β-Hydroxybutyric acid impairs host antimicrobial defense by targeting the CEACAM1-NADPH-ROS axis to disrupt macrophage bactericidal activity.

During bacterial infections, the immune function of macrophages in the host is crucial for eliminating bacteria. Metabolic disturbances can … elate with increased infection-related parameters, such as procalcitonin (PCT) and high-sensitivity C-reactive protein (hsCRP). A similar clinical phenomenon was observed in mice suffering from bacterial peritonitis. Further, animal and cell experiments confirmed that BHBA stimulation could impair the antibacterial defense response of hosts by disrupting the bacteri …

Surface Layer Protein A Expressed in Clostridioides difficile DJNS06-36 Possesses an Encephalitogenic Mimotope of Myelin Basic Protein.

Multiple sclerosis (MS) is an inflammatory demyelinating disease of the central nervous system (CNS). Recent studies suggest that migration of Th1 and Th17 cells specific for enteric bacteria from the gut to the CNS may lead to the initiation and/or exacerbation of autoimmune diseases including MS. Human leukocyte antigen (HLA)-DR15 is an MHC class II (MHCII) haplotype highly associated with the development of MS that contains the two HLA-DRB* genes, DRB1*1501 (DR2b) and DRB5*0101 (DR2a). To identify enteric bacteria which harbor antigenic epitopes that activate myelin-specific T cells and dri

Linear IgA bullous dermatosis with IgA antibodies exclusively directed against the 180- or 230-kDa epidermal antigens.

This study describes the presence in sera from patients with linear IgA bullous dermatosis (LABD) of IgA antibodies specific for 230- or 180-kDa epidermal antigens. Of 11 patients' sera with IgA antibodies reactive with the epidermal antigens obtained from cultured keratinocytes, 6 sera recognized the 230-kDa antigen and co-migrated with the polypeptide recognized by a human monoclonal antibody against the 230-kDa bullous pemphigoid antigen (BPAgl). Five sera recognized the 180-kDa antigen and co-migrated with the polypeptide stained by a polyclonal antibody to the 180-kDa bullous pemphigoid a

Utility of Blood Neutrophil to Diagnose Bacterial Infections under Satralizumab Treatment for Aquaporin-4 Antibody-positive Neuromyelitis Optica Spectrum Disorder: A Report of Two Cases.

Satralizumab is an anti-interleukin-6 receptor biologic that may mask bacterial infections. We report two patients with neuromyelitis optica spectrum disorder who developed afebrile bacterial infections with normal C-reactive protein levels during satralizumab treatment. During non-infectious periods under satralizumab treatment, the neutrophil count and neutrophil-to-lymphocyte ratio decreased, whereas the neutrophil change ratio, defined as the ratio of present to previous neutrophil counts, did not. At the time of infection, these variables were elevated compared to the intra-individual con…

Systemic AA Amyloidosis Following Immune Checkpoint Inhibitor Therapy for Lung Carcinosarcoma: An Autopsy Case and Review of the Literature.

AA amyloidosis is a rare but serious complication of chronic inflammation. We report an autopsy case of a 68-year-old man without a documented history of inflammatory disease who developed lung carcinosarcoma and achieved a durable complete response after sequential immune checkpoint inhibitor (ICI) therapy. Despite tumor control, the patient developed a chronic intestinal pseudo-obstruction, persistent C-reactive protein elevation, progressive renal dysfunction, and later aspiration pneumonia. Autopsy revealed systemic AA amyloid deposition, predominantly in the intestines and kidneys, chroni…

Sequential treatment with valemetostat and conventional anti-cancer drugs for refractory aggressive adult T-cell leukemia/lymphoma: A case report.

A 79-year-old man presented with respiratory distress associated with a mediastinal mass and pleural effusion, and was diagnosed as having adult T-cell leukemia/lymphoma. The patient was highly refractory to anticancer drugs and radiotherapy from the time of onset and had progressive respiratory deterioration. However, his condition became stable with the administration of valemetostat for 11 days, and subsequent low-dose-anticancer agents led to a rapid improvement accompanied by high fever and a surge in C-reactive protein. In this case, the in vivo priming effect of valemetostat on tumor ce…

[Sepsis caused by Pasteurella multocida after a dog bite].

