Epithelial downgrowth. Report of 2 cases diagnosed by ocular biopsy.

Epithelial downgrowth is an uncommon complication of ocular surgery or adverse consequence of accidental trauma, caused by surface squamous epithelium that has gained access to the inner compartments of the eye. Once embedded in the eye, squamous epithelium spreads over contiguous structures, interfering with normal aqueous outflow and vision. The pursuit of improved therapies is ongoing, but the greatest chance of preventing blindness is with early recognition. Two cases of epithelial downgrowth diagnosed from a corneal button and an iris biopsy are presented to familiarize pathologists with

Current Treatment Modalities of JIA-associated Uveitis and its Complications: Literature Review.

Uveitis is a common and serious complication of juvenile idiopathic arthritis. Up to 75% of all cases of anterior uveitis in childhood are associated with juvenile idiopathic arthritis. Despite the remarkable progress in early detection and treatment of inflammation, vision-threatening complications of uveitis still occur in almost 60% of patients. Structural complications include band keratopathy, maculopathy (macular edema, macular cysts, and epiretinal membrane), glaucomatous optic neuropathy, and cataracts. The management of complications in juvenile idiopathic arthritis is usually complex

[Current status and future prospect of uveitis research in China].

Uveitis, as one of the major cause of blindness worldwide, seriously harms patients' vision and quality of life. The social and economic burden caused by uveitis cannot be ignored. Remarkable achievements in basic and clinical uveitis research were gained after decades of unremitting efforts by Chinese ophthalmologists. A number of original results and breakthroughs were achieved in researches on Behcet's disease, VKH syndrome and other important types of disease that lead to blindness. We are integrating the power of the Chinese Ocular Immunology Association, conducting multi-center collabora

Ocular histoplasmosis syndrome.

Ocular histoplasmosis syndrome (OHS) is a chorioretinal disorder with a distinct fundus appearance that is commonly found in regions endemic for Histoplasma capsulatum. Choroidal neovascularization (CNV) secondary to OHS is considered one of the principal causes of central vision loss among young adults in endemic areas. Although there is no consensus regarding its pathogenesis, evidence points to Histoplasma capsulatum as the most probable etiology. Once considered an intractable hemorrhagic maculopathy, CNVs are now treatable. Extrafoveal CNVs are successfully treated with laser photocoagula

Keratoprostheses for corneal blindness: a review of contemporary devices.

According to the World Health Organization, globally 4.9 million are blind due to corneal pathology. Corneal transplantation is successful and curative of the blindness for a majority of these cases. However, it is less successful in a number of diseases that produce corneal neovascularization, dry ocular surface and recurrent inflammation, or infections. A keratoprosthesis or KPro is the only alternative to restore vision when corneal graft is a doomed failure. Although a number of KPros have been proposed, only two devices, Boston type-1 KPro and osteo-odonto-KPro, have came to the fore. The

Primary tubercular chorioretinitis.

The aim of this report is to emphasize that corticosteroids should not be prescribed when the etiological factors remain unidentified. A 34-year-old male visited our ophthalmology clinic suffering from blurred vision. Behçet's disease had been diagnosed 5 years prior, and the patient was taking an oral immunosuppressant and a systemic corticosteroid. Vitreous cell count and foci of chorioretinitis (apparently confined to the ocular fundus) were evident. A vitreous sample was subjected to polymerase chain reaction, which resulted in the identification of and Mycobacterium tuberculosis. Treatmen

[Ocular manifestations in Behçet's disease].

Uveitis is the most common ocular symptom in Behçet's disease. It is considered as a diagnostic criterion of the disease. Anterior uveitis is always non granulomatous, and sometimes associated with hypopion. Posterior involvement may include vitritis, retinal infiltrates, sheathing of retinal veins, occlusive vasculitis, and macular edema. Behçet's disease is chronic, and characterized by a spontaneously relapsing and remitting course. Male patients with younger age at onset and worse visual acuity at presentation, have higher risk of visual loss over time. The main goals in the management of

Refractory pituitary granulomatosis with polyangiitis (Wegener's) treated with rituximab.

OBJECTIVE: Granulomatosis with polyangiitis (GPA), also known as Wegener's granulomatosis, is an autoimmune disease characterized by inflammation of blood vessels most often seen in the upper respiratory tract, lungs, kidneys, and skin. Central nervous system (CNS) involvement of GPA is rare, particularly in the pituitary, and can be difficult to treat. METHODS: Case report. RESULTS: We present a 30-year-old woman with pituitary and ocular GPA, whose unusually recalcitrant disease led to the development of pan-hypopituitarism and near-total vision loss. After failing multiple systemic immuno

Thymosin β4: a potential novel dry eye therapy.

