This study aims to investigate the medicolegal aspects of cardiothoracic trauma, including injury patterns, mechanism of infliction, survival time, predictors, and mechanism of mortality. A prospective cohort study included 229 cardiothoracic traumatized patients admitted to the Emergency Departments, who were categorized into patients with fatal or nonfatal injuries. Males constituted more than seven times the females with a 22.3% mortality rate. Intentional, self-inflicted injuries, road traffic accidents (RTAs), and falls from height (FFH) were significantly associated with a higher mortali

BACKGROUND: Chest tube dislodgement is a frequent and potentially life-threatening complication after thoracic trauma. Reliable fixation is essential, particularly in emergency and combat casualty care. Several suture techniques exist, but comparative biomechanical evidence is limited.
METHODS: This prospective biomechanical cadaveric study compared three fixation techniques-Purse String (PS), Roman Sandal (RS), and Modified Johannesburg (JO) technique -across three tube sizes (20, 24, 28 Charrière) and two materials (polyvinyl chloride (PVC) and silicone). A total of 360 tests were performed

INTRODUCTION: Rib fractures are common in blunt chest trauma and are associated with significant morbidity. Although increasing rib fracture count has been linked to worse outcomes, a clear and clinically applicable threshold for complication risk remains uncertain. This study aimed to evaluate the predictive value of rib fracture burden in patients with isolated blunt chest trauma and to identify a clinically meaningful cut-off.
METHODS: This retrospective observational study included adult patients with isolated blunt chest trauma presenting between January 2021 and December 2025 at a terti

Serratus anterior plane block (SAPB) is an effective regional anesthesia technique for chest wall analgesia. In this article, we report three cases of critically ill patients with chest trauma in whom a single-shot SAPB combined with low-dose dexmedetomidine (DEX) was used as part of a multimodal analgesia strategy. In all cases, SAPB with DEX provided effective and prolonged analgesia, with pain relief lasting up to 72 hours. The technique also contributed to improved respiratory function and facilitated clinical decision-making during the weaning process in one patient by helping to distingu

IntroductionWe report the successful use of erector spinae (ESP) plane block in the management of a patient with severe respiratory failure secondary to chest trauma requiring invasive ventilation and Veno-venous extracorporeal membrane oxygenation (V-V ECMO).Case reportA 64-year-old man with flail chest and severe respiratory failure required V-V ECMO. An ESP plane block on day 3 enabled extubation, mobilisation, and secretion clearance, leading to ECMO weaning after six days and discharge 18 days post-injury.DiscussionThis case demonstrating that regional anaesthesia is a safe, feasible opti

BACKGROUND: Tension pneumothorax following trauma is a life-threatening emergency and radiological investigation is normally discouraged prior to treatment in traditional trauma doctrines such as ATLS. Some trauma patients may be physiologically stable enough for diagnostic imaging and occult tension pneumothorax is discovered radiologically. We assessed the outcomes of these patients and compared them with those with clinical diagnosis of tension pneumothorax prior to imaging.
METHODS: A multicentre civilian-military collaborative network of six major trauma centres in the UK collected obser

INTRODUCTION: Despite the high incidence of blunt thoracic trauma and frequently performed conservative treatment, studies on very long-term consequences for these patients remain sparse in current literature. In this study, we identify prevalence of long-term morbidity such as chronic chest pain, shortness of breath, and analyze the effect on overall quality of life and health-related quality of life.
METHODS: Questionnaires were send to patients admitted for blunt thoracic trauma at our institution and who were conservatively treated between 1997 and 2019. We evaluated the presences of curr

Delayed cardiac tamponade following blunt chest trauma is a rare incident, and we provide a report including a literature review for further discussion. The patient is a 56-year-old male. He fell off a motorcycle and sustained contusions to the left side of his chest. He presented to our emergency department. His consciousness was clear and vital signs were normal. Computed tomography (CT) scan revealed multiple left rib fractures, left hemothorax, and left pulmonary contusion, but no pericardial effusion was detected. He returned home, and stayed at rest for a few days. He walked for 15 minut

PURPOSE: We aimed to evaluate the accuracy and reproducibility of the CT-based volume estimation formula V = d2 * h, where d and h represent the maximum depth and height of the effusion, for acute traumatic hemothorax.
MATERIALS & METHODS: Prospectively identified patients with CT showing acute traumatic hemothorax were considered. Volumes were retrospectively estimated using d2 * h, then manually measured on axial images. Subgroup analysis was performed on borderline-sized hemothorax (200-400 mL). Measurements were repeated by three non-radiologists. Bland-Altman analysis was used to assess

