Reframing spontaneous preterm birth as a preventable adverse outcome-A clinical audit of a preventative toolbox.

INTRODUCTION: Spontaneous preterm birth is a major cause of perinatal morbidity and mortality and has traditionally been viewed as an inevitable event affecting a small proportion of pregnancies. Clinical practice guidelines recommend measures to predict pregnancies at risk of spontaneous preterm birth and prophylactic treatments. We performed this study to assess adherence to current guidelines for prediction and prevention of spontaneous preterm birth and to evaluate the effectiveness of current predictive tools. MATERIAL AND METHODS: Retrospective audit of all spontaneous preterm birth in

Changes in preterm birth and birthweight during the SARS-CoV-2 pandemic: a nationwide study in South Korea.

Birthweight is a strong determinant of a neonate's health. The SARS-CoV-2 pandemic's impact on birthweight has not been investigated in-depth, with inconsistent conclusions from initial studies. To assess changes in preterm birth and inappropriate birthweight between the SARS-CoV-2 pandemic and pre-pandemic periods. A nationwide birth micro-data consisted with exhaustive census of all births in 2011-2020 in South Korea was accessed to examine whether the mean birthweight and rates of under/overweight births changed significantly during the SARS-CoV-2 pandemic year (2020) compared to those of t

Inositol for subfertile women with polycystic ovary syndrome.

BACKGROUND: Subfertile women are highly motivated to try different adjunctive therapies to have a baby, and the widespread perception is that dietary supplements such as myo-inositol (MI) and D-chiro-insoitol (DCI) are associated with only benefit, and not with harm. Many fertility clinicians currently prescribe MI for subfertile women with polycystic ovary syndrome (PCOS) as pre-treatment to in vitro fertilisation (IVF) or for ovulation induction; however no high-quality evidence is available to support this practice. This review assessed the evidence for the effectiveness of inositol in subf

Craniocerebral birth injuries in term newborn infants: a retrospective series.

OBJECTIVE: In an attempt to further define the spectrum of cranial birth injuries, we analyzed 21 consecutive cranial birth injuries in term neonates presenting to the neurosurgical department of our institution over the period 1994-2015. METHODS: We performed a retrospective chart review from the medical records of the University Hospitals of the KU Leuven, from 1994 to 2015. We included 21 infants of 36-week gestational age or older with a diagnosis of cranial birth injury. The types and locations of injuries, the presenting signs, symptoms and their timing, and the required treatment(s) we

Obesity, Twin Pregnancy, and the Role of Assisted Reproductive Technology.

IMPORTANCE: The prevalence of overweight and obesity (body mass index [BMI] ≥25) has increased globally, and high BMI has been linked to higher rates of twin birth. However, evidence from large population-based studies is lacking; the issue needs careful study, as women with obesity are also more likely to use assisted reproductive technology (ART), which frequently results in twin pregnancy. OBJECTIVE: To examine the association between BMI and twin birth and the role of ART as a potential mediator in this association. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study inclu

Exposure to gonadotropin-releasing hormone agonist in early pregnancy leads to adverse pregnancy outcomes: a retrospective analysis.

BACKGROUND: GnRH-a is commonly used in the luteal phase for pituitary down-regulation during in-vitro fertilization (IVF). There is an ineluctable risk of spontaneous pregnancy for infertile couples who lack the use of contraception during the luteal phase down-regulation before IVF treatment. However, it is unclear whether exposure to GnRH-a affects clinical pregnancy outcomes. METHODS: A single-center retrospective cohort study based on propensity score matching was used to analyze the clinical data of a total of 6602 infertile women who were about to undergo assisted reproduction with IVF

Efficacy of progestin-primed ovarian stimulation in women with polycystic ovary syndrome undergoing in vitro fertilization: a systematic review and meta-analysis.