The purpose of this manuscript is to review the clinical entity of dry eye syndrome (DES) and to provide a scientific basis and rationale for the usage of thymosin beta 4 (Tβ4) as a novel therapy for DES. DES is a common disorder affecting an estimated 25-30 million people in the United States alone and is characterized by inflammation of the ocular surface. Consequently, patients can suffer from burning, irritation, severe discomfort, foreign body sensation, and blurry and decreased vision. Recent animal studies of DES demonstrate that Tβ4 eye drops significantly reduce corneal fluorescein st

[Expression and function of ICOS on CD4 T cells and application to therapy in patients with ocular Behçet's disease with uveitis].

Uveitis in Behçet's disease (BD) is an important cause of vision loss through recurrent ocular inflammatory attacks. CD4 T cells that react to various self-antigens play a central role in the pathogenesis of this disease, and the activation or inactivation of the CD4 T cells is controlled by costimulatory molecules. In Behçet's disease patients with active uveitis, among the various costimulatory molecules, the inducible costimulator (ICOS), an active form of stimulatory molecule, is most strongly expressed, and is associated with cytokine production by Th1 and Th17 cells. It is suggested that

Orbital fractures: role of imaging.

The orbit may be injured directly or indirectly. Blunt and penetrating trauma occurs with equal frequency. Soft tissue swelling often obscures direct clinical evaluation of the globe, limits ocular motion, and may limit clinical assessment of vision. Plain film radiographs of the orbits and sinuses are rarely used for diagnosis in orbital trauma. Computed tomography is considered the imaging modality of choice in this circumstance, as it is deemed to be the most accurate method in detecting fractures. The protocol is based on obtaining thin-section axial scans and multiplanar reformatted image

Isolated sixth cranial nerve palsy as the presenting symptom of a rapidly expanding ACTH positive pituitary adenoma: a case report.

BACKGROUND: Pituitary adenoma may present with neuro-ophthalmic manifestations and, typically, rapid tumor expansion is the result of apoplexy. Herein, we present the first case of an isolated sixth cranial nerve palsy as initial feature of a rapidly expanding ACTH positive silent tumor without apoplexy. CASE PRESENTATION: A 44 year old female with a history of sarcoidosis presented with an isolated sixth cranial nerve palsy as the initial clinical feature of a rapidly expanding ACTH positive silent pituitary adenoma. The patient underwent emergent transsphenoidal hypophysectomy for this rapi

Molecular wipes: application to epidemic keratoconjuctivitis.

Epidemic keratoconjunctivitis (EKC) is a severe disease of the eye, caused by members of the Adenoviridae (Ad) family, with symptoms such as keratitis, conjunctivitis, pain, edema, and reduced vision that may last for months or years. There are no vaccines or antiviral drugs available to prevent or treat EKC. It was found previously that EKC-causing Ads use sialic acid as a cellular receptor and demonstrated that soluble, sialic acid-containing molecules can prevent infection. In this study, multivalent sialic acid constructs based on 10,12-pentacosadiynoic acid (PDA) have been synthesized, an

[Peter's anomaly, clinical and therapeutic aspects: case report].

Peter's anomaly consists in the most common congenital corneal opacity related to a malformation of the anterior segment of the eye. Its main characteristics are central leukoma and iridocorneal adherences at the area affected by the leukoma. It can be identified isolated or in association with other ocular or systemic abnormalities, and the prognostic tend to be worse in the latter cases. The etiology of Peter's anomaly remains uncertain, but the most likely causes are related to genetic, infectious, traumatic and toxic factors. A range of possible treatment strategies exists, though the effe

Surgical flap amputation for central flap necrosis after laser in situ keratomileusis.

A 52-year-old man had uneventful bilateral myopic laser in situ keratomileusis (LASIK) with planned monovision. Three months later, the left eye was retreated to improve the near vision. On postoperative day 1, the uncorrected distance visual acuity (UDVA) was 0.95 and the patient reported mild ocular pain. Stage 2 diffuse lamellar keratitis was observed. Despite hourly prednisolone instillation, the UDVA decreased to 0.1 and central flap necrosis syndrome was diagnosed. The flap was lifted and stromal bed irrigation performed. One month later, the UDVA was 0.05 with a hyperopic shift of +3.25

Visual plasticity and its clinical applications.

Normal visual development requires: 1) environmental factors (i.e. sensory experience) and 2) molecular programs that are genetically determined. Experience determines the development and preservation of visual cortical circuitry in accordance with Hebb's principle. The molecular and genetic mechanisms that regulate visual plasticity are less known. Visual experience induces postnatal neural activity that triggers a cascade of molecular processes including release of neurotrophic factors from target neurons and genetic expression of protein synthesis, transcription factors and neurotransmitter
