Patients with traumatic spinal injuries treated in a low- and middle-income country: what happens after discharge?

OBJECTIVE: Traumatic spinal injuries (TSIs) represent significant global health challenges, particularly in low- and middle-income countries (LMICs). In a cohort of patients with TSI from a major East African center, the authors sought to address long-term outcomes after hospital discharge through the following objectives: 1) describe who was successfully contacted after hospital discharge, 2) report posthospital complications, and 3) assess predictors of successful posthospital contact and posthospital complications. METHODS: An institutional TSI registry of patients treated from 2016 to 202

"Presurgical Infant Orthopedics in Low-Income and Middle-Income Countries: Demographics, Access, and Treatment Outcomes From the Smile Train Express Database".

BACKGROUND: Presurgical infant orthopedics (PSIO) is widely used to reduce cleft severity, improve nasal symmetry, and facilitate primary lip repair. While extensively implemented in high-income countries (HICs), its feasibility, accessibility, and effectiveness in low-income countries and middle-income countries (LMICs) remain uncertain. This study assessed PSIO implementation, accessibility, and outcomes across 5 Smile Train partner centers in LMICs. METHODS: A retrospective multicenter analysis was conducted using the Smile Train Express (STX) database from 5 LMIC centers in Brazil, Mexico

Distinct maternal DNA methylation associations with gestational age at early and late-mid term pregnancy in a low- and middle-income country: evaluation of biological, genetic, and psychosocial contributors.

Mothers undergo physiological and molecular changes over the course of gestation. These modifications "get under the skin" and may be reflected in the maternal epigenome through processes such as DNA methylation. Such an epigenetic mark may offer insights into maternal responses to prenatal influences and biological cues from the developing fetus, thereby functioning as an indirect indicator of the conditions the fetus experiences in utero. We measured whole blood DNA methylation using the MethylationEPIC BeadChip Infinium microarray v1.0 in 22 pregnant women from Pakistan, a low- and middle-i

Effectiveness of intrapartum azithromycin to prevent infections in planned vaginal births in low-income and middle-income countries: a post-hoc analysis of data from a multicentre, randomised, double-blind, placebo-controlled trial.

BACKGROUND: In 2023, the Azithromycin Prevention in Labor Use (A-PLUS) trial showed intrapartum azithromycin reduces maternal sepsis or death in women with planned vaginal delivery in low-resource settings, but whether it reduces maternal infection is unknown. We aimed to evaluate the effectiveness of intrapartum azithromycin in reducing maternal infection. METHODS: We performed a post-hoc analysis of the multicentre, facility-based, randomised, double-blind, placebo-controlled A-PLUS trial. This trial compared prophylactic intrapartum single oral dose of 2 g azithromycin versus placebo on ma

Unusual causes of perineal trauma in paediatric patients: lessons learned from a high-volume colorectal clinic in a low-and-middle-income country.

PURPOSE: In low-and-middle-income countries (LMIC), a non-negligible number of cases of paediatric perineal trauma is observed. Trauma can arise from less conventional mechanisms such as traditional enemas and flame burns, alongside blunt and penetrating injuries. The data on surgical management and long-term outcomes are limited. This study aims to evaluate surgical treatment and continence outcomes post-reconstruction. METHODS: A retrospective review was conducted at Johannesburg's Chris Hani Baragwanath Academic Hospital (2018-2025). Cases of sexual abuse were excluded. Patients were strat

Seven-day versus 14-day antibiotic course for culture-proven neonatal sepsis: a multicentre randomised non-inferiority trial in a low and middle-income country.

OBJECTIVE: Definitive guidance regarding the duration of antibiotics for neonatal sepsis is lacking. We hypothesised that a 7-day antibiotic course is non-inferior to a 14-day course for treating culture-proven sepsis. DESIGN: Randomised, controlled, non-inferiority trial with masked outcome assessment in eight centres in a low and middle-income country. PATIENTS: Neonates with a birth weight (BW) ≥1000 g and blood culture-proven sepsis were randomised on day 7 of sensitive antibiotic therapy provided sepsis had clinically remitted. EXCLUSIONS: Staphylococcus aureus or fungal sepsis, and in

Pilot study of radiation oncology peer review in low middle income country (LMIC) through cloud-based platform.

