How to Best Measure Disease Progression in Adult Spinal Muscular Atrophy Patients: A Clinical and Neurophysiological Study.

INTRODUCTION/AIMS: Natural history data for adult patients with spinal muscular atrophy (SMA) remain scarce, which is particularly relevant in the current therapeutic era. This study aimed to identify the most sensitive clinical, patient-reported, and neurophysiological measures to detect short-term disease progression in untreated adult SMA patients. METHODS: This prospective, one-year longitudinal study included 21 genetically confirmed adult patients with SMA types 2B and 3. Clinician-reported outcomes (CROs) included the Motor Function Measure (MFM), Hammersmith Functional Motor Scale Exp

A Higher Burden of Abnormal Antioxidant-, Nutritional-, and Inflammation-Related Biomarkers Is Associated with Shoulder-Hand Syndrome, Hand Edema, and Short-Term Upper-Limb Outcomes During Stroke Rehabilitation.

Post-stroke shoulder-hand syndrome and hand edema can complicate inpatient rehabilitation and may reflect interactions between antioxidant, nutritional, inflammatory, and neuro-musculoskeletal factors. This single-center retrospective cohort study included 453 adults admitted for inpatient rehabilitation within 60 days after a first-ever unilateral ischemic or hemorrhagic stroke between 1 January 2018 and 31 December 2025. The primary exposure was the number of abnormal antioxidant-, nutritional-, and inflammation-related biomarkers, defined as serum zinc <70 µg/dL, albumin <3.5 g/dL, C-reacti

Serum Uric Acid as a Biomarker of Disease Severity in Children With Complicated Malaria: A Cross-Sectional Study From Eastern India.

BACKGROUND: Malaria continues to be a significant contributor to morbidity among children in tropical countries. Increasing evidence suggests that elevated uric acid levels may reflect inflammatory activity and disease severity in malaria. The aim of this study was to measure serum uric acid (SUA) levels in children with complicated malaria and to examine their association with clinical outcomes and biochemical markers of hepatic dysfunction. METHODS: A hospital-based cross-sectional investigation was carried out in the Pediatrics Department of a tertiary care teaching hospital in eastern Ind

A practical 5-stage clinical scale for electrical storm: the STORM classification.

AIMS: Electrical storm (ES) is a highly heterogeneous condition with wide-ranging clinical presentations. The absence of standardized classification hampers risk stratification and limits effective multidisciplinary coordination. OBJECTIVE: The aim of this study was to develop a classification system based on simple clinical characteristics and stratify 30-day mortality in patients with ES. METHODS AND RESULTS: Patients admitted to intensive care units for ES between 2010 and 2023 across four tertiary centres were retrospectively included. The five-stage STORM severity-response classificatio

What Does the PCL-5 Measure? A Conjoint Hierarchical Analysis Using the MMPI-3.

The PTSD Checklist for DSM-5 (PCL-5) is a widely used screening instrument for posttraumatic stress disorder (PTSD) symptoms. There is active debate about what the PCL-5 actually measures (i.e., PTSD specific symptoms vs general distress or other transdiagnostic factors), especially when positive screenings are observed using total sum scores. The Minnesota Multiphasic Personality Inventory-3 (MMPI-3) is a broadband clinical instrument used to assess a wide array of self-reported psychological and behavioral difficulties within an established hierarchical structure. Examining the PCL-5 togethe

Blood Transfusions in Patients with Advanced Cancer at the End-of-Life: Are They Really Beneficial?

OBJECTIVES: Anaemia is prevalent in individuals with advanced cancer and may contribute to adversely affecting their quality of life. In palliative care (PC), red blood cell (RBC) transfusions are regularly administered to address bothersome symptoms such as fatigue and dyspnoea. However, they are not without risks and adverse effects in individuals who are often frail at the end of life. Within this context, RBC transfusion benefits and drawbacks have yet to be demonstrated. This study aimed to assess RBCs transfusion-targeted indications, practices and clinical outcomes at the end-of-life in

The diagnostic value of native and early post-contrast CT attenuation in differentiating benign from potentially malignant adrenal lesions.

PURPOSE: Adrenal gland lesions are increasingly detected as incidental findings during standard abdominal CT examinations. The aim of this study was to evaluate the diagnostic utility of native and early post-contrast (1-min) CT attenuation values, as well as lesion size, in differentiating benign from potentially malignant adrenal lesions, and to determine optimal cut-off values for these parameters in a routine clinical setting. METHODS: An observational, historical cohort study included 50 patients who underwent adrenalectomy between 2012 and 2024, with available preoperative CT imaging. B

Noncompaction and dilated cardiomyopathy in carvajal syndrome.

INTRODUCTION: Carvajal syndrome, a rare autosomal recessive disorder caused by mutations in the DSP gene, is characterised by woolly hair, palmoplantar keratoderma, and left ventricular dilated cardiomyopathy. Although less frequently reported, noncompaction cardiomyopathy can co-occur, further complicating the clinical picture. Early diagnosis and management are crucial due to the high risk of progressive heart failure and sudden cardiac death in affected individuals. CASE REPORT: A 13-year-old male with autism presented with a 1.5-month history of persistent cough and worsening clinical sym

Effectiveness of imipenem-relebactam for multidrug-resistant Pseudomonas aeruginosa in pneumonia and bloodstream infections in the United States (MIRAGE).

Imipenem-relebactam demonstrates in vitro activity against multidrug-resistant (MDR) Pseudomonas aeruginosa, but real-world effectiveness data are limited. MIRAGE was a multicenter, retrospective, observational study of imipenem-relebactam for MDR P. aeruginosa pneumonia and bacteremia. Patients were included if they received imipenem-relebactam for >48 h within 7 days of index P. aeruginosa culture. The primary outcome was clinical success at day 30, defined as survival, resolution of signs and symptoms of infection, completion of intended treatment course, and absence of recurrent infection.

Immune Checkpoint Inhibitor-associated Pneumonitis: A Narrative Review.

Immune checkpoint inhibitors (ICI), such as pembrolizumab, nivolumab, durvalumab and ipilimumab, have significantly enhanced survival rates for multiple cancer types such as non-small cell lung cancer, melanoma, Hodgkin lymphoma, and breast cancer, and they have emerged as an adjunct or primary therapy for malignant disease. Approximately 40% of patients with cancer on ICI therapy experience side effects called immune-related adverse events (irAE). While not the most common, pulmonary toxicities can be rapidly progressive, potentially fatal, and pose a three-fold increased risk for requiring i
