The Diagnostic Challenge of Rapunzel Syndrome: Multimodal Imaging Findings in a Child.

We report the case of a school-aged patient with attention-deficit/hyperactivity disorder who presented with a palpable epigastric mass. The initial abdominal ultrasonography indicated the presence of a heterogeneous space-occupying lesion in the upper abdomen. Subsequent computed tomography revealed a large intragastric mass with a mottled air-containing density, an imaging feature characteristic of a bezoar. Esophagogastroduodenoscopy confirmed a massive trichobezoar extending beyond the pylorus into the duodenum, consistent with Rapunzel syndrome. Although the endoscopic removal proved unsu

The predictive utility of conduct disorder symptoms in preschool children: a 3-year follow-up study.

Conduct disorder (CD) symptoms often emerge during the preschool years, but it is not clear whether they predict later symptoms. The present study examined whether age 3 CD symptoms predict age 6 CD symptoms beyond oppositional defiant disorder (ODD) and attention-deficit/hyperactivity disorder-hyperactive/impulsive (ADHD-HI) symptoms. Participants were 216 preschool children (M Age  = 44.19 months), including an externalizing sample (n = 161) and a comparison group (n = 55). Parents were administered a diagnostic interview when children were 3 years old and again 3 years later. The externaliz

Bilateral congenital glaucoma in a child with Nicolaides-Baraitser syndrome: a case report.

INTRODUCTION AND IMPORTANCE: Nicolaides-Baraitser syndrome (NCBRS) is a rare autosomal dominant disease characterized by developmental delay, distinctive craniofacial features, sparse hair, and is caused by de novo mutations in the SMARCA2 gene. CASE PRESENTATION: We report the case of an 11-year-old male with NCBRS presenting with bilateral congenital glaucoma. The diagnosis was based on clinical presentation, such as attention deficit hyperactivity disorder, cognitive delay, and characteristic craniofacial features, and confirmed by the presence of a de novo mutation in the SMARCA2 gene thr

A case of mood disorder with severe side effects of antidepressants in association with resistance to thyroid hormone beta with a THRB mutation.

BACKGROUND: Although resistance to thyroid hormone beta (RTHβ) is associated with attention-deficit/hyperactivity disorder, there are few reports of other concomitant mood disorders in individuals with RTHβ. CASE PRESENTATION: A 67-year-old woman who had been previously diagnosed with RTHβ (Refetoff syndrome) came to our department as a depressed patient. She was hospitalized twice for depression and treated with antidepressants both times. Paroxetine (37.5 mg/day) treatment during the first hospitalization did not cause any side effects, but treatment with mirtazapine (15 mg/day) and venlafa

Screening for reading disability in university students with phonological processing and working memory tasks.

Many students in higher education have undiagnosed reading disabilities (RDs), but there are few measures to screen for RD in this population. The aim of this study was to determine the ability of tasks that are sensitive to RDs-such as measures of phonemic awareness and working memory-to differentiate university students previously diagnosed with RDs from controls. Participants were university students with an RD (n = 26), a clinical control group diagnosed with attention-deficit/hyperactivity disorder (n = 24), and neurotypical controls (n = 44). Participants completed brief phonological pro

Acute disseminated encephalomyelitis associated with enteroviral infection.

Acute disseminated encephalomyelitis (ADEM) is an acute demyelinating infection of the CNS that is characterized by multifocal white matter involvement often following an infection and vaccination. A 16-year-old child receiving risperidone for 7 years for the treatment of attention-deficit/hyperactivity disorder was hospitalized with complaints of double-vision during the past month and weakness on his right side, ataxia, and vomiting over the past 3 days. The child was diagnosed with ADEM after clinical, laboratory, and cranial MRI was conducted. Following an initial 3-day therapy with pulsed

[Oral healthcare for the persons with special needs.].

