Advanced biomaterials for repairing and reconstruction of mandibular defects.

Mandibles are the largest and strongest bone in the human face and are often severely compromised by mandibular defects, compromising the quality of life of patients. Mandibular defects may result from trauma, inflammatory disease and benign or malignant tumours. The reconstruction of mandibular defect has been a research hotspot in oral and maxillofacial surgery. Although the principles and techniques of mandibular reconstruction have made great progress in recent years, the development of biomedical materials is still facing technical bottleneck, and new materials directly affect technologic

Preoperative anemia-screening clinics.

Preoperative anemia is associated with increased postoperative morbidity and mortality and with increased risk of perioperative transfusion. It is an important and modifiable risk factor for surgical patients. For high-blood-loss surgery, preoperative anemia is defined as hemoglobin <13 g/dL for both male and female patients. Preoperative anemia is common, ranging from 25% to 40% in large observational studies. The most common treatable cause of preoperative anemia is iron-deficiency anemia; the initial laboratory tests should focus on making this diagnosis. Management of iron-deficiency anemi

A New Method of Treatment of Temporomandibular Joint Ankylosis with Osteodistraction Using the Sh-Device: A Case Report.

This case report presents a 16-year-old boy with bilateral temporomandibular joint (TMJ) bony ankylosis due to trauma. The patient had undergone several unsuccessful therapeutic surgeries and was experiencing reduced mouth opening, difficulty in eating and speaking, poor oral hygiene, snoring, and depression. Bilateral gap arthroplasty and distraction osteogenesis (DO) were performed. After the surgery, we were able to move the osteodistractors forward and prevent the upward and backward movement of the proximal mandibular segment with the use of our custom-made Sh-device, which allowed bone g

Spinal Cauda Equina Stimulation for Alternative Location of Spinal Cord Stimulation in Intractable Phantom Limb Pain Syndrome: A Case Report.

Phantom limb pain is a phenomenon in which patients experience pain in a part of the body that no longer exists. In several treatment modalities, spinal cord stimulation (SCS) has been introduced for the management of intractable post-amputation pain. A 46-year-old male patient complained of severe ankle and foot pain, following above-the-knee amputation surgery on the right side amputation surgery three years earlier. Despite undergoing treatment with multiple modalities for pain management involving numerous oral and intravenous medications, nerve blocks, and pulsed radiofrequency (RF) treat

Bilateral femoral head avascular necrosis with a very low dose of oral corticosteroid used for panhypopituitarism.

Avascular necrosis (AVN) of the femoral head is a rare complication related to glucocorticoid administration and traditionally has been associated with high doses and/or prolonged therapy. Occurrence of osteonecrosis with a physiological replacement dose of glucocorticoids has not been reported previously. We report a 38-year-old man with non-secreting pituitary adenoma who developed bilateral AVN while on a very small dose of oral prednisolone for secondary adrenal insufficiency after surgery for pituitary adenoma. The patient was switched to hydrocortisone. Zolindronic acid was administered

A case of submacular cysticercosis treated by pars plana vitrectomy in Kuwait.

We report a case of submacular cysticercosis in the left eye of an Indian patient living in Kuwait. Though he was systemically asymptomatic, his magnetic resonance imaging showed multiple cysts in the brain. The patient underwent pars plana vitrectomy (PPV), and the cyst was removed in total through a retinotomy over the cyst. He had another small cyst in the periphery that was also removed. He was treated with oral albendazole and systemic steroids after the surgery to treat his neurocysticercosis. He developed rhegmatogenous retinal detachment after the surgery, and the retina was successful

Edoxaban for the prevention of thromboembolic events after surgery.

Without thromboprophylaxis, hospital-acquired deep vein thrombosis following major orthopedic surgery occurs with an incidence of approximately 40-60%, which is why thromboprophylaxis has been standard of care in these cases for more than 20 years. Edoxaban (DU-176b; Lixiana) is a novel, once-daily, orally active antithrombotic agent that directly inhibits factor Xa activity in a potent and selective way. It was recently approved in Japan for the prevention of venous thromboembolism after total knee replacement, total hip replacement and hip fracture surgery. In phase III trials performed in p

Intraoral piezosurgery: preliminary results of a new technique.

The piezosurgery instrument, developed in 1988, uses a modulated ultrasonic frequency that permits highly precise and safe cutting of hard tissue. Nerves, vessels, and soft tissue are not injured by the microvibrations (60 to 200 mm/sec), which are optimally adjusted to target only mineralized tissue. The selective and thermally harmless nature of the piezosurgery instrument results in a low bleeding tendency. In addition, the instrument can be used in operations requiring either local or general anesthesia. The precise nature of the instrument allows exact, clean, and smooth cut geometries du

Duodenal obstruction by gallstones (Bouveret's syndrome). Presentation of a new case and literature review.

Bouveret's syndrome is a rare entity consisting in a duodenal obstruction due to the passage of gallstones from the gallbladder to the duodenum through a cholecystoduodenal fistula. Approximately 225 cases are reported in the literature. It is most common in old women with a previous history of biliary tract disease. The clinical picture is nonspecific and pre-operative diagnosis is not easy. Oral endoscopy is the main diagnostic procedure and sometimes, a therapeutic option, too. Surgery is the elective treatment specially when endoscopy is unsuccessful. We report a new case of this syndrome

[Patients with heart valve prostheses: dental care, dental procedures and prevention].

