[Possibilities of traffic-injury prevention using seat belts and their disadvantages].

As a result of the increasing number of serious injuries, and/or deaths caused by automobile accidents, seatbelts, along with other safety items are installed in cars. The idea of having automobile seatbelts came from the aircraft seatbelt. In our country there is no law requiring installation of seatbelts in all automobiles. Most imported cars, however do have built-in seat builts which are more or less used by the drivers and passengers. Wearing seatbelts is a necessity because, according to statistics, of 100,000 registered vehicles, 340.8 drivers and/or passengers lose their lives. In Swed

Ilizarov Circular Fixation for High-Velocity Firearm Injuries Causing High-Energy Tibial Fractures: A Prospective Cohort Study.

High-velocity gunshot tibial fractures present major reconstructive challenges owing to extensive contamination, soft-tissue devitalisation, and segmental bone loss that render early internal fixation hazardous. We aimed to document outcomes of a structured Ilizarov-based protocol for this injury type and describe a reproducible treatment framework for resource-constrained settings. Between January 2019 and December 2023, we prospectively enrolled 20 adult male patients with open tibial fractures from high-velocity firearm injuries at three Cairo trauma centres; all were treated definitively w

[Pilon Tibial Fractures - What Can We Achieve?].

Pilon fractures, as intra-articular fractures of the distal tibia involving the ankle joint, represent one of the most challenging injuries in trauma surgery. This review analyzes current treatment concepts and their outcomes based on a narrative review. Pilon fractures account for 3-10% of all tibial fractures and typically result from axial compression mechanisms in high-energy trauma or torsional forces in low-energy injuries. The complex anatomy of the tibial plafond with its limited vascular supply and thin soft tissue coverage considerably complicates treatment. Established classificatio

Global epidemiology, burden, and causes of lower extremity and pelvic fractures in the past 32 years.

BACKGROUND: Lower extremity and pelvic fractures (LEPFs) are common and debilitating injuries with substantial global health and economic burden, yet comprehensive epidemiological data remain limited. METHODS: Based on Global Burden of Disease 2021, we analyzed incidence, years lived with disability (YLDs), and causes of LEPFs across 204 countries from 1990 to 2021, along with their temporal trends. The impact of age, sex and Socio-Demographic Index (SDI) was considered. RESULTS: In 2021, 78.05 million new LEPF cases were reported globally, a 32% increase since 1990. Despite this, the age-st

Associations of musculoskeletal dysfunction, time to brake, and patient reported outcomes following lower extremity trauma.

INTRODUCTION: Inability to drive a motor vehicle due to lower extremity injury is a major inconvenience. Timing of safe return to driving has not been much studied. Objective measurements such as time to brake (TTB) have been proposed; however, utility and concordance of functional outcome measurements have not been previously evaluated. The purpose of this project is to measure these parameters and to assess for associations with ability to safely return to driving, improving the ability of clinicians to assess for measurements of driving readiness without specifically measuring TTB. METHODS

Predictors of health-related quality of life following injury in childhood and adolescence: a pooled analysis.

BACKGROUND: Injury is a leading contributor to the global disease burden in children and places children at risk for adverse and lasting impacts on their health-related quality of life (HRQoL) and development. This study aimed to identify key predictors of HRQoL following injury in childhood and adolescence. METHODS: Data from 2259 injury survivors (<18 years when injured) were pooled from four longitudinal cohort studies (Australia, Canada, UK, USA) from the paediatric Validating Injury Burden Estimates Study (VIBES-Junior). Outcomes were the Paediatric Quality of Life Inventory (PedsQL) tot

Conservative management of isolated medial subtalar joint dislocations in volleyball players: a report of three cases and literature review.

Isolated subtalar dislocations (SDs) are rare injuries, representing only 1% of all foot traumas. In the current literature, only a few reports have described this acute injury as a consequence of low-middle-energy trauma during sports activities and none in professional or recreational volleyball players. Further, to the best of our knowledge, no validated standard rehabilitation programs have been described for SDs as most of them are usually treated like an ankle sprain. This report describes 3 cases of isolated, closed medial SD, which occurred during non-professional volleyball activities

PURPOSE: To develop and evaluate a multifactorial model based on landing performance to estimate injury risk for surfing athletes. METHODS: Five measures were collected from 78 competitive surfing athletes and used to create a model to serve as a screening tool for landing tasks and potential injury risk. In the second part of the study, the model was evaluated using junior surfing athletes (n = 32) with a longitudinal follow-up of their injuries over 26 wk. Two models were compared based on the collected data, and magnitude-based inferences were applied to determine the likelihood of differe

Combined medial column primary arthrodesis, middle column open reduction internal fixation, and lateral column pinning for treatment of Lisfranc fracture-dislocation injuries.

Lisfranc fracture-dislocation can be a devastating injury with significant long-term sequelae, including degenerative joint disease, progressive arch collapse, and chronic pain that can be potentiated if not effectively treated. We present a case to demonstrate our preferred surgical approach, consisting of combined medial column primary arthrodesis, middle column open reduction internal fixation, and lateral column pinning, with the primary goal of minimizing common long-term complications associated with Lisfranc injuries. We present the case of a typical patient treated according to this co

Minimally invasive osteosynthesis of ankle fractures.

BACKGROUND: In this publication, we present a technique for internal ankle fracture fixation through minimally invasive surgical access. MATERIAL/METHOD: Observations were carried out in a group of 17 patients (9 men and 8 women) treated due to closed injuries. Fractures were reduced under fluoroscopic guidance and stabilized percutaneously with 2 or 3 Kirschner wires (lateral malleole), 1-2 malleolar screws (medial malleole) and cortical screws (syndesmosis). If displaced or involving articular surfaces, posterior tibia was reduced and stabilized with cortical screws. RESULTS: We compared t

Successful use of reaming and intramedullary nailing of the tibia.

The records and radiographs of 87 patients with 88 fractures of the tibial diaphysis who underwent intramedullary reaming and nailing were retrospectively reviewed. The mean patient age was 37 years. Seventy fractures were closed, and 18 were open. The indications for nailing were failures of closed treatment (n = 48), nonunions (n = 24), multiple injuries (n = 14), and fixation of osteotomy (n = 2). Seventy-six patients with 77 nails who were followed for an average of 26.2 months were studied. For acute fractures, the average time to complete clinical and radiographic union was 5.3 months. F

Locking Nail versus Plate Fixation in Calcaneal Fractures: Brief report on a Retrospective Analysis of Treatment Characteristics and Radiographic Correction Potential.

PURPOSE OF THE STUDY Minimal and limited access techniques are gaining increasing interest for the treatment of displaced intra-articular calcaneal fractures. The ideal treatment method is however still debated and largely based on individual case factors and surgeon experience. Aim of this study was thus to compare the treatment characteristics and radiographic correction potential of a locking nail system with a sinus tarsi approach to plate fixation via an extended lateral approach. MATERIAL AND METHODS We retrospectively reviewed 39 cases of patients with calcaneal fractures that received