The field of special care dentistry is rapidly gaining recognition as a service that should be provided to the persons with physical, mental or intellectual disabilities by general practitioner, pediatric or geriatric dentists as well as dental hygienists. Because the oral healthcare and dental treatments are given in the narrow space of oral cavity and also accompanying technical difficulties with high risk of injury, the special needs patients are treated under being controlled their behavior or body motion by applying psychological, physical or pharmacological techniques. Those persons also

Which factors impact on clinician-rated impairment in children with ADHD?

OBJECTIVES: To describe the associations between a range of demographic, family and clinical factors and clinician-rated measures of global impairment in children with ADHD symptoms obtained at the baseline assessment in the ADHD Observational Research in Europe (ADORE) study. METHODS: Global impairment was measured by clinicians using the Clinical Global Impression-Severity (CGI-S) scale and the Children's Global Assessment Scale (CGAS). Associations with independent variables were investigated using forward-stepwise regression models. RESULTS: For the CGI-S and CGAS analyses, complete data

The Association between Autism Spectrum Disorder and Precocious Puberty: Considering Effect Modification by Sex and Neuropsychiatric Comorbidities.

Limited knowledge is available about the association between autistic spectrum disorder (ASD) and precocious puberty. Our study examined the association between the two medical conditions and effect modification by sex and neuropsychiatric comorbidities in a nationwide population. To compare the risk of precocious puberty between ASD and non-ASD cases, we conducted a Cox regression analysis using ASD as the exposure and time to precocious puberty as the outcome. We adjusted for sex, attention-deficit/hyperactivity disorder (ADHD), tic disorder, obsessive-compulsive disorder (OCD), anxiety diso

Prevalence, clinical correlates and maternal psychopathology of deliberate self-harm in children and early adolescents: results from a large community study.

OBJECTIVES: Little is known about the prevalence and correlates of deliberate self-harm (DSH) in children from low- and middle-income countries. We investigated the prevalence of DSH and its clinical and maternal psychopathological associations in Brazilian children (n=2,508, ages 6-14y) in a community-based study. METHODS: Participants of the High Risk Cohort Study for the Development of Childhood Psychiatric Disorders (HRC) and their mothers were assessed in structured interviews. Current (last month) and lifetime DSH were estimated, including analysis stratified by age groups. Logistic reg

Elevated Rates of Gastrointestinal Dysfunction in Children with Neurodevelopmental Disabilities: Not Just an Autism Issue.

Gastrointestinal (GI) conditions are common among children with neurodevelopmental disabilities (NDDs), and are associated with functional impairment, behavioral symptoms, and increased health care utilization. A unique relationship between autism and GI dysfunction has been proposed, leading to a focus on autism in GI research, management guidelines, and clinical tool development. Leveraging >20 years of electronic health record data and a cohort of 42,204 cases with attention-deficit/hyperactivity disorder, autism, cerebral palsy, epilepsy, or intellectual disability and 297,402 controls wit

ADHD and Sex Differences in School Bullying Victimization and Perpetration Based on Inverse Probability Treatment Weighting.

Adolescents with attention-deficit/hyperactivity disorder (ADHD) exhibit heightened vulnerability to school bullying, with sex-specific patterns. This study analyzed data from the 2021 Korean National Survey on Mental Health of Youth, comprising 6,689 adolescents (ages 12-18; 50.9% male). After inverse probability of treatment weighting (IPTW) adjustment, the effective sample comprised 3,1789 adolescents at risk of ADHD and 3,511 controls. Logistic regression analyses revealed pronounced sex differences. Female adolescents with ADHD symptoms showed significantly elevated odds of overall victim

Evaluating Large Language Models' Ability Using a Psychiatric Screening Tool Based on Metaphor and Sarcasm Scenarios.

Metaphors and sarcasm are precious fruits of our highly evolved social communication skills. However, children with the condition then known as Asperger syndrome are known to have difficulties in comprehending sarcasm, even if they possess adequate verbal IQs for understanding metaphors. Accordingly, researchers had employed a screening test that assesses metaphor and sarcasm comprehension to distinguish Asperger syndrome from other conditions with similar external behaviors (e.g., attention-deficit/hyperactivity disorder). This study employs a standardized test to evaluate recent large langua

Genotypic and phenotypic variability of 22q11.2 microdeletions - an institutional experience.

