PURPOSE OF REVIEW: The purpose of this manuscript is to review the progress in the field of therapeutics for malignant pheochromocytomas and sympathetic paraganglioma (MPPG) over the past 5 years.
RECENT FINDINGS: The manuscript will describe the clinical predictors of survivorship and their influence on the first TNM staging classification for pheochromocytomas and sympathetic paragangliomas, the treatment of hormonal complications, and the rationale that supports the resection of the primary tumor and metastases in patients with otherwise incurable disease. We report a patient with systemic lupus erythematosus (SLE) complicated with nocardiosis. Churg-Strauss syndrome (CSS) is a rare disease belonging to the group of necrotizing vasculitides affecting medium and small vessels, classified together with Wegener granulomatosis and microscopic polyarteritis. A 75-year-old male presented with new symptoms of a bleeding diathesis associated with a decline in the functional activity of von Willebrand factor (type 2a von Willebrand's disease). Replacement therapy was ineffective and he was subsequently treated with intravenous immunoglobulin (IvIg). Evidence supporting the claim that specific phenothiazines, notably chlorpromazine and promethazine, may be used as sole agents for the treatment of cancer in man, has been reviewed. Selective destruction of cancerous tissue can be achieved by modulating energy metabolism in malignant cells. A 61-year-old woman was admitted to our hospital with a two week history of knee-joint pain, dry coughing and dyspnea on exertion. A chest roentgenogram revealed bilateral patchy and infiltrative shadows. Laboratory examination revealed high CK and aldolase levels. There is a large range of neurologic complications in periarteritis nodosa, but oculomotor motility disorders are rarely seen. Two patients presented with third nerve palsy and one presented with internuclear ophthalmoplegia due to periarteritis nodosa vasculitis. We hypothesize that the third nerve palsy and internuclear ophthalmoplegia was caused by a vascular infarction in the brain stem.

Hypercalcemia is a common cause of morbidity in cancer patients. The mechanism of malignancy-associated hypercalcemia includes increased bone resorption and decreased renal calcium clearance which also occur in primary hyperparathyroidism. Norethisterone can inhibit bone resorption and has recently been shown to be effective treatment for mild hyperparathyroidism in post menopausal women. We report the successful use for the first time of norethisterone (5 mg daily) in a case of malignancy-associated hypercalcemia after other standard agents failed.

A muscle biopsy established the diagnosis of dermatomyositis in a 22-year-old woman with normal muscle enzyme levels, 18 months after she presented with constitutional symptoms, a rash and polyarthritis. Her subsequent course was complicated by cutaneous ulcers with histological changes suggesting vasculitis, interstitial pulmonary involvement and recurrent pneumomediastinum, the latter being recently reported in 2 other cases of dermatomyositis. Cutaneous vasculitis has been present in all cases and it is suggested that the pneumomediastinum may be due to pulmonary vasculitis.

He lost all distal phalanges of both hands and big toes at the age of nine. Remarkable was the laboratory finding of an extremely high serum IgE concentration. Antinuclear antibodies (ANA) are found in the majority of patients with systemic lupus erythematosus (SLE). We report here the only documented case, out of a series of 38 patients, in which SLE was diagnosed in spite of the fact that we failed to demonstrate any type of autoantibodies. Hyperkeratotic scabies and onychomycosis were associated in an immunosuppressed patient. Onychomycosis healed spontaneously after treatment of scabies. A possible interrelationship between Sarcoptes scabiei infestation of the hyponychium and dermatophytes infestation of the nail plate could be responsible for this peculiar evolution.

Three apparently pure ependymomas of the ovary occurred in young women 25 to 35 years of age who presented with abdominal swelling or pain. At operation one tumor was stage IC, and two were stage III.
