Bone-Penetrating TASER Bolt: A Rare Case of Penetrating Injury to the Middle Phalanx of the Little Finger.

TASER© (Axon Enterprise, Scottsdale, Arizona) is a type of electric gun that delivers a sudden shock to temporarily disable a human subject. It is used by law enforcement, as well as civilians, worldwide. Despite its wide use as a "non-lethal" form of self-defence, it can lead to serious injuries. We are presenting a rare case report of a 28-year-old man who presented to our Plastic Surgery Trauma Service following a penetrating TASER bolt injury to the middle phalanx of his left little finger.

Use of an Omental Flow-Through Flap for Recipient Vessels in the Reconstruction of a Lumbar Spine Defect: A Case Report.

CASE: Spine reconstruction after en bloc spondylectomy is challenging and may require multidisciplinary intervention. En bloc spine tumor resection with embolization of local recipient vessels for tumor control limits reconstructive options. Herein, we describe a case where combined efforts from orthopaedic, general, and plastic surgery teams permitted the successful reconstruction of a multilevel lumbar vertebral defect. CONCLUSION: A fibula-free flap within a titanium cage construct anastomosed to the left gastroepiploic vessels via a pedicled omental flow-through flap is a viable and novel

[Fournier gangrene in a child with steroid-resistant nephrotic syndrome. Report of one case].

Fournier gangrene is a necrotizing fasciitis that affects the genital, perineal and perianal regions, of sudden onset and rapidly progressive dissemination. Its diagnosis requires an urgent and interdisciplinary intervention. The association with nephrologic diseases is rare. We present a case of Fournier gangrene in a child with steroidresistant nephrotic syndrome and anasarca with severe scrotal edema. He received a broad-spectrum antibiotic scheme and extensive an immediate surgical debridement of the necrotic lesion was carried out. Subsequently, it was repaired by Plastic Surgery. He pres

Evaluation of Plastic-Compressed Collagen for Conjunctival Repair in a Rabbit Model.

Conjunctival integrity is crucial for a healthy ocular surface and visual acuity. In severe cases of inflammatory surface disorders or after trauma, thermal or chemical burns as well as after ocular surgery, a surgical reconstruction using conjunctival substitutes is required. Due to limitations of currently used substitutes, such as the amniotic membrane, there is a need for the development of new scaffolds of consistent quality for conjunctival reconstruction. This study explored the biocompatibility and surgical usability of plastic-compressed collagen as an alternative conjunctival substit

Biologics-based regenerative technologies for periodontal soft tissue engineering.

This manuscript provides a state-of-the-art review on the efficacy of biologics in root coverage procedures, including enamel matrix derivative, platelet-derived growth factor, platelet concentrates, and fibroblast-growth factor-2. The mechanism of action and the rationale for using biologics in periodontal plastic surgery, as well as their anticipated benefits when compared with conventional approaches are discussed. Although the clinical significance is still under investigation, preclinical data and histologic evidence demonstrate that biologic-based techniques are able to promote periodont

Autologous Fat Grafting in Cleft Lip and Palate.

Structural fat grafting is a technically simple procedure that has revolutionized the field of plastic surgery, with applications ranging from tissue regeneration to scar modulation to volume restoration. This technique has found applications in the treatment of cleft lip and palate, where it can be used to fill deficient midfacial volume and treat velopharyngeal insufficiency. In this article, the authors summarize the current literature supporting the use of fat grafting in patients with clefts. Evidence as to the safety, efficacy, longevity, and potential complications of the technique are

Degloving lower leg injury - the importance of additional treatment: negative pressure and hyperbaric oxygen therapy.

Degloving injury poses a severe therapeutic challenge concerning both trauma and plastic surgery. The injury involves separation of skin and subcutaneous tissue from fascia and muscles. Treatment is often long-lasting and brings unsatisfying results due to the extent of damage, risk of infection and massive blood loss. In this article, we present the management and therapeutic outcomes of a patient admitted due to the degloving injury of the lower extremity caused by workplace accident. We described the complexity of treatment including surgical intervention as well as additional treatment, wh

The Role of Fat Grafting in Breast Reconstruction.

Breast lipomodeling, or breast fat grafting, is a major development in breast plastic surgery. This technique has a low complication rate, excellent results, and patient acceptance. Radiologic evaluation mostly shows a normal-appearing breast. During breast reconstruction, fat grafting is the ideal complement of the latissimus dorsi flap. Fat grafting for Poland syndrome seems to be a great step and will most likely drastically change the surgical treatment of severe cases. Finally, lipomodeling is a new alternative in the treatment of pectus excavatum, tuberous breasts, and breast asymmetries

[Ultrasonography in normal eyelids assessment].

The article presents the results of a complex study on ultrasonography which included high-resolution grey scale B-scan, color Doppler imaging, and ultrasonic density measurement of eyelids and periorbital tissues. A total of 48 patients were enrolled. Echographic anatomy of eyelid layers, i.e. skin, orbicularis oculi muscle, tarsus, and orbital fat, is described in detail. Depth and ultrasonic density values for all layers are provided. The authors suggest performing an ultrasound examination of eyelids prior to reconstructive and plastic surgery in order to facilitate the choice of surgical

Surgically mismanaged ptosis in a patient with congenital fibrosis of the extraocular muscles type I.

Fibrosis syndromes comprise a rare form of severe limitation of ocular motility. An 11-year-old girl was referred for the correction of eyelid retraction. The eyelid retraction occurred immediately following levator resection surgery performed by a plastic surgeon who missed the restrictive extraocular muscle abnormalities. On examination, both eyes were fixed in an infraducted position (20 prism diopters (Δ)), with a chin-up position and significant lagophthalmos. Bilateral 12-mm inferior rectus recession with adjustable sutures was performed, which resulted in significant reduction of lagoph

A case of Fournier's gangrene after hydrocelectomy.