Patients with chromosome 22q11.2 deletion syndromes classically present with variable cardiac defects, parathyroid and thyroid gland hypoplasia, immunodeficiency and velopharyngeal insufficiency, developmental delay, intellectual disability, cognitive impairment, and psychiatric disorders. New technologies including chromosome microarray have identified smaller deletions in the 22q11.2 region. An increasing number of studies have reported patients presenting with various features harboring smaller 22q11.2 deletions, suggesting a need to better elucidate 22q11.2 deletions and their phenotypic c

Guanfacine overdose resulting in initial hypertension and subsequent delayed, persistent orthostatic hypotension.

INTRODUCTION: Guanfacine is an alpha(2)-adrenoreceptor agonist used for the treatment of attention-deficit hyperactivity disorder and tic disorders. Few reports exist regarding overdose with guanfacine. CASE REPORT: A 16-year-old female with Tourette's syndrome presented with diaphoresis, dry mouth, and hypertension 8 h after ingesting 25 mg of guanfacine. These symptoms improved and her blood pressure (BP) normalized over 2 h. Thirty hours following ingestion, she experienced near syncope and returned to the emergency department. She was noted to have orthostatic hypotension and a prolonged

From reinforcement learning models to psychiatric and neurological disorders.

Over the last decade and a half, reinforcement learning models have fostered an increasingly sophisticated understanding of the functions of dopamine and cortico-basal ganglia-thalamo-cortical (CBGTC) circuits. More recently, these models, and the insights that they afford, have started to be used to understand important aspects of several psychiatric and neurological disorders that involve disturbances of the dopaminergic system and CBGTC circuits. We review this approach and its existing and potential applications to Parkinson's disease, Tourette's syndrome, attention-deficit/hyperactivity d

Rate and predictors of divorce among parents of youths with ADHD.

Numerous studies have asserted the prevalence of marital conflict among families of children with attention-deficit/hyperactivity disorder (ADHD), but evidence is surprisingly less convincing regarding whether parents of youths with ADHD are more at risk for divorce than are parents of children without ADHD. Using survival analyses, the authors compared the rate of marital dissolution between parents of adolescents and young adults with and without ADHD. Results indicated that parents of youths diagnosed with ADHD in childhood (n = 282) were more likely to divorce and had a shorter latency to

Keep taking the tablets? Evidence-based approaches to AD(H)D. Part 2: Audit and changing practice.

The audit cycle was used to look at our practice in the light of new developments in the literature and the new evidence available to us. Using this new information we were able to refine our practice, set new standards and measure our practice against these new standards. This new information allowed us to further develop new ways of making our assessment and to implement this into our practice. We still need to continue the audit cycle by revisting the process, and auditing other parts of our service to children and young people diagnosed as having Attention Deficit (Hyperactivity) Disorder.

Inappropriate uses of stimulant medications.

The utility of stimulant medications in the treatment of attention deficit disorder (ADD) has been demonstrated repeatedly. Over 500,000 children receive these medications in the United States each year. The drugs, however, may be used inappropriately in cases where a presumptive diagnosis of "hyperactivity" is mistakenly made on the basis of nonspecific increases in activity levels or when an initial sedative response is taken as diagnostic confirmation of ADD. In this series of six cases, stimulant medications had been used inappropriately to manage a range of developmental problems. The imp

Editorial: Better Treatments and the Importance of Publishing Negative Clinical Trials.

We need better treatments for children with mental health problems, particularly those struggling with depression and suicidality. Every reader of this editorial already knows this. Although our best treatments help many children and adolescents, some experience only modest improvements such that they are still burdened with much of the weight of mental illness. And some experience no benefit at all. These conclusions are supported by two recent meta analytic studies. In a comprehensive meta-analysis of the efficacy of treatments for anxiety, attention-deficit/hyperactivity disorder, conduct p
