Investigation of factors related to successful puncture of the intrahepatic bile duct in endoscopic ultrasound-guided biliary drainage.

BACKGROUND: The factors associated with the success of each step of EUS-BD have not been fully explored. This study aimed to identify the factors associated with successful bile duct puncture for EUS-BD. METHODS: We retrospectively reviewed the data of patients with malignant biliary obstruction who underwent EUS-BD at a single institution between 2015 and 2025. RESULTS: EUS-BD was used to perform 46 bile duct punctures on 33 patients. The success rate of bile duct puncture was 80.4%. According to multivariate analysis, the angle of the scope and the puncture needle on the EUS monitor was id…

Bio-Memristor Based on Clinical Bile Samples: A Proof-of-Concept Pilot Study for Detection of Gallbladder Cancer.

Gallbladder cancer (GBC) is the most common malignant tumor of the biliary system, and the 5-year survival rate of GBC patients is only 5% due to delayed diagnosis. The sensitivity and specificity of existing clinical testing equipment for gallbladder malignancy remain insufficient. In this proof-of-concept pilot study, we developed a bio-memristor featuring an Ag/bile/fluorine-doped tin oxide structure using clinically sourced bile samples from a small clinical cohort and characterized its resistive switching behavior. The memristive electrical signatures, particularly negative peak current a…

Laparoscopic management of a rare intrahepatic bile duct variation in a patient with cholelithiasis: a case report and review of the literature.

BACKGROUND: Anatomical variations of the biliary system constitute one of the principal risk factors for bile duct injury (BDI) during laparoscopic cholecystectomy (LC). Preoperative imaging modalities, such as abdominal ultrasonography and magnetic resonance cholangiopancreatography (MRCP), play a pivotal role in identifying potential biliary anatomical variations. In addition, meticulous adherence to standardized surgical techniques during LC serves as a critical safeguard against BDI. Therefore, heightened awareness and comprehensive understanding of biliary anatomical variations are essent…

Bile acid scaffold engineering reveals an androstane-triol derivative as a potent immunomodulator with therapeutic efficacy in EAE.

BACKGROUND: Neuroinflammation driven by dysregulated adaptive and innate immune responses plays a central role in the pathogenesis of multiple sclerosis and related autoimmune disorders of the central nervous system. While bile acids are increasingly recognized as endogenous immunomodulators, their therapeutic exploitation has been limited by modest potency and incomplete mechanistic understanding. Here, we report the rational engineering of a bile acid-derived scaffold that yields a potent small molecule immunomodulator with therapeutic efficacy in experimental autoimmune encephalomyelitis, a…

Distinct Interfacial Behavior of Bile Salts and Its Interaction with Cetylpyridinium Chloride Cosurfactant at the Air/Water Interface.

Bile salts have received considerable interest in pharmaceutical formulations for increased bioavailability of the drug. Peculiar physicochemical properties of bile salts and their interface-specific interactions with cosurfactants are believed to be crucial for such formulations. Here, we have investigated the interfacial properties of sodium deoxycholate (NaDC; bile salt) at the air/water and air/water-cetylpyridinium chloride (CPC; pharmaceutically important cosurfactant) interfaces using vibrational sum frequency generation (VSFG), Raman spectroscopy, and surface tensiometry. VSFG, being a…

Plasma POSTN Derived From Bile Proteome Is a Promising Biomarker for Cholangiocarcinoma With Efficacy Comparable and Complementary to CA19.9.

BACKGROUND: Cholangiocarcinoma presents a global health challenge due to its increasing incidence and poor prognosis, primarily resulting fr … ltiple reaction monitoring, and enzyme-linked immunosorbent assay. RESULTS: Using a discovery set, bile proteome profiling identified 57 upregulated proteins that were either unique to cholangiocarcinoma or exhibited ≥ 2-fold changes compared to controls. The GEO transcriptome datasets yielded 48 upregulated genes consistently expressed in cholangioca …

[Use of parenteral nutrition in a patient undergoing pancreaticoduodenectomy for distal bile duct cancer: rationale and clinical implications].

INTRODUCTION: Major pancreatic surgery, such as pancreaticoduodenectomy, often involves patients with compromised nutritional status. Parenteral nutrition (PN) may serve as temporary support while awaiting adequate oral intake. AIM: This report presents a clinical case to discuss the indications, benefits, and implications of PN within an ERAS-based perioperative management protocol. CLINICAL CASE: A 71-year-old woman with recent weight loss and a BMI of 18.28 underwent a Whipple procedure for distal biliary tract cancer. Preoperatively, nutritional supplementation with immunonutrients was i

Decline of Persistent Jaundice in a Patient With Autoimmune Hepatitis and Vanishing Bile Duct Syndrome Treated With Elobixibat for Constipation.

We present the case of a 49-year-old woman with autoimmune hepatitis and persistent jaundice. On admission, pathology and laboratory results supported the diagnosis of autoimmune hepatitis-primary biliary cholangitis (AIH-PBC) overlap syndrome with vanishing bile duct syndrome (VBDS). Standard treatment, including methylprednisolone pulse therapy, prednisolone, azathioprine, bezafibrate, and ursodeoxycholic acid, failed to resolve jaundice. In addition to jaundice, the patient also had constipation and regularly used magnesium oxide and sennoside. Notably, the addition of elobixibat, initially…

Shifting of Distribution and Changing of Antibiotic Resistance in Gram-Positive Bacteria from Bile of Patients with Acute Cholangitis.

BACKGROUND: Gram-negative bacteria are the predominant pathogens responsible for biliary infections; however, the prevalence of Gram-positive bacteria is currently increasing. Investigating the bacterial spectrum and evolving antibiotic resistance patterns of Gram-positive bacteria is crucial for optimizing the management of acute cholangitis, particularly in the context of the global rise in antibiotic resistance. METHODS: This retrospective analysis focused on Gram-positive bacteria isolated from the bile of patients undergoing biliary drainage with acute cholangitis at our hospital from Ja…

The bile duct and liver cancer: ON-treatment surveillance of tumor evolution and response to systemic treatment (BILLIONSTARS) study.

BACKGROUND: Malignancies of the liver and bile ducts are associated with high recurrence rates after surgery and poor prognosis when disseminated. Medical treatment has been improved in recent years, with chemotherapy, targeted therapy and checkpoint inhibitors offering opportunities to influence the course of the diseases. Many patients do not benefit from treatment, however, and predictive and prognostic markers are lacking. The BILe duct and LIver cancer: ON-treatment Surveillance of Tumor evolution And Response to Systemic treatment (BILLIONSTARS) study aims to map how molecular tumor char…

Determining Urinary Bile Acid Profiles to Predict Maternal and Neonatal Outcomes in Patients with Intrahepatic Cholestasis of Pregnancy.

Objective: Intrahepatic cholestasis of pregnancy (ICP) is associated with an elevated risk of adverse perinatal outcomes, including perinatal morbidity and mortality. The objectives of this study were to evaluate the bile acid (BA) metabolism profiles in the urine of patients with ICP and to investigate the association between specific BAs and maternal and neonatal outcomes in patients with ICP. Methods: A total of 127 Chinese women with ICP and 55 healthy pregnant women were enrolled in our retrospective study. Spot urine samples and clinical data were collected from pregnant women from Janua…
