Renal arteriovenous fistula (RAVF) is an infrequent vascular anomaly characterized by an aberrant connection between the renal artery and vein, potentially leading to significant clinical consequences. This condition may be congenital, acquired, or idiopathic and can manifest with symptoms such as hypertension, cardiac strain, renal impairment, or hematuria. Diagnostic imaging, including digital subtraction angiography (DSA) as the definitive standard, alongside noninvasive modalities like Doppler ultrasound and computed tomography (CT), plays a critical role in identifying and planning treatm…

INTRODUCTION: Fistula formation from the intraductal papillary mucinous neoplasm (IPMN) into neighboring organs is rare. We describe a case of pancreaticobiliary fistula with obstructive jaundice caused by an intraductal papillary mucinous adenoma (IPMA).
CASE PRESENTATION: An 81-year-old man who was incidentally diagnosed with IPMN 11 years previously based on follow-up CT performed after nephrectomy for renal cell carcinoma. Gradual dilation of the main pancreatic duct was observed over time, confirming the high-risk stigmata for which surgery was recommended; however, the patient declined …

BACKGROUND AND OBJECTIVES: External pancreatic fistula (EPF) is a common consequence of partial pancreatectomy or percutaneous treatments for infected pancreatic necrosis. It has traditionally been treated surgically with distal pancreatectomy, Roux-en-Y pancreaticoenterostomy, or fistuloenterostomy.
METHODS AND RESULTS: This study introduces a novel technique of laparoscopic stent-bridging fistulogastrostomy, which was used for a 40-year-old female experiencing refractory post-pancreatectomy high-output EPF. We present a standardized management protocol for this procedure, which includes pre…

BACKGROUND: Coronary stent infection (SI) is a rare but catastrophic complication, occurring in 0.3% to 6% of cases post-percutaneous coronary intervention (PCI). Early diagnosis and surgical interventions are crucial for definitive diagnosis and management.
CASE SUMMARY: A 52-year-old man underwent PCI for an inferior wall myocardial infarction to the left circumflex artery (LCx) with a single sirolimus-eluting stent. He developed a fever 10 days later due to methicillin-sensitive Staphylococcus aureus. Imaging showed a coronary cameral fistula with a left atrial appendage (LAA) mycotic aneu…

Obesity affects about 39% of the global population, and bariatric surgery is a leading treatment option. As the number of primary bariatric procedures increases, so does the demand for revisional bariatric surgeries (RBSs), which are often more complex and associated with increased risks of complications. This case report details an unusual complication of RBS that surgeons should be aware of. A 41-year-old male patient with morbid obesity, after an initial sleeve gastrectomy, experienced weight regain due to gastric pouch dilatation and underwent conversion to mini-gastric bypass. Postoperati

Hemodialysis is the most commonly used renal replacement therapy worldwide, and arteriovenous fistulas (AVFs) are the preferred vascular access. The functional patency of AVFs directly determines the dialysis efficiency and quality of life of patients with end-stage renal disease (ESRD). However, in clinical practice, the failure of AVFs seriously affects the treatment prognosis of these patients. Vascular smooth muscle cells (VSMCs), as the main component of the vascular media, not only maintain the integrity and tension of the vascular wall under physiological conditions but also play a cruc…

An 86-year-old woman with end-stage renal disease was being dialyzed via a left brachiocephalic arteriovenous fistula. She developed a pseudoaneurysm at one of the angioaccess sites that did not resolve spontaneously. She developed significant pain and discomfort and presented for treatment. Ultrasound evaluation identified and characterized the pseudoaneurysm. Thrombin injection with ultrasound guidance successfully resolved the pseudoaneurysm, and at her follow-up her symptoms has resolved. Although off-label, thrombin injection remains an important tool in the management of pseudoaneurysms,…

