BACKGROUND: Research into common pathological mechanisms and genetic factors is essential for better understanding, prevention and management of cardiovascular disease in congenital heart disease (CHD) survivors. This study aims to explore the possible causal associations between CHD and acquired cardiovascular diseases with the help of genetic instruments.
METHODS: This study utilized summary data from genome-wide association studies (GWASs) of CHD (including congenital anomalies of great vessels and heart septal defect) and seven different cardiovascular diseases, employing a two-sample Men

BACKGROUND: Hybrid approach in hypoplastic left heart syndrome (HLHS) implies placement of bilateral pulmonary artery bands and ductal stenting. This approach serves as a less invasive surgical-interventional alternative to the Norwood procedure with reported equivalent survival. Potential benefits include deferrable surgical risks and in some borderline left ventricle patients, when combined with a restrictive inter-atrial communication, it offers a chance for biventricular circulation.
CASE SUMMARY: Term newborn with total anomalous pulmonary venous connection (TAPVC), hypoplastic left hear

OBJECTIVE: To evaluate the role of ultrahigh-density mapping for conduction isthmus (CI) characterization in adult congenital heart disease (ACHD).
BACKGROUND: Catheter ablation remains suboptimal for ACHD with atypical intra-atrial reentrant tachycardias (IART) that can be challenging to define using existing mapping technology.
METHODS: An ultrahigh-density mapping system was selectively employed over a 1-year period for procedures involving noncavotricuspid isthmus-dependent-IART. A global activation histogram (GAH) was assessed for the ability to predict ablation targets. Procedural char

The long-term outcome of patients with congenitally corrected transposition of the great arteries is mainly determined by progressive morphologically tricuspid valve regurgitation, heart block, atrial arrhythmias, and/or systemic ventricular dysfunction. Situs abnormalities have been reported in ≤34% of cases, but whether clinical differences exist between a situs inversus and situs solitus arrangement has not yet been studied. The clinical records of 38 adults with congenitally corrected transposition of the great arteries (mean age 40 ± 15 years) followed for a mean period of 7.4 years were

OBJECTIVE: Children with ventricular septal defects (VSD) can have chronic volume overload, which can result in changes of left heart echocardiographic parameters. To evaluate the changes before and after surgical closure, the children were divided into three groups according to the degree of mitral regurgitation (MR), and their echocardiographic characteristics were reviewed at serial follow-up after surgical closure.
METHODS: The preoperative, and one-, three-, and 12-month postoperative echocardiographic data of 40 children who underwent surgical closure of VSD were retrospectively reviewe

Among different anatomical causes of subaortic obstruction, anomalous attachment of the mitral valve on ventricular septum is one of the rarest. We report our experience with 4 such cases of subaortic obstruction. Their age at first observation ranged between 1 and 6 months. All patients had viscero-atrial situs solitus, levocardia and normal atrio-ventricular and ventriculo-arterial connections. None had associated cardiac septal defects. Cases with atrio-ventricular canal were excluded. All patients showed a systolic ejection murmur and electrocardiographic features of left ventricular hyper

Although atrial switch is rarely performed today except as part of a "double switch" operation, there continues to be interest in the long-term outcome of the procedure because of the many Mustard and Senning survivors who are in follow-up. In contrast to most other reported series on atrial switch, this study by the Congenital Heart Surgeons Society (CHSS) is a prospective multiinstitutional study of patients encountered in a relatively short time frame during the late 1980s. Between 1985 to 1989, 20 centers with surgeons belonging to the CHSS prospectively registered patients <15 days of age

BACKGROUND: Aneurysm of the fossa ovalis is an out pouching, space-occupying, interatrial septal structure. The anatomic morphology and characteristics of this aneurysm are of interest for pathologists and cardiologists alike.
METHODS: We identified 33 specimens of adult hearts with a large size aneurysm of the fossa ovalis (length equal to or more than 10 mm from the plane of the atrial septum) in a registry of cardiovascular disease. Anatomic-morphologic features of these aneurysms were examined by macroscopic and histopathologic studies.
RESULTS: Nineteen aneurysms were from females (57%)

