Septic arthritis due to streptococci and enterococci in native joints: a 13 year retrospective study.

OBJECTIVES: Streptococcal species are the second most common cause of native joint septic arthritis (SA). However, there are few systematic data about streptococcal SA. METHODS: The medical records of adults with SA caused by streptococci, pneumococci, and enterococci at our tertiary care centre between 2003 and 2015 were reviewed. RESULTS: 71 patients (34% female) with 83 affected joints were included. Median age was 62 years. A single joint was involved in 62 patients (87%). One or more comorbidities were present in 58 patients (82%). 16 patients (23%) had a concomitant soft-tissue infecti

Tophaceous joint disease strongly predicts hand function in patients with gout.

OBJECTIVES: The functional impact of gout is poorly understood. The aim of this study was to determine predictors of hand function in gout. METHODS: Twenty unselected patients with gout were recruited from rheumatology clinics. No patient had an acute gout flare at assessment. Participants were assessed for clinical characteristics of gout, including the site and number of tophi. Hand function was assessed by the Sollerman hand function test. Fingertip to palm (FTP) distance measurement, grip strength and the Disability Assessment of Shoulder and Hand (DASH) questionnaire scores were also rec

Minimally Invasive Acromioclavicular Joint Stabilization Using an Adjustable Endobutton: A Report of Two Cases.

Acromioclavicular joint dislocations (ACJD) are frequent shoulder injuries in young, active patients and athletes and can lead to pain, functional impairment, and cosmetic deformity if not adequately treated. Surgical management of high‑grade ACJD remains controversial, with numerous techniques described and no clear gold standard, particularly regarding the balance between stability, invasiveness, and the need for hardware removal. This study of two cases (Rockwood types III and IV) evaluated a novel minimally invasive technique using an adjustable Endobutton (Smith & Nephew, London, UK) with

Biomimetic gradient scaffolds for the tissue engineering and regeneration of rotator cuff enthesis.

Rotator cuff tear is one of the most common musculoskeletal disorders, which often results in recurrent shoulder pain and limited movement. Enthesis is a structurally complex and functionally critical interface connecting tendon and bone that plays an essential role in maintaining integrity of the shoulder joint. Despite the availability of advanced surgical procedures for rotator cuff repair, there is a high rate of failure following surgery due to suboptimal enthesis healing and regeneration. Novel strategies based on tissue engineering are gaining popularity in improving tendon-bone interfa…

Evaluation of a machine-learning-driven active-passive upper-limb exoskeleton robot: Experimental human-in-the-loop study.

Evaluating exoskeleton actuation methods and designing an effective controller for these exoskeletons are both challenging and time-consumin … n estimation, mid-level strength regulation, and low-level actuator control. It was then applied to shoulder joint elevation experiments to verify the exoskeleton controller's effectiveness. The active-passive assistance was compared with fully passive and fully active exoskeleton control using the following criteria: (1) post-test survey, (2) load tolerance dur …

A Mysterious Case of Left Shoulder Pain in AIDS: A Case Report and Literature Review.

Neisseria (N.) gonorrhea is a gram-negative diplococcus and one of the most commonly reported sexually transmitted infections (STIs) in the United States. Disseminated gonococcal infection is a rare but serious complication of N. gonorrhoeae infection that can result in arthritis-dermatitis syndrome or purulent gonococcal arthritis. Co-infection with human immunodeficiency virus (HIV) has been shown to reduce the efficacy of complement recruitment, which may lead to an increased risk of disseminated gonococcal spread. We present a case of a 41-year-old male with concomitant HIV-gonorrhea infec

Successful repair of thoracic outlet syndrome in a growing young patient due to posterior sternoclavicular joint dislocation.

Posterior sternoclavicular dislocation is an extremely rare injury, usually related to heavy trauma, such as a traffic accident. The anatomical proximity of vital mediastinal structures often discourages a closed reduction during emergency situations. In this case report, we present a 17-year-old male patient who was admitted to our emergency department after having fallen down a flight of stairs five days previously. He was eventually found to have thoracic outlet syndrome. Chest computed tomography (CT) and shoulder magnetic resonance imaging (MRI) showed a posterior dislocation of the right

Rotator cuff rehabilitation: current theories and practice.

A fully functioning, painless shoulder joint is essential to maintain a healthy, normal quality of life. Disease of the rotator cuff tendons (RCTs) is a common issue that affects the population, increasing with age, and can lead to significant disability and social and health costs. RCT injuries can affect younger, healthy patients and the elderly alike, and may be the result of trauma or occur as a result of chronic degeneration. They can be acutely painful, limited to certain activities or completely asymptomatic and incidental findings. A wide variety of treatment options exists ranging fro…

Calcific tendinitis of the rotator cuff: a review.

Calcifying tendinitis of the rotator cuff is a common disorder; its underlying mechanism still remains unknown. Although details of the clinical presentation(s) and pathological changes which are associated with calcific tendinitis are available, conservative management of this condition remains a topic of debate. About 90% of the patients can be treated non - operatively, but as some are resistant to conservative treatment; newer techniques or surgery should be indicated. Rheumatologists and radiologists have often described this shoulder abnormality, leading to its progressive differentiatio

2012 Provisional classification criteria for polymyalgia rheumatica: a European League Against Rheumatism/American College of Rheumatology collaborative initiative.