Aorto-duodenal fistula (ADF) is a rare clinical condition characterized by abnormal communication between the 3rd or 4th portion of the duodenum and the aorta, typically associated with aneurysmal disease. The incidence of ADF is expected to increase, however, there remains a lack of consensus on the optimal approach for intestinal restoration. In this study, we present three cases of ADF and their respective treatments using three distinct surgical techniques. Initially, endovascular aortic repair was employed as a life-saving procedure, followed by elective open repair. The choice of surgica…

This case report describes a 62-year-old female with a history of heart failure with preserved ejection fraction (HFpEF) who presented to the emergency department with heart failure exacerbation. She was initially diagnosed with bilateral pleural effusions and treated with thoracentesis and diuretics. When follow-up indicated a lack of improvement, an ischemic workup was ordered. A transesophageal echocardiogram (TEE) revealed an abnormal connection between the right atrium and proximal aorta (aorto-atrial fistula), which was promptly fixed with surgical patching. This report also discusses th…

Bronchopleural fistula (BPF) is a serious and potentially fatal postoperative complication after pulmonary resection. Benign emptying of the postpneumonectomy space (BEPS) is a rare, self-limiting condition characterized by a sudden reduction in pleural fluid volume without evidence of fistula formation, observed after pneumonectomy. To date, BEPS-like conditions following lobectomy have not been reported. Because the clinical and radiological manifestations of BEPS overlap significantly with those of BPF, distinguishing between them is challenging. We present an unusual case of a BEPS-like ph…

The case reported in this paper is a vascular-bronchial fistula associated with fatal massive hemoptysis. The patient was rescued successfully by experts from a multidisciplinary team. The bronchoscopy physician cleared the accumulated blood in the airway timely and maintained the airway patency, and the anesthesiologist established artificial ventilation quickly; eventually, the thoracic surgeon performed an emergency thoracotomy to control the bleeding. It reflected the significance of multidisciplinary collaborative treatment.

Obstetric fistula is a major maternal health challenge in low-income countries like Ethiopia. Misperceptions about obstetric fistula delay seeking and accessing healthcare. Understanding community attitudes is crucial for prevention, early detection, and support for affected women. A community-based study was conducted from February 1 to April 26, 2024, among 640 women and men to assess attitudes and influencing factors. Participants were selected using a multistage sampling. Multivariable logistic regression analysis was performed, with a p-value < 0.05 considered statistically significant. T…

Pancreaticopleural fistula (PPF) is a rare complication of chronic pancreatitis that is often overlooked in the initial differential diagnosis of pleural effusion, resulting in delays to diagnosis and treatment. This manuscript reported a case of a patient with recurrent hemorrhagic pleural effusion, in whom markedly elevated levels of amylase and lipase in the pleural fluid led to a diagnosis of pancreaticopleural fistula via magnetic resonance cholangiopancreatography (MRCP). This case aimed to raise awareness among clinicians about the importance of considering uncommon etiologies in the di…

BACKGROUND: Autologous radio-cephalic arteriovenous fistulas (RC-AVFs) represent the first option for hemodialysis in China. However, they exhibit a high rate of failure to mature.
METHODS: A total of 196 first-time RC-AVFs were included. We analyzed preoperative clinical and ultrasonic characteristics and bioelectrical impedance to screen risk factors using univariate and multivariate logistic regression. Subsequently, we constructed a nomogram and employed bootstrap resampling for internal validation. Additionally, we developed a risk score equation using a simplified Framingham heart study…

BACKGROUND: Bronchopleural fistula is a rare and severe condition characterized by an abnormal connection between the bronchial tree and pleural space, often leading to significant morbidity and mortality. Although typically resulting from surgical procedures, trauma, or pulmonary infections, bronchopleural fistula caused by smoke inhalation injury is exceptionally uncommon. Smoke inhalation can lead to extensive thermal and chemical damage to the respiratory tract, increasing the risk of infection and necrotic tissue changes that can complicate recovery.
CASE PRESENTATION: We report a case o…
