Clesrovimab in the new era of RSV prevention: one season, one dose, one chance.

Respiratory syncytial virus (RSV) remains a leading cause of lower respiratory tract infection and hospitalization in infancy worldwide. Historically, prevention relied on monthly palivizumab administration for selected high-risk infants, despite evidence that a substantial proportion of severe RSV disease occurs in previously healthy term infants. The recent introduction of maternal RSV vaccination and long-acting monoclonal antibodies has transformed the prevention landscape. Clesrovimab (MK-1654; Enflonsia) is a long-acting monoclonal antibody designed to provide season-long protection foll

CNPY3 variants associated with a recessive developmental and epileptic encephalopathy affect inflammasome and TLR function.

BACKGROUND: Canopy FGF signalling regulator 3 (CNPY3) is an endoplasmic reticulum (ER)-resident chaperone essential for toll-like receptor (TLR) folding, trafficking and inflammasome activation. In rare paediatric cases, biallelic CNPY3 variants have been associated with developmental and epileptic encephalopathy (DEE), but available data remain sparse. METHODS: 50 individuals from 38 unrelated families with biallelic CNPY3 variants and DEE were evaluated for exome and genome findings, standardised clinical phenotype descriptions, EEG results, and neuroimaging data. A selection of variants un

Park Proximity and Emergency Department Visit Rates for Asthma and Acute Myocardial Infarction in Los Angeles County: A Cross-Sectional Ecological Study.

Background Parks are promoted as a public health resource, and neighborhoods with limited park access often face co-occurring air pollution and socioeconomic disadvantage. The primary objective of this study was to estimate the association between census tract park proximity and emergency department (ED) visit rates for asthma and acute myocardial infarction (AMI) in Los Angeles County, California, adjusted for fine particulate matter (PM2.5), ozone, and poverty. The two outcomes were co-primary, and no directional hypothesis was prespecified. A secondary objective was to assess whether the as

Pregnancy outcomes following surgical repair of female genital fistula in Uganda.

BACKGROUND: Women developing genital fistula and undergoing fistula repair in sub-Saharan Africa and South Asia are largely of reproductive age, and fistula prevalence is highest in countries with high fertility and where social values are placed on childbearing. Optimizing women's health following female genital fistula and surgical repair requires further understanding of the risks to subsequent pregnancies and how to mitigate them, to enable women to achieve their desired family size without additional morbidity. OBJECTIVE: We sought to contribute to the postfistula repair evidence base th

Structural model of determinants of medication adherence in elderly individuals with tuberculosis in Iran.

BACKGROUND AND OBJECTIVE: "Medication adherence" is essential for the successful treatment of tuberculosis. Numerous studies have indicated a higher probability of non-adherence to medication among elderly individuals with tuberculosis. As the elderly population continued to grow, non-adherence to medication in this group could lead to the failure of achieving the goal of tuberculosis eradication and make them a significant source of tuberculosis infection transmission in the community. Recognizing the significance of medication adherence in elderly individuals with tuberculosis, this study ai

Healthcare workers' perceptions and experiences of communicating with people over 50 years of age about vaccination: a qualitative evidence synthesis.

BACKGROUND: Infectious diseases are a major cause of illness and death among older adults. Vaccines can prevent infectious diseases, including against seasonal influenza, pneumococcal diseases, herpes zoster and COVID-19. However, the uptake of vaccination among older adults varies across settings and groups. Communication with healthcare workers can play an important role in older people's decisions to vaccinate. To support an informed decision about vaccination, healthcare workers should be able to identify the older person's knowledge gaps, needs and concerns. They should also be able to sh

Hypofractionated breast radiotherapy for 1 week versus 3 weeks (FAST-Forward): 10-year efficacy and late normal tissue effects from a multicentre, open-label, non-inferiority, phase 3, randomised controlled trial and 5-year efficacy results from a randomised axillary substudy.

BACKGROUND: FAST-Forward aimed to identify a 1-week adjuvant radiotherapy schedule for early-stage breast cancer that was as safe and efficacious as the standard 3-week schedule of 40 Gy in 15 fractions over 3 weeks. Primary analysis showed non-inferiority of 5-year ipsilateral breast recurrence for 26 Gy and 27 Gy in five fractions over 1 week, with 26 Gy also having similar results to 40 Gy for normal tissue effects. Here, we report 10-year outcomes of the FAST-Forward trial and 5-year efficacy outcomes of a substudy assessing the approach in patients requiring axillary treatment. METHODS:

An evaluation of the effect of an angiotensin-converting enzyme inhibitor on the growth rate of small abdominal aortic aneurysms: a randomised placebo-controlled trial (AARDVARK).

