Nowadays, there is a trend towards the concept of damage control in the management of multiple trauma patients. However, the question remains whether all patients benefit from this concept. We report the primary total definitive treatment of a patient with multiple fractures of the lower extremities. Postoperative respiratory insufficiency was treated successfully by non-invasive ventilation.

The use of external fixation in the immobilization of diaphyseal and metaphyseal fractures in children is still controversial, as these fractures are generally managed by immediate plaster casting, by traction followed by casting, by various methods of internal fixation, including the recently developed flexible rods, and by plating. The authors present a series of 26 cases of ipsilateral fractures of the shafts of the femur and tibia treated by internal fixation. Twenty have been followed up for more than 2 years. Thirteen healed without incident and achieved excellent function. Six others regained a similar level of mobility after complications had been treated. The early surgical stabilisation of such fractures is advocated, allowing recognition of simultaneous injuries to ligaments of the knee.

Investigations on the load capacity of an ultrasonic weld in transverse fractures, oblique fractures, and multiple fragment fractures by means of homologous bone fragments and Fimomed were taken up in vitro on 84 human femur diaphyses. It was possible to demonstrate the essential influence of the form of the fracture on the stability of the weld. The mechanical density was the highest in transverse fractures, small in oblique fractures, and the smallest in multiple fragment fractures. Conclusions were drawn up for the clinical use.

PURPOSE: New research is focusing on the use of autologous growth factors to increase the effect of bone fracture healing while decreasing the amount of healing time for the patient. Platelets have been demonstrated to be the natural storage vessel for several growth factors and cytokines that promote blood coagulation, tissue repair, and the process of bone mineralization. The present study aims to evaluate the role of platelet-rich plasma (PRP) in healing of acute femoral shaft fractures radiologically.