BACKGROUND: In open thoracoabdominal aortic aneurysm (TAAA) repair, we have been performing vascular reconstruction under moderate to deep hypothermia and assisted circulation using simultaneous upper and lower body perfusion. This method is effective for protecting the spinal cord and the brain, heart, and abdominal organs and for avoiding lung damage.
METHODS: TAAA repair was performed under hypothermia at 20-28 °C in 18 cases (Crawford type I in 0 cases, type II in 5, type III in 3, type IV in 4, and Safi V in 6) between October 2014 and January 2023. Cardiopulmonary bypass was conducted b

INTRODUCTION: Two trauma treatment principles are Early Total Care (ETC), and Damage Control Orthopedics (DCO). Cellular mechanisms that underlie the connection between treatment type, its systemic effects, and tissue regeneration are not fully known. Therefore, this study aimed to: 1) profile microRNA (miRNA) expression in plasma derived Extracellular Vesicles (EVs) from a porcine multiple trauma model at different timepoints, comparing two surgical treatments; and 2) determine and validate the miRNA's messengerRNA (mRNA) targets.
METHODS: The porcine multiple trauma model consisted of blunt

PURPOSE: Resuscitative thoracotomies (RT) are the last resort to reduce mortality in patients suffering severe trauma. In recent years, indications for RT have been extended from penetrating to blunt trauma. However, discussions on efficacy are still ongoing, as data on this rarely performed procedure are often scarce. Therefore, this study analyzed RT approaches, intraoperative findings, and clinical outcome measures following RT in patients with cardiac arrest following blunt trauma.
METHODS: All patients admitted to our level I trauma center's emergency room (ER) who underwent RT between 2

BACKGROUND: The recovery of autonomic functions and the ability to reproduce in particular is of the highest priority to individuals with spinal cord injury (SCI). The potential of epidural spinal cord stimulation (ESCS) for promoting recovery of sensorimotor functions in the chronic phase of SCI has long been studied. In recent years, several studies have emerged confirming the positive effect of ESCS also on the cardiovascular system and neurogenic bladder and bowel. However, the potential of ESCS in restoring sexual function, especially ejaculation, has not yet been addressed.

BACKGROUND: Commotio cordis is an event in which a blunt, non-penetrating blow to the chest occurs, triggering a life-threatening arrhythmia and often sudden death. This phenomenon is often seen in young, male athletes and has become increasingly well-known over the past few decades. We present a unique case in which ventricular fibrillation occurs in an older male athlete after blunt trauma.
CASE PRESENTATION: Patient with no known medical history was brought to the ER after being found unconscious after a soccer ball kick to the chest. He was found to be in ventricular fibrillation and succ

BACKGROUND: Traumatic colon injury (TCI) is a common disease during wartime. Prolongation of posttraumatic survival time is an effective approach to patient outcome improvement. However, there is a lack of basic research in this field. This study aimed to elucidate the mechanisms underlying TCI progression and to develop novel regimens to buy time for TCI patients on the battlefield.
METHODS: A total of 669 Sprague-Dawley rats were used in this study. Surgical colon incision was performed to generate the TCI rat model. The landscape of colon microbiota compositions was depicted using 16S rRNA

BACKGROUND: Transvenous pacemaker and defibrillator lead extraction is a higher risk procedure with variation in preferred technique. A frequently fatal complication of this procedure is perforation of the superior vena cava. We have developed a tandem femoral-superior technique that incorporates snaring of targeted leads from a femoral approach combined with use of a rotational cutting sheath advanced over the lead from the subclavian vein.
OBJECTIVE: We sought to evaluate the safety and efficacy of a tandem femoral-superior approach to lead extraction.
METHODS: Consecutive patients undergo

Spontaneous pneumomediastinum (SPM) is an uncommon and usually benign self-limiting clinical disorder found in young people, often without apparent precipitating factors or diseases. A pressure gradient exists between the peripheral pulmonary alveoli and the hilum, and increased intra-alveolar pressure causes rupture of the terminal alveoli. We present the case of a 15-year-old male soccer player who presented with a complaint of anterior chest pain and dysphagia after stopping the strong ball with his chest. His symptom gradually progressed over hours. We can make the diagnosis of SPM using b