UNLABELLED: Polycystic ovary syndrome (PCOS) is a common endocrinopathy causing infertility in childbearing women. Progestin-primed ovarian stimulation (PPOS) protocol has recently been used for infertile women. However, whether PPOS provides a significant benefit over gonadotropin-releasing hormone (GnRH) analogue protocols in PCOS is still controversial. The objective of this systematic review is to investigate the efficacy of PPOS in patients with PCOS during in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI). We searched Medline, Embase, Google Scholar, ClinicalTrials,

First birth Caesarean section and subsequent fertility: a population-based study in the USA, 2000-2008.

STUDY QUESTION: Is first birth Caesarean delivery associated with a lower likelihood of subsequent childbearing when compared with first birth vaginal delivery? SUMMARY ANSWER: In this study of US women whose first delivery was in 2000, those who had a Caesarean delivery were less likely to have a subsequent live birth than those who delivered vaginally. WHAT IS ALREADY KNOWN: Some studies have reported lower birth rates subsequent to Caesarean delivery in comparison with vaginal delivery, while other studies have reported no difference. STUDY DESIGN, SIZE, DURATION: We conducted a retrospe

Intrauterine exposure to maternal diabetes and the risk of developing epilepsy in children: a national cohort study of 2.3 million children.

BACKGROUND: Approximately half of the epilepsy cases lack a documented cause. Despite growing evidence linking maternal diabetes to pregnancy complications and impaired child neurodevelopment, little is known about its potential association with child epilepsy. This study aims to determine whether intrauterine exposure to maternal pre-existing type 1 diabetes mellitus (T1DM), type 2 diabetes mellitus (T2DM), or gestational diabetes mellitus (GDM) increases the risk of epilepsy in children. METHODS: This study included live-born children without major malformations in Sweden from 1998 to 2021.

Influence of genotype and environment on piglet vitality and metabolic state during the suckling period of divergent birth weight classes.

The high mortality rate of suckling piglets in commercial farms is a cause for concern and requires the adoption of innovative breeding and management approaches for effective mitigation. Piglets are particularly vulnerable during the first days of life, which concerns especially neonates with a low birth weight. In this study, neonatal adaptation was analysed regarding genotype and housing conditions following hormonally induced farrowing. A total of 96 piglets from 27 farrowings of German Landrace (GL; birth induction at day 115 of gestation) and German Saddleback sows (GS; birth induction a

Determinants of preterm birth among newborns in resource limited settings.

The World Health Organization defines preterm birth as any birth before 37 completed weeks of gestation, or fewer than 259 days since the first day of the woman's last menstrual period. Globally, 1 in 10 babies is born prematurely and more than a million babies are estimated to die annually. The determinants of preterm birth may vary from region to region within the same country due to variation in socioeconomic status and health care service coverage. Therefore, this study was designed to identify determinants of preterm birth among newborns born in selected public hospitals in west Guji, eas

Uniform International Method to Measure Cervical Length: Are We There Yet?

INTRODUCTION: Cervical length is an important predictor of spontaneous preterm birth. So far, the best way to measure cervical length has not been established. We aimed to compare the incidence of short cervical length between three methods of cervical length measurement with and without the inclusion of cervico-isthmic complex (CIC) (six methods in total) and to determine the positive predictive value for spontaneous preterm birth. MATERIAL AND METHODS: We performed a prospective single-center cohort study in women with a singleton pregnancy between August 2014 and December 2018. During the

Knowledge about birth preparedness and complication readiness and associated factors among primigravida women in Addis Ababa governmental health facilities, Addis Ababa, Ethiopia, 2015.

BACKGROUND: Globally, every minute, at least one woman dies from complications related to pregnancy or childbirth. The situation is more serious for women in Sub-Saharan Africa which also include Ethiopia. Birth preparedness is a strategy to promote the timely use of skilled maternal and neonatal care, especially during childbirth. Based on the theory, preparing for childbirth reduces delays in obtaining this care. In adequate preparation for rapid action in the event of obstetric complications are well documented factors contributing to delay in receiving skilled obstetric care. Hence, the ai
