What is the impact of diabetes for Australian Aboriginal women when pregnant?

The impact differential of diabetes for Aboriginal maternal and infant health outcomes is different to Caucasian outcomes. With maternal diabetes, Aboriginal infant's birth weight increases and stillbirth rate is 22/1000 for gestational diabetes mellitus (GDM) and 53/1000 for pre-existing diabetes while for Caucasian infant's birth weight decreases and stillbirth rate is 3/1000 for GDM and 11/1000 for pre-existing diabetes. Equity of services is required to achieve equality of health outcomes.

Perinatal outcomes in a school-based program for pregnant teen-agers.

Teen-age pregnancy constitutes a high-risk factor for prematurity, low birth weight, intrauterine growth retardation, infant mortality, and later health, behavioral, developmental, and educational disadvantages. A case-controlled study was performed to evaluate the influence of the Teen-age Parent Center on pregnancy outcomes. Improved outcomes were identified for low-birth-weight rate, high school graduation rate, smoking during pregnancy, and prenatal care. These results indicate that a comprehensive, school-based program can enhance perinatal outcomes.

What's so bad about teenage pregnancy?

A systematic literature review identified the most frequently cited medical consequences of teenage pregnancy as anaemia, pregnancy-induced hypertension, low birth weight, prematurity, intra-uterine growth retardation and neonatal mortality. Critical appraisal suggested that increased risks of these outcomes were predominantly caused by the social, economic, and behavioural factors that predispose some young women to pregnancy. Maternal age less than 16 years was associated with a modest (1.2-2.7 fold) increase in prematurity, low birth weight and neonatal death.

Bilirubin and jaundice in the micropremie.

Although it has been customary to treat neonatal jaundice at lower serum bilirubin levels in low-birth weight infants than in term infants, the threshold bilirubin levels and long-term benefits for early treatment of preterm infants have not been validated. This article summarizes and evaluates existing evidence and strategies for early treatment of bilirubin in micropremies and recommends a conservative but flexible approach to early monitoring and phototherapy for jaundice in extremely low-birth weight infants.

Cigarette smoking, alcohol consumption and fetal outcome in Tasmania 1981-82.

Analysis of cigarette smoking and alcohol consumption patterns recorded in pregnancy could not detect a harmful effect of alcohol at levels below 2 glasses a day. Total abstainers fared very slightly worse than those who drank occasionally. Smoking was associated with lower birth-weight and a worse fetal outcome independent of socioeconomic status, parity, maternal age or alcohol consumption. There was evidence of an interaction between smoking and adverse social and parity factors in low birth-weight.

Performance of ultrasound fetal weight estimation in twins.

BACKGROUND: Ultrasonography is an essential tool in the management of twin pregnancies. Fetal weight estimation is useful to anticipate neonatal care in case of weight restriction or growth discordance. AIMS: To assess the accuracy of estimated fetal weight (EFW) in twins and to assess the accuracy of sonographic examination to predict birth weight discordance (BWD) and small birth weight (SBW).    Methods : This was  a longitudinal prospective study over a period of one year. We have included 50 twin pregnancies with a first trimester ultrasound calculated term and specified chorionicity. An…

Fetal adrenal gland enlargement - prenatal and postnatal management.

BACKGROUND: The enlargement of suprarenal gland is related to preterm birth and the birth weight. The ultrasound measurement of fetal adrenal gland volume may identify women at risk for impending preterm birth. The aim of our study was to investigate the newborns in the region of western Slovakia followed up due to suprarenal gland enlargement. To set the ratio of prenatally diagnosed suprarenal gland enlargment, postnatal managment and treatment and interventions. The newborns with congenital adrenal hyperplasia were excluded. METHODS: We have analyzed 6 years of medical records of all cases…

Postpartum Weight Retention and Subsequent Pregnancy Outcomes.

The effects of postpartum weight retention on gestational weight gain in successive pregnancies require elucidation. The purpose of the study was (1) to examine the association between postpartum weight retention and subsequent adherence to the Institute of Medicine gestational weight gain guidelines and (2) to determine whether the association varies by body mass index status and affects birth outcomes. Florida vital records for 2005-2010 were analyzed using χ tests and multivariable Poisson regression, adjusted for interpregnancy interval, tobacco use, maternal age, and race/ethnicity. Obese

The incidence of retinopathy of prematurity in a neonatal care unit.

In India, with advancement in neonatal care units, a large number of low-birth weight premature babies are now surviving and are at risk of developing retinopathy of prematurity (ROP). However, there are not enough reports on the incidence of ROP in this country. To determine the incidence of ROP in a prospective manner, 165 babies weighing < or = 1700 gm over a period of one year were examined. An incidence of 47.27% of ROP at the mean age of 7.21 +/- 0.3 weeks of life was detected. The maximum stage reached was stage 1 in 28 (16.97%), stage 2 in 29 (17.58%), stage 3 in 19 (11.52%) and stage …

Maternal iron status during pregnancy: relationship with fetal growth; ECLIPSES study.

BACKGROUND: Maternal iron deficiency anemia has been linked to reduced fetal growth assessed at birth, but research on elevated maternal iro … ewborns, high maternal SF levels without anemia, but not low levels, was negatively associated with birth weight and HC. A high SF trajectory doubled this negative effect on birth weight (β:-107.96, p = 0.011). The risk of having a low birth weight (< 2.500 g) was consistent with high SF levels at both T1 and T3, and was markedly tripled in the high FS traject …

Using a Kirschner Wire as a Stylet for the Management of a Difficult Neonatal Airway: A Case Report.

We report the successful management of a difficult airway in an extremely low birth weight neonate (700 g) using a Kirschner wire as a substitute for an endotracheal tube stylet. Several intubation attempts were unsuccessful because of the difficulty in guiding a very small and malleable tube under the epiglottis. This study highlights that every maternity hospital should be prepared to manage airways in unexpected extremely low birth weight neonates. Appropriate size equipment and protocols should be readily available.

Somatomedin-like activity in cord blood from infants of hypertensive mothers.

Somatomedin-like activity (SLA) was measured by bioassay in cord serum from infants of hypertensive mothers. Levels of SLA were lowest in "light for date' infants with birth weights below the 3rd centile, but did not correlate with birth weight, placental weight or maturity at delivery. Cord serum from female infants had significantly (P less than 0.001) higher levels of SLA than male infants. No differences in SLA were found between infants born after spontaneous labor or following induction.

Clinical Risk Factors Associated With Late-Onset Invasive Group B Streptococcal Disease: Systematic Review and Meta-Analyses.

BACKGROUND: Group B streptococcal (GBS) infection remains one of the most significant causes of late-onset sepsis and meningitis (LOGBS) among young infants. However, transmission routes and risk factors for LOGBS are not yet fully understood. METHODS: We conducted systematic reviews on clinical risk factors previously reported in the literature (prematurity, low birth weight [<2500 g], antenatal colonization, multiple-gestation pregnancy, maternal age <20 years, male infant sex, intrapartum fever, prolonged rupture of membranes) and meta-analyses to determine pooled estimates of risk.