Arrhythmia in the adult congenital heart disease (ACHD) population is recognized as a major source of morbidity and mortality and has contributed to an increasing burden on the health care system over the past several decades. Abnormalities of impulse formation and propagation encountered in this population are distinct from those encountered in the general adult population. Such differences are related to the anatomic variability of congenital heart disease, the resulting postoperative hemodynamic disturbances, and the effects of prior cardiac surgery on the underlying myocardial substrate. We report a case of a successful orthotopic heart transplantation in a seven year old boy with heterotaxia syndrome, d-transposition of the great arteries, rudimentary right ventricle, double inlet left ventricle, valvular pulmonary stenosis, left-sided peripheral pulmonary artery stenosis, deficient atrial septation, and left-sided superior and inferior vena cava. In this case of congenital heart defects, three specific surgical procedures were necessary to make heart transplantation feasible. A synthetic patch was interposed in the single atrium to create two separate atria. To handle the mi

OBJECTIVE: Aortopulmonary window (APW) is a rare congenital heart defect. It occurs as an isolated cardiac lesion or in association with other cardiac anomalies and rarely with abnormal coronary arteries. The spectrum of cardiovascular anomalies associated with APW and overall management and outcome in the current era were reviewed.
METHODS: Between 2001 and 2018, all patients diagnosed with APW were included. Based on associated cardiovascular anomalies, those patients were divided into 2 groups: simple APW group and complex APW group (APW with associated other cardiovascular anomalies). All

BACKGROUND: Adult patients with small, unrepaired atrial septal defects (ASD) have higher late mortality than the background population. In this nationwide study, we characterize the late natural history of adults with small, unrepaired ASD.
METHODS: Using the Danish National Patient Registry, we included all Danish patients, diagnosed between 1953 and 2011 with an unrepaired ASD. Additionally, all patients, aged 18-65, were invited for clinical testing. Patients also completed a general health survey for comparison with the general population.
RESULTS: We identified 723 patients with a smal

Between 1985 and 2002, 60 patients (58% female) with a mean age of 20.3 +/- 12.1 years (range, 2-55 years) were treated for anterior mitral leaflet cleft. There was a primum atrial septal defect in 52 patients (87%) and a secundum type in 8 (13%). Concomitant cardiac defects were patent foramen ovale in 6 patients, cleft tricuspid valve in 3, ventricular septal defect in 2, cor triatriatum in 1, and persistent left superior vena cava in 1. Mean grade (1-4) of mitral insufficiency was 2.28 +/- 0.74. Atrial septal defects were closed with a pericardial patch in 45 patients, with a prosthetic pat

The formation of atrial hematomas with a mass effect in patients who have undergone cardiac surgery originates a clinical and hemodynamic syndrome that is difficult to assess. Hypotension with high right atrial pressure and equalization of pulmonary wedge pressure is not always present due to the irregular distribution of the hematoma in cardiac chambers. Transesophageal echocardiography is a useful diagnostic procedure for atrial hematomas, differentiating them from other similar clinical and hemodynamic situations like left ventricular or prosthetic valve dysfunction. We present five clinica

PURPOSE: Pulmonary hypertension (PH) is a progressive complication in adults with atrial septal defect (ASD), significantly impacting prognosis. Although right heart catheterization (RHC) is the gold standard for diagnosis, it is invasive. This study aimed to evaluate the diagnostic accuracy of various echocardiographic parameters compared to RHC for detecting PH specifically in adult patients with uncorrected secundum ASD.
METHODS: This cross-sectional diagnostic study utilized retrospective data from the COngenital HeARt Disease in adult and Pulmonary Hypertension (COHARD-PH) registry at Sa

BACKGROUND: There is variation in care of secundum atrial septal defects. Defects <3 mm and patent foramen ovale are not clinically significant. Defects >3 mm are often followed clinically and may require closure. Variation in how these lesions are monitored may result in over-utilisation of routine studies and higher than necessary patient charges.
PURPOSE: To determine utilisation patterns for patients with secundum atrial septal defects diagnosed within the first year of life and compare to locally developed optimal utilisation standard to assess charge savings.
METHODS: This was a retros
