Pentalogy of Cantrell which usually comprises of anomalies of the ectodermal and mesodermal tissues is a very rare congenital condition which in the extreme of cases is incompatible with life. In this report a variant of the condition in a live newborn baby who presented to the University of Nigeria Teaching Hospital with abnormalities of the heart, urinary bladder and sternum is discussed. The aim of the report is to create an awareness and high index of suspicion amongst health professionals.

OBJECTIVE: To report our experience of foreign bodies in the tracheobronchial tree in adults.
DESIGN: Retrospective study.
SETTING: Teaching hospital, Athens.
SUBJECTS: 25 patients referred to the department of thoracic surgery for treatment.
INTERVENTIONS: Bronchoscopic extraction in 16, thoracotomy with bronchotomy in 5, and resection in 4.
RESULTS: No major complications and no deaths.
CONCLUSION: Bronchoscopic extraction is usually possible, but thoracotomy and bronchotomy or resection may be necessary.

The author uses a configurational analysis method for case formulation and to establish links between individualized formulation and treatment techniques. A prototype of formulation for the histrionic personality disorder is presented, using theories for formulation about states of mind, defensive control processes, and person schemas. A phase-oriented prototype of a treatment plan is linked to these levels of formulation. The result can provide a guideline for clinicians and a teaching document for trainees.

This paper sets out some of the issues relating to certification of specialists. It first defines the purpose of certification as assuring the public of the level of competence of specialist practice. It then describes some of the issues involved, which include the assessment of competence, the need for public involvement, the issue of re-certification, and the problem of dealing with poor doctors. It concludes with a challenge to doctors to seize the opportunities and to enhance the quality of teaching and research in medical education.

The authors report the results of a retrospective study of 144 cases of lung abscess observed in the Pneumo-Phthisiology Department of Tokoin University Teaching Hospital of Lomé (Togo) between 1981 and 1993 for to determine epidemiological, clinical, therapeutical and evolutive aspects of this affection on West Africa. The results like those of other authors confirm still grievous evolution nowadays of lung abscess in spite of medical drugs efficacy.

Alfred Parsons (1864-1952) was noted for his vigour of mind and body and Spartan habits and his dramatic teaching. In 1891 while a junior doctor he cared for three fatal cases of acute perforation of a chronic ulcer of the stomach. In May 1892 he made a passionate plea for early diagnosis and surgery in this condition. The first successful operation was performed in the same month by Ludwig Heusner in Germany.

A DOS-compatible model for use on personal computers has been constructed for teaching in parasite epidemiology. The program is based on the Macdonald model of schistosome dynamics and enables simulation of the effect of a control campaign on a human worm load by reducing four transmission factors in the parasite life-cycle: (1) egg contamination, (2) snail lifetime, (3) exposure to cercariae and (4) adult worm lifetime.

Diabetes insipidus is seen most frequently in neurosurgical patients. The transient form is more common and usually subsides prior to discharge. Permanent DI is seen in patients with extensive damage in the hypothalamic area. Treatment of either form of DI is usually with a preparation of vasopressin. Nursing assessements and interventions are crucial to a successful outcome in patients with this disorder. Health teaching for patients with permanent DI is an integral part of the nursing responsibility.

Teaching point: Gallbladder volvulus is a rare but serious diagnosis that needs urgent surgical treatment to avoid morbidity and mortality related to gallbladder ischemia and should be suspected in elderly female patients with acute right upper quadrant pain with imaging features including enlarged, abnormally positioned gallbladder and specific key findings on CT including "whirl sign" representing twisting of the cystic pedicle and "beak sign" at the gallbladder neck.

The air crescent (AC) is a common radiological sign. Even if its commonest aetiology remains pulmonary aspergillosis, various other causes have been described. In this study, we report four rare causes of ACs seen on chest radiographs that haven't been described in the literature. Teaching point: The differential diagnosis of an air crescent sign on chest radiographs includes oesophageal bezoar, interstitial lung emphysema, central bronchial stenosis and perforated emphysematous cholecystitis.

Conventional teaching is that Helicobacter pylori infection is required for the development of non-cardia gastric adenocarcinoma (NCGC) through a sequence of atrophic gastritis, intestinal metaplasia, dysplasia, and finally, cancer.1-3 However, studies have not demonstrated a 100% rate of H pylori infection in gastric malignancy, hypothesized to be due to false negatives, gastric atrophy leading to apparent loss of infection, and inclusion of cardia, nonintestinal cancers.1-3.

With an objective to study the bacteriological profile of neonatal sepsis a retrospective study was conducted in the neonatal unit of a referral teaching hospital in Northern India. Among neonates born over 5-year period (n=22363) incidence of culture-positive sepsis was 7.5/1000 live births (7.5%). Staphylococcus aureus (47.3%), Klebsiella pneumonia (14.9%) and Acinetobacter (14.9%) were most common organisms isolated. Sensitivity pattern of isolated organisms is presented.

A 67 years old man attended surgery OPD of Nepal Medical College and Teaching Hospital with complain of multiple painless swelling all around the body gradually increasing in numbers for last 15 years. Symptomatically some lumps were becoming large, covering skin of those swelling were sloughing with ulceration. Symptomatic lumps were excised. He was clinically diagnosed as Von Recklinghausen Disease. Histopathological report was Neurofibroma with non specific chronic inflammatory reaction.

In a five-year prospective study, 1188 consecutive adult patients were admitted and treated for acute generalized peritonitis at the Surgical Emergency Unit of the Komfo Anokye Teaching Hospital, Kumasi, Ghana. Appendicitis and typhoid ileal perforation were the commonest causes, occurring in 43.1% and 35.1% of patients, respectively. Other conditions (gastroduodenal perforations, ruptured abscesses, traumatic bowel perforations and amoebic colonic perforations) accounted for fewer than 25% of cases.

Twenty-one of 18,000 (0.1%) dermatology patients seen at the Ahmadu Bello University (ABU) Teaching Hospital, Kaduna, Nigeria, during the 9-year period October, 1979-August, 1988, had cutaneous leishmaniasis (CL). Their age range was 14 to 50 years (mean 28.7 years), with a peak at the 14- to 25-year age group. Eleven of the 21 patients (52%) were students in Government Colleges located within the leishmaniasis belt of Nigeria.

The excretory urograms of 70 Nigerian women presenting to the University of Benin Teaching Hospital with obstetric vesicovaginal fistula over a 7-year period are analyzed. The common abnormalities are medial deviation of distal ends of ureters (34%); discrete but smooth pseudoprostatic bladder base indentation (33%); and calyceal clubbing of varying degrees (19%). Other abnormalities observed include hydroureters (18%); bladder calculi (7%); bladder wall calcification (3%), and unilateral nonfunctioning kidneys (3%).

A bacteriological study of vaginal swabs taken from 78 parturient women at Lagos University Teaching Hospital (L.U.T.H.) is presented. 26.83% of the vaginal cultures were sterile. The most frequently isolated bacteria were Staphylococcus spp. 21.4%, followed by beta hemolytic streptococci 16.65%, of which approximately a half belonged to Lancefield group B. Escherichia coli was isolated in 11.1% of the cases. The implications for neonatal and puerperal sepsis are discussed.