An uncommon case of Fournier's gangrene following hydrocelectomy is described. A 78-year-old male with no remarkable previous history, who underwent hydrocelectomy in another hospital, developed Fournier's gangrene 15 days later. The patient required wide aggressive surgical debridement, hyperbaric oxygen chamber and broad-spectrum antibiotic coverage. Afterwards the patient was referred for plastic surgery. Fournier's gangrene is a polymicrobial infection of perineoscrotal region that manifests as a rapidly progressive necrotizing fasciitis. Fournier's gangrene following hydrocelectomy is unc

Microtia repair with rib cartilage grafts: a review of personal experience with 1000 cases.

Surgical construction of the auricle with autogenous tissues is a unique marrying of science and art. Although the surgeon's facility with both sculpture and design is imperative, the surgical result is equally influenced by adherence to sound principles of plastic surgery and tissue transfer. The material reviewed in this article is derived from clinical experience with congenital microtia: 1094 completed ears in 1000 patients (94 cases were bilateral). This article focuses on total repair of major congenital ear defects, but includes relevant supplementary input from experience gained by man

Treatment of gingival recession with collagen membranes.

Periodontal plastic surgical techniques to obtain root coverage traditionally have utilized autogenous soft tissue grafts. Guided tissue regeneration (GTR) increasingly is being used as an alternative to soft tissue grafting in attempting root coverage. Resorbable membranes in GTR therapy prevent the need for a second surgery to harvest donor tissue. GTR membranes composed of collagen are effective barriers that add to the thickness of the overlying gingiva and possess platelet-aggregating properties that may help in early wound healing. The cases presented here demonstrate the treatment of gi

Vacuum-assisted closure to treat deep sternal wound infection following cardiac surgery.

Deep sternal wound infection is a devastating and potentially lethal complication following cardiac surgery. Established treatment may involve debridement, packing, delayed closure, plastic reconstruction, re-wiring and irrigation, depending on the severity of infection. The authors prefer early debridement and rewiring with irrigation if possible. If this method fails the results are often poor. Vacuum-assisted closure (VAC) of complex infected wounds has recently gained popularity among various surgical specialties. This article describes the successful application of this novel technique in

Distraction osteogenesis of the midface: a new implantable distraction device.

This report describes the use of a new implantable device for distraction osteogenesis of the maxilla in treatment of midface hypoplasia seen in craniofacial syndromes. The technique of distraction osteogenesis is a recent advance in plastic surgery for treatment of bone growth disorders. The device is totally implantable except for the activating pin and can provide up to 30 mm of total advancement. We will describe our surgical technique and distraction protocol, illustrated by a case report of the device successfully used in a child with Crouzon syndrome.

[Flatfoot].

The flat foot is a very frequent cause of consultation in podology. The lesion is rapidly fixed, and osteoarthrosis appears. The diagnosis is clinical. A podoscopic and radiographic examination is necessary to appreciate the evolutivity of the deformity. The treatment is almost conservative: the wearing of orthesis is often sufficient. The indications of the surgery are rare; sometimes it is possible to do plastic techniques (in particular on the posterior tibial tendon) but, when the foot is painful, the osteoarthrosis is present and it is necessary to do arthrodesis, in particular in the tal

Flap prefabrication using the "vascular crane" principle: an experimental study and clinical application.

Techniques for tissue transfer have continued to evolve. Free flap prefabrication represents a further progression of well known plastic surgery principles in the arena of tissue transfer. This report presents an experimental study demonstrating that the repeated use of a vascular pedicle to prefabricate flaps in the rabbit is possible. There is a decrease in flap viability with the second transfer. A clinical case using this repeated transfer or "crane" principle is also reported. We conclude that the repeated use of a vascular pedicle to prefabricate multiple flaps is a viable option in sele

[The factors influencing the incidence of heart rhythm disorders after the radical correction of the tetralogy of Fallot].

The frequency and factors favouring the development of cardiac rhythm disorders were studied in 306 patients operated on radically for Fallot's tetralogy (follow-up: 6 months--29 years). This complication was found in 99 patients (32.3%). Most patients showed ventricular arrhythmia--62 (62.6%). Paroxysmal tachycardia was noted in 11 patients, atrio-ventricular block in 15 patients, combination of different arrhythmias in 11 patients. Risk factors were: inadequate correction of the malformation, age over 15 at the time of operation, approach through the right ventricle with plastic repair of th

Perspective: three-dimensional imaging of the musculoskeletal system.

An evaluation of musculoskeletal disorders in 202 patients using three-dimensional CT displays has revealed the usefulness of the technique, especially in patients with fractures of skeletal areas with complex anatomy, articular disorders of the hip, and spinal stenosis. In some cases, plastic models of diseased areas have been created from the CT data and are reasonably accurate, providing a graphic representation of the disease and the ability to perform rehearsal surgery. A preliminary investigation of three-dimensional displays of MR images indicates that the technique is feasible, althoug

Hidradenitis suppurativa--a clinical review.

A retrospective review of 72 patients suffering from hidradenitis suppurativa treated surgically in one plastic surgery unit has been carried out. Treatment of the axilla by excision and suture was found to result in a high rate (54%) of reoperation for recurrence at the same site when compared to the groups treated by either excision and split skin grafting (13%) or excision and local flap cover (19%). It is recommended that axillary hidradenitis suppurativa is best treated surgically by excision of the affected skin and split skin grafting of the resultant defect.