Patients with birth or rheumatic heart defects, or wearing prosthetic cardiac valves, are at high risk of developing bacterial endocarditis following dental treatment. Two hundred patients with cardiac valves were surveyed with respect to both their oral hygiene habits and their last visit to the dentist. More than half of the respondents (112) were fully edentulous, and most of them had not seen a dentist for many years. Of the 88 respondents who had natural teeth, 23 had not been to a dentist since surgery. For the purpose of this study, focus was placed primarily on the answers given by the

Current practice in biliary surgery: the Indian scenario.

OBJECTIVE: Documentation of current biliary surgical practices of surgeons in India. METHODS: A postal questionnaire survey of the consultants in surgery in all the medical colleges in India was done to establish their current practice regarding cholecystectomy and to assess the influence of recent studies on their practice. RESULTS: The response rate was 23% (264/1127). About half of the surgeons did both ultrasonography and oral cholecystography for the diagnosis of cholelithiasis. Acute cholecystitis was managed conservatively by 80% of surgeons. Antibiotics were used by all surgeons but

Medical treatment of nasal polyps.

Polyps are a multifactorial disease that affect the nasal lining and sinus mucosa, and in about one-third of the patients are associated with asthma. Polyps may occur in other respiratory diseases such as cystic fibrosis, primary ciliary dyskinesia and immune deficiencies. Allergy does not predispose to polyp formation, although mast cell reactions appear to be important. This explains why corticosteroids are effective in controlling some cases and helping to prevent recurrence in some others. Polyp formation in the sinuses is due to three factors: (1) the balance between the inflammatory resp

[Role of computed tomography in malignant tumors of the oral cavity and pharynx].

The CT findings of 22 patients with histologically verified malignant tumors of the oral cavity and pharynx were evaluated to demonstrate the value of CT. Comparison with conventional radiological methods was made. In 16 patients an attempt was made to define tumor expansion and 6 patients were examined after surgery and radiation therapy with regard to tumor recurrence. In the first group we found tumors of different stages: in advanced cancer it was evident that with CT it is possible to describe tumor expansion very exactly and its relationship to surrounding structures, as well as the inva

A comprehensive review of dental bone regeneration.

This review provides an in-depth analysis of dental bone regeneration, tracing its evolution and applications. Recent advancements in dental bone regeneration have allowed the utilization of barrier membranes to direct the regeneration process, excluding undesired cell types and fostering the growth of beneficial tissues, particularly in treating periodontal and bone defects. Over the past forty years, bone regeneration has advanced significantly, effectively enhancing periodontal ligament restoration while preventing unwanted soft-tissue growth. It is widely applied in periodontal, oral, impl

Inflammatory Changes after Medical Suppression of Suspected Endometriosis for Implantation Failure: Preliminary Results.

Unexplained euploid embryo transfer failure (UEETF) is a frustrating and unanswered conundrum accounting for 30 to 50% of failures in in vitro fertilization using preimplantation genetic testing for aneuploidy (PGT-A). Endometriosis is thought by many to account for most of such losses and menstrual suppression or surgery prior to the next transfer has been reported to be beneficial. In this study, we performed endometrial biopsy in a subset of women with UEETF, testing for the oncogene BCL6 and the histone deacetylase SIRT1. We compared 205 PGT-A cycles outcomes and provide those results foll

Enhanced recovery after head and neck cancer surgery: a review of current literature.

PURPOSE OF REVIEW: Enhanced recovery after surgery (ERAS) is well documented in a number of surgical specialities. However, it remains an emerging concept in surgical head and neck cancer care. The purpose of this review is to appraise the current evidence investigating enhanced recovery in head and neck cancer, and explore areas for future clinical research. RECENT FINDINGS: There were three key themes in enhanced recovery from the current head and neck cancer literature: early oral feeding, fistula closure and service delivery. This evidence is emerging and the quality of papers remains var

Consensus statement. Corticosteroid therapy in ENT in the context of the COVID-19 pandemic.

This consensus statement about the indications and modalities of corticosteroid treatment in the context of the COVID-19 pandemic was jointly written by experts from the French Association of Otology and Oto-Neurology (AFON) and from the French Society of Otorhinolaryngology, Head and Neck Surgery (SFORL). There is currently not enough data in favour of danger or benefit from corticosteroids in COVID-19, so until this matter is resolved it is advisable to limit their indications to the most serious clinical pictures for which it is well established that this type of treatment has a positive im

Cardiovascular implications in the treatment of obstructive sleep apnea.

Epidemiological studies provide strong evidence that obstructive sleep apnea (OSA) is associated with cardiovascular complications such as systemic hypertension, congestive heart failure, and atrial fibrillation. Successful OSA treatment with continuous positive airway pressure (CPAP) has resulted in coincident reductions in systemic hypertension, improvements in left ventricular systolic function, and reductions in sympathetic nervous activity. These data suggest that successful treatment of OSA may reduce cardiovascular morbidity in such patients. Although CPAP is the more successful treatme
