DACA aneurysms, we reviewed our surgical strategy and its results postoperatively. METHODS: A total of 845 patients with ruptured cerebral aneurysms were operated in our hospital from January 1991 to December 2005. Twenty-three of 845 patients had ruptured DACA aneurysms which were operated on according to our surgical strategy. Our surgical strategy was as follows; early surgery, a…

[Artery-to-artery embolism due to ruptured calcified carotid plaque].

Calcified carotid plaques are thought to be less likely to be symptomatic than non-calcified plaques. We present a patient with an unusual cerebral embolism that appeared as very high density on CT and was ascertained to derive from a calcified plaque. This 46-year-old male was admitted within 1 hr of sudden aphasia onset. The admission CT scan showed multiple high-intensity lesions that appeared like calcification. They were high intensity on MRI FLAIR images. MRA showed occlusion of the posterior trunk of the middle cerebral artery. As we considered cerebral embolism, the patient received he