PURPOSE: Peer review is an essential step in clinical quality assurance for radiation therapy. There are very little data on peer reviews from low-middle-income countries (LMIC). With increasing access to advanced technologies in LMIC also, peer review is becoming more important to ensure quality and standard of care. We evaluated cloud-based e-Peer review in our network of cancer centers in India with an aim to study its feasibility and impact on care. MATERIALS AND METHODS: Four out of 13 cancer centers across India were selected for this pilot study. All team members were trained on the e-

Accuracy of the Wound Healing Questionnaire in the diagnosis of surgical-site infection after abdominal surgery in low- and middle-income countries.

INTRODUCTION: Telemedicine is being adopted for postoperative surveillance but requires evaluation for efficacy. This study tested a telephone Wound Healing Questionnaire (WHQ) to diagnose surgical site infection (SSI) after abdominal surgery in low- and middle-income countries. METHOD: A multi-centre, international, prospective study was embedded in the FALCON trial; a factorial RCT testing measures to reduce SSI in seven low- and middle-income countries (NCT03700749). It was conducted according to a pre-registered protocol (SWAT126) and reported according to STARD guidelines. The reference

COVID-19 and the vaccine tax: an egalitarian, market-based approach to the global vaccine inequality.

The global inequality in the distribution of vaccines is unjust. As countries scrambled to ensure enough vaccines, the world's poorest were left to fend for themselves, and the generosity meant to mitigate this through COVAX was not sufficiently forthcoming. In light of this, I proposed a vaccine tax, which obligates those willing and able to pay to protect their own population to contribute to protecting those residing in the world's low-income countries. Petrovic has offered an important critique of this proposal, questioning both the fairness and the efficiency of the tax. However, when pro

Modifying burn care in a low- and middle-income countries (LMIC) to meet global best practice: A burn unit or burn isolation ward?

The overwhelming burden of burns in low-income (LIC) and low-middle-income (LMIC) countries has been well-documented. Though best global practice is for major burns to be treated in burn units, the prohibitive cost makes it difficult. In this article we attempt to present the improvement in outcome recorded over a period of 3 years as we transitioned from nursing our burns patients in the general surgical wards (Group A), then a separate cubicle within the wards (Group B) and then an isolated burn ward (Group C). Other practices such as physiotherapy, traditional wound dressings, and limb spli

Implementation of an adapted perioperative ventriculoperitoneal shunting protocol in a tertiary center located in a low-to-middle-income country.

OBJECTIVE: Preventative protocols have efficaciously reduced shunt infection in developed countries. However, the generalizability of these protocols in low-to-middle-income countries (LMICs) remains unclear. Previously, shunt insertion in the authors' center was routinely performed under institutional preventative precautions, which was updated via merging with the Hydrocephalus Clinical Research Network (HCRN) protocol. This study aimed to investigate the ventriculoperitoneal shunt (VPS) infection rates in pediatric patients following the implementation of the adapted protocol. METHODS: The

The impact of international health worker migration and recruitment on health systems in source countries: Stakeholder perspectives from Colombia, Indonesia, and Jordan.

INTRODUCTION: To address domestic shortages, high-income countries are increasingly recruiting health workers from low- and middle-income countries. This practice is much debated. Proponents underline benefits of return migration and remittances. Critics point in particular to the risk of brain drain. Empirical evidence supporting either position is yet rare. This study contributes to filling this gap in knowledge by reporting high-level stakeholders' perspectives on health system impacts of international migration in general, and active recruitment of health workers in specific, in Colombia,

Estimation of couple fecundity in the general population and the association with monthly time-varying ambient particulate matter exposure in low- and middle-income countries: A population-based multi-center epidemiological study.

BACKGROUND: Declining total fertility rates have been observed in low- and middle-income countries (LMICs). However, it remains unclear if this trend is related to a reduction in fecundity of general population. Research evidence on contributing factors to fecundity reduction is also limited. We aimed to first estimate couple fecundity in LMICs and then investigate its association with ambient particulate matter (PM) exposure. METHODS: Using the information from Demographic and Health Surveys between 2003 and 2019, we estimated median time to pregnancy (TTP) and infertility prevalence across

Cost-effectiveness of COVID rapid diagnostic tests for patients with severe/critical illness in low- and middle-income countries: A modeling study.

BACKGROUND: Rapid diagnostic tests (RDTs) for coronavirus disease (COVID) are used in low- and middle-income countries (LMICs) to inform treatment decisions. However, to date, it is unclear when this use is cost-effective. Existing analyses are limited to a narrow set of countries and uses. The aim of this study is to assess the cost-effectiveness of COVID RDTs to inform the treatment of patients with severe illness in LMICs, considering real world practice. METHODS AND FINDINGS: We assessed the cost-effectiveness of COVID testing across LMICs using a decision tree model, differentiating resu