BACKGROUND: Although data are inconsistent, angiotensin-converting enzyme inhibitors (ACE-Is) have been associated with a reduced incidence of abdominal aortic aneurysm (AAA) rupture in analysis of administrative databases. OBJECTIVES: (1) To investigate whether or not the ACE-I perindopril (Coversyl arginine, Servier) reduces small AAA growth rate and (2) to evaluate blood pressure (BP)-independent effects of perindopril on small AAA growth and to compare the repeatability of measurement of internal and external aneurysm diameters. Comparing the effectiveness, cost-effectiveness and implementation of low-dose oral modified release morphine in people with chronic breathlessness: a synopsis of a RCT and process evaluation.

BACKGROUND: Chronic breathlessness is common and disabling in chronic medical conditions. There is good rationale to use opioids from neuroscience and exercise laboratory studies. OBJECTIVES: We assessed the 56-day effectiveness, cost-consequences and safety of long-acting oral morphine for breathlessness in people with cardio-respiratory diseases or cancer. METHODS: We conducted a parallel-group, randomised, placebo-controlled trial in 11 UK outpatient services, comparing 10-20 mg daily oral morphine with placebo (and blinded laxative) in people with moderate to severe breathlessness, with

Integrated community-based versus facility-based care for people with HIV, diabetes, and hypertension in sub-Saharan Africa (INTE-COMM): an open-label, multicountry, cluster-randomised trial.

BACKGROUND: In sub-Saharan Africa, the burden of diabetes and hypertension is high, alongside a high prevalence of HIV. Whether these conditions can be managed in an integrated way in the community is unknown. We aim to compare integrated community-based care with integrated facility-based care for people with HIV, diabetes, and hypertension in Tanzania and Uganda. METHODS: This open-label, multicountry, cluster-randomised trial was conducted in 14 primary care facilities across Tanzania and Uganda. Adults aged 18 years or older with a diagnosis of HIV, type 2 diabetes, or hypertension (or a

United Kingdom Oscillation Study: long-term outcomes of a randomised trial of two modes of neonatal ventilation.

BACKGROUND: One in 200 infants in the UK is born extremely prematurely, i.e. before 29 weeks of gestation. Seventy-five per cent of such infants survive, but many have long-term respiratory and/or functional problems. OBJECTIVES: To compare respiratory and functional outcomes of school-age children born extremely prematurely who received either high-frequency oscillation (HFO) or conventional ventilation (CV) immediately after birth to test the hypothesis that the use of HFO would be associated with superior small airway function at school age without adverse effects. DESIGN: Follow-up of a

Case management for integrated care of older people with frailty in community settings.

BACKGROUND: Ageing populations globally have contributed to increasing numbers of people living with frailty, which has significant implications for use of health and care services and costs. The British Geriatrics Society defines frailty as "a distinctive health state related to the ageing process in which multiple body systems gradually lose their inbuilt reserves". This leads to an increased susceptibility to adverse outcomes, such as reduced physical function, poorer quality of life, hospital admissions, and mortality. Case management interventions delivered in community settings are led b

Interventions for quitting vaping.

RATIONALE: There is limited guidance on the best ways to stop using nicotine-containing vapes (otherwise known as e-cigarettes) and ensure long-term abstinence, whilst minimising the risk of tobacco smoking and other unintended consequences. Treatments could include pharmacological interventions, behavioural interventions, or both. OBJECTIVES: To conduct a living systematic review assessing the benefits and harms of interventions to help people stop vaping compared to each other or to placebo or no intervention. To also assess how these interventions affect the use of combustible tobacco, and

Development and Validation of a Multidimensional Nursing Empowerment Scale (NES).

BACKGROUND: Empowerment in nurses is a multidimensional construct that has favorable results for both the patient and the employee and is emphasized in order to provide high-quality healthcare. For this concept to be translated into practice, measurement tools developed in this field are of critical importance. Although several validated instruments assess structural, psychological, or behavioral empowerment separately, each having been validated independently, using distinct samples, scoring frameworks, and item structures, no instrument evaluating all three dimensions simultaneously within a

Effects of an oral exercise intervention on pre-frailty or frailty in older people: a randomized clinical trial.

BACKGROUND: Frailty is often experienced by older adults, which can lead to long-term health problems. We aimed to examine associations with improvements in nutritional status, sarcopenia (age-related loss of skeletal muscle mass and strength), and frailty in four groups with different oral exercise frequencies. METHODS: We conducted a prospective, parallel multi-arm randomized controlled trial (Japan Registry of Clinical Trials (jRCT) 1062210063) to test the effects of oral exercise on frailty in older adults. Each intervention consisted of a standardized oral exercise protocol including nec