This article reports a case of systemic infection caused by Pasteurella multocida. The infection was confirmed in a 79-year-old man who was admitted to the hospital after falling from a couch. The disease was manifested by the development of fever, chills, joint pain. Laboratory tests revealed elevated C-reactive protein levels, slightly elevated nitrogen metabolites, borderline leukocytosis, and thrombocytopenia. The pathogen was identified in a blood culture and a wound swab culture. The patient was initially treated with third-generation cephalosporin (cefotaxime) and later with cefuroxime.…

Rare Presentation of Postpericardiotomy Syndrome After Left Atrial Myxoma Removal.

Postpericardiotomy syndrome (PPS) is a known complication of cardiac valve surgery, but it has not been commonly reported as a postoperative complication of cardiac myxoma removal. A 78-year-old female with hypertension and atrial fibrillation presenting with angina was found to have a large left atrial myxoma (7.5 cm × 4.4 cm). The myxoma was resected; however, 1-week postoperation hemoglobin and blood pressure decreased with elevated erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP). Limited transthoracic echocardiogram (TTE) showed moderate pericardial effusion, confirming t…

COVID-19 Vaccine-Induced Multisystem Inflammatory Syndrome With Polyserositis Detected by FDG PET/CT.

Of the various adverse reactions to COVID-19 vaccines, fever is a common systemic symptom that often resolves spontaneously without treatment. However, rare vaccine-induced conditions that present with fever and systemic inflammation have been reported. In this case, a 65-year-old man with BNT162b2 mRNA COVID-19 vaccination underwent 18F-FDG PET/CT to evaluate prolonged fever and elevated serum C-reactive protein. PET/CT showed hypermetabolic infiltration in the pericardium and peritoneum suggesting immune-mediated pericarditis and peritonitis. After administration of high-dose corticosteroids…

[A Case of Granulocyte Colony-Stimulating Factor-Associated Aortitis during Neoadjuvant Chemotherapy for Esophageal Cancer].

A 71-year-old man with middle thoracic esophageal cancer was treated with neoadjuvant chemotherapy using docetaxel plus 5-FU plus cisplatin therapy and was also administered pegfilgrastim. Blood tests showed elevated white blood cell counts and C-reactive protein levels before the start of the third course. Contrast-enhanced computed tomography revealed wall thickening of the aortic arch. We diagnosed this as aortitis due to pegfilgrastim. Inflammation was improved with conservative treatment. We then performed video-assisted thoracoscopic esophagectomy. Drug-induced vasculitis should be inclu…

Elevated White Blood Cell Count and Lactate Dehydrogenase Levels Are Important Markers for Diagnosing Relapse of Adult-onset Still's Disease under Tocilizumab Use.

We encountered a case of refractory adult-onset Still's disease (AOSD) with two relapses. Prednisolone and methotrexate were begun as induction therapy, resulting in the patient's first relapse during tapering of prednisolone. After the introduction of tocilizumab, she achieved remission. However, she experienced a second relapse following prednisolone tapering. While lactate dehydrogenase (LDH) levels and white blood cell (WBC) counts increased in both relapses, interleukin-6 (IL-6) suppression resulted in stable C-reactive protein and ferritin levels in the second relapse. A comparison of th…

A Ruptured Jejunal Arterial Aneurysm in a Young Woman Undergoing Chronic Hemodialysis Due to Myeloperoxidase-antineutrophil Cytoplasmic Antibody-associated Vasculitis.

A 21-year-old woman was admitted to our hospital because of massive intestinal bleeding. She started hemodialysis due to myeloperoxidase antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) at 18 years of age. Her ANCA titers remained stable; however, her C-reactive protein increased on 5 mg/day prednisolone before admission. Computed tomography angiography revealed a ruptured jejunal arterial aneurysm. Transcatheter arterial embolization, blood transfusion and the reinforcement of steroid therapy resolved her symptoms of AAV. Our case of a young patient with AAV and medium-s…

Cryopyrin-associated periodic syndrome: a treatable genetic inflammatory condition.

A 20-year-old man presented with recurrent subdural haemorrhages on a background of progressive sensorineural hearing loss, juvenile idiopathic arthritis and intracranial hypertension of unknown cause. His mother had a similar previous history. They both had a persistently mildly elevated serum C reactive protein. Repeat lumbar punctures identified persistently elevated intracranial pressure and mild pleocytosis. A dural biopsy showed necrotising pachymeningitis with granulomatous vasculitis. The underlying cause in both patients was a cryopyrin-associated periodic syndrome. We discuss its var
