In a qualified detoxication, states of anxiety caused by hallucinations can be specifically assimilated and psychologically reduced. Knowledge of the hallucinations experienced promotes the subsequent motivation phase. Fetal alcohol exposure is associated with slower reaction times (RTs) in children, suggesting an alcohol-related deficit in "speed of central processing." This study examined effects of prenatal alcohol exposure on a new paradigm which, for the first time, directly assesses RT in infancy. Thirty six-patients with inoperable cancers of the oesophagus or gastric tumour in the cardia were treated by endoscopic alcohol injection. After dilatation using Savary dilators, absolute alcohol was injected in 0.5-1 ml aliquots into protuberant parts using a sclerotherapy needle. The mean volume per session was 7.8 ml. We examined the management and outcome of 15 patients admitted consecutively to a specialist Liver Unit with severe alcoholic hepatitis complicated by renal failure. Fourteen patients were managed conservatively. Of these, 11/14 underwent renal dialysis for a mean of 24 days. Three patients, including two requiring dialysis, recovered renal function.
