OBJECTIVE: To report a case of successful testicular sperm extraction performed during an emergency scrotal exploration for trauma in a patient with a history of previous contralateral cryptorchidism.
DESIGN: Case report.
SETTING: Clinica Urologica I, Università degli Studi di Milano, Fondazione Istituto di Ricerca e Cura a Carattere Scientifico Ca' Granda, Ospedale Maggiore Policlinico.
PATIENT(S): A 28-year-old man with a history of right orchiopexy for cryptorchidism presenting with blunt trauma of the pelvis.
INTERVENTION(S): Seminiferous tubules extraction.

Duodenal atresia is a common cause of neonatal bowel obstruction. Double duodenal atresia has been reported as a rare variation of duodenal atresia. This report discusses the importance of an appreciation of unusual anatomy in neonatal duodenal obstruction and highlights the importance of assessing bowel continuity intraoperatively.

In our study, we introduce the use of a scrotal septal skin flaps in repair of hypospadias. Between June 2006 and October 2008, 30 patients were treated by using a penile local flap to reconstruct the urethra, and a scrotal septal skin flap to cover the wound located on the ventral side of the penis. Of 30 patients, 4 presented with urine leakage, but this condition healed spontaneously after 2 to 4 weeks. Urine leakage healed without problem in the remaining patients, and they had unobstructed urinary flow. Additionally, they all were satisfied with genital appearance after the reconstruction

Koyanagi described an elegant technique for one-stage proximal hypospadias repair. It is particularly suited for the most severe forms of hypospadias. A modified Koyanagi technique was used to repair the hypospadias in 14 children, aged 3-9 years. One child developed breakdown of the suture line, three other children developed small urethrocutaneous fistula needing closure and one child had post-operative meatal stenosis. Despite the high rate of complications, modified Koyanagi's procedure is a good procedure of choice for severe/proximal hypospadias.

The incidence of metallic rings causing genitalia strangulation has been widely reported. The removal of the constricting rings can be challenging and might need resources not usually available in the hospital. We report one such case where the assistance of a fire crew was required to remove the ring.

A case of meconium periorchitis detected by fetal MRI and misdiagnosed during pregnancy as inguinoscrotal hernia is reported for the first time. A full-term black boy presented at birth with an asymptomatic, 'stony-hard', scrotal mass suggestive of an in utero testicular torsion or testicular/paratesticular tumor. Early surgical treatment resulted in the removal of paratesticular yellowish amorphous material. Histology was consistent with the diagnosis of meconium periorchitis, a rare and benign condition resulting from healed intrauterine bowel perforation.

Several studies have suggested that male infertility and testicular cancer may have common aetiological factors. Scrotal ultrasonography (US) has an important role in the diagnosis of testicular tumours when not palpable by physical examination. In this study, we present two infertile men referred to our clinic. Patients were evaluated by a detailed physical examination, semen analyses and hormonal assessment. Scrotal hematoma is a rare and unusual complication of transfemoral cardiac catheterization. We describe the case of a 43-year-old man who presented with a delayed retroperitoneal hemorrhage and massive scrotal hematoma following cardiac catheterization. Emergent groin exploration was performed with ligation of the inferior epigastric artery and evacuation of the scrotal hematoma. Urological sequelae from transfemoral arterial access are rare. However, scrotal hematoma may occur if bleeding occurs between anatomic fascial planes.

PURPOSE: Traditionally cuff placement of an artificial urinary sphincter is done through a perineal approach. A new approach through a penoscrotal incision or transscrotal approach is reportedly more rapid and easier than the traditional incision. Injury to the male external genitalia is relatively uncommon when compared with other parts of the human body. Trauma to external genitalia could result from accidents involving motor vehicles, power farm machinery, gun shot and stab injuries. We present a rare case of traumatic degloving injury of male external genitalia associated with partial penile amputation and loss of both testes sparing the internal abdominal and pelvic organs, resulting from run over by a heavy vehicle tyre.

OBJECTIVE: To evaluate the differential diagnosis of scrotal mass with color Doppler ultrasound.
METHODS: Retrospective analysis was made of 21 cases of scrotal mass confirmed both surgically and pathologically in our hospital.
RESULTS: Eight of the total number were malignancy of the testis origin, accounting for 38.1% of whole study group and 13 were benign, accounting for 61.9%. Of the 13 benign cases, only 2 were of the testis origin (15.4%) while the other 11 (84.6%) were not.

Sixteen yearling bulls were utilized to investigate administration of ergotamine tartrate on semen parameters, fertilization, and endocrinology. We report a case of a perforated rectal adenocarcinoma presenting as Fournier's in the emergency department. CT findings in this case confirmed the clinical diagnosis, established the extent of disease, and suggested the presence of an underlying rectal neoplasm as the primary source of infection.