The objective of this study was to develop European League Against Rheumatism/American College of Rheumatology classification criteria for polymyalgia rheumatica (PMR). Candidate criteria were evaluated in a 6-month prospective cohort study of 125 patients with new-onset PMR and 169 non-PMR comparison subjects with conditions mimicking PMR. A scoring algorithm was developed based on morning stiffness >45 minutes (2 points), hip pain/limited range of motion (1 point), absence of rheumatoid factor and/or anti-citrullinated protein antibody (2 points), and absence of peripheral joint pain (1 poin

Rotator cuff coactivation ratios in participants with subacromial impingement syndrome.

Coactivation of the rotator cuff is vital to glenohumeral joint stability by centralising the humeral head within the glenoid fossa. Yet in individuals with subacromial impingement, it is hypothesised that rotator cuff coactivation abnormalities are present that could contribute to their shoulder pain. The purpose of this study was to determine if abnormal rotator cuff coactivation and deltoid activation patterns exist in participants with subacromial impingement. Rotator cuff (supraspinatus, infraspinatus, and subscapularis) coactivation and middle deltoid activation was assessed during an el

Concentric and/or eccentric training-induced alterations in shoulder flexor and extensor strength.

To examine the effect of specific concentric and eccentric training on muscular strength following an initial standardized period of excessive training, shoulder extensors and flexors were tested. The shoulder is a joint particularly susceptible to injury and requires a specific strength training program. For a period of 12 weeks, subjects performed training, including concentric and eccentric actions. Thereafter, they were divided into three groups that performed specific 12-week programs, including either concentric (C-E/C), eccentric (C-E/E), or a combination of both concentric and eccentri

A comparison of hemiarthroplasty with aortograft vs. reverse total shoulder arthroplasty following proximal humeral oncologic resection.

BACKGROUND: Reconstruction after proximal humerus tumor resection poses challenges in restoring stability and function due to loss of rotator cuff attachments and soft-tissue integrity. Hemiarthroplasty (HA) with synthetic mesh augmentation has traditionally been used, whereas reverse total shoulder arthroplasty (rTSA) has emerged as an alternative. Comparative data between these techniques in oncologic settings remain limited. This study compared functional outcomes and complication profiles between HA with aortograft mesh (HA-aortograft) and rTSA endoprosthetic reconstructions following prox

The effect of surgeon volume on complications after total shoulder arthroplasty: a nationwide assessment.

BACKGROUND: Increased surgeon volume has been demonstrated to correlate with improved outcomes after orthopedic surgery. However, there is a lack of data demonstrating the effect of surgeon volume on outcomes after total shoulder arthroplasty. METHODS: The PearlDiver Mariner database was retrospectively queried from the years 2010 to 2022. Patients who underwent shoulder arthroplasty were selected using the Current Procedural Terminology code 23472 (total shoulder arthroplasty). Patients younger than 40 years, those who underwent revision arthroplasty, and cases of bilateral arthroplasty were

The effect of smoking on outcomes of reverse total shoulder arthroplasty.

BACKGROUND: The effect of smoking status on clinical outcomes in reverse total shoulder arthroplasty (rTSA) has not been thoroughly characterized. We sought to compare pain and functional outcomes, complications, and revision-free survivorship between current smokers, former smokers, and nonsmokers undergoing primary rTSA. METHODS: We retrospectively reviewed a prospectively collected shoulder arthroplasty database from 2004-2020 to identify patients who underwent primary rTSA. Three cohorts were created based on smoking status: current smokers, former smokers, and nonsmokers. Outcome scores

Disease diagnosis and arthroplasty type are strongly associated with short-term postoperative patient-reported outcomes in patients undergoing primary total shoulder arthroplasty.

BACKGROUND: Prognostic factors for total shoulder arthroplasty (TSA) clinical outcomes are incompletely understood. This study investigates the associations of preoperative patient, disease-specific, and surgical factors with 1-year postoperative PENN Shoulder Score (PSS) in patients undergoing primary TSA. METHODS: Cleveland Clinic patients undergoing primary anatomic TSA (aTSA) or reverse TSA (rTSA) for glenohumeral osteoarthritis (GHOA) or rotator cuff tear arthropathy (CTA) between February 2015 and August 2019, and having complete preoperative and 1-year postoperative patient-reported ou

Discordance between patient-reported and objectively measured internal rotation after reverse shoulder arthroplasty.

BACKGROUND: Patient satisfaction after reverse shoulder arthroplasty (RSA) partly relies on restoring functional internal rotation (IR). Although postoperative assessment of IR includes objective appraisal by the surgeon and subjective report from the patient, these evaluations may not vary together uniformly. We assessed the relationship between objective, surgeon-reported assessments of IR and subjective, patient-reported ability to perform IR-related activities of daily living (IRADLs). METHODS: Our institutional shoulder arthroplasty database was queried for patients undergoing primary RS

Comparison between Anatomic Total Shoulder Arthroplasty and Reverse Shoulder Arthroplasty for Older Adults with Osteoarthritis without Rotator Cuff Tears.

BACKGROUND: Many older adults with glenohumeral osteoarthritis without rotator cuff tears experience muscle atrophy and fatty degeneration. In these cases, range of motion (ROM) recovery and clinical results after total shoulder arthroplasty (TSA) could be poor, with low subjective satisfaction after surgery. The purpose of this study was to compare the clinical outcomes of anatomic TSA and reverse shoulder arthroplasty (RSA) in patients aged over 70 years with primary glenohumeral osteoarthritis without rotator cuff tears. We hypothesized that the clinical outcomes of anatomical TSA would be
