Fetal Cardiac Remodeling and Subclinical Myocardial Dysfunction in Pregnancies Complicated by Placenta Accreta Spectrum: A Prospective Echocardiographic Study.

OBJECTIVES: To investigate the pattern of fetal cardiac remodeling and dysfunction associated with placenta accreta spectrum (PAS), a disorder characterized by abnormal placental angioarchitecture, and to test the hypothesis that PAS imposes a distinct hemodynamic burden on the fetal heart. METHODS: This prospective case-control study included 50 singleton pregnancies diagnosed with PAS and 50 gestational age-matched uncomplicated pregnancies as controls. Comprehensive fetal echocardiography was performed to assess structural parameters-including right and left ventricular free wall thickness

Clinico-imaging profile and correlations of global longitudinal strain abnormalities in Indian hypertrophic cardiomyopathy patients.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a prevalent genetic cardiac disorder with variable clinical expression. While global data are available, phenotypic characterization of Indian HCM patients remains limited. Speckle-tracking echocardiography (STE)-derived global longitudinal strain (GLS) may serve as a surrogate marker of myocardial fibrosis in settings with limited access to cardiac magnetic resonance (CMR). OBJECTIVES: To characterize the clinical and imaging features of Indian patients with HCM and evaluate the correlation of GLS with CMR-derived late gadolinium enhancement (

Cardiac pathology associated with hypertension and chronic kidney disease in aged cats.

Hypertension is a common condition in older cats, often secondary to chronic kidney disease (CKD). Although the heart is one of the organs damaged by hypertension, the pathology of the feline hypertensive (HT) heart has been poorly studied. The aim of this retrospective study was to describe the gross and microscopic pathology of hearts obtained from cats at post-mortem examination and to compare cats diagnosed with hypertension with cats of similar age and kidney function for which antihypertensive treatment was not deemed clinically necessary. Hearts from 32 cats were examined‒18 from HT and

Lujiao Formula attenuates cardiac hypertrophy by modulating AMPK-SIRT1 and PI3K-Akt signaling pathways.

Cardiac hypertrophy (CH) represents a fundamental morphological adaptation that precedes and drives the transition to heart failure (HF). The traditional formulation Lujiao Formula (LJF) has been widely prescribed in clinical practice for more than two decades to alleviate HF, largely through suppressing CH and ventricular remodeling. Yet, the molecular mechanisms that mediate its cardioprotective benefits remain incompletely understood. In this study, we sought to elucidate both the therapeutic efficacy and mechanistic basis of LJF against CH by integrating metabolomic and proteomic profiling

Angiotensin-Converting Enzyme Inhibitors or Angiotensin Receptor Blockers After Transcatheter Aortic Valve Replacement: A Meta-Analysis.

BACKGROUND: Persistent left ventricular hypertrophy after transcatheter aortic valve replacement (TAVR) has been associated with poor outcomes. Angiotensin-converting enzyme inhibitors (ACEIs) and angiotensin receptor blockers (ARBs), due to their favorable effects on ventricular remodeling, have been hypothesized to improve outcomes post-TAVR, yet there are no recommendations regarding their use. OBJECTIVES: This study aimed to compare the outcomes of patients receiving ACEIs/ARBs with those not receiving ACEIs/ARBs after TAVR. METHODS: We performed a literature search on PubMed and Cochran

Right predominant electrical remodeling in a pure model of pulmonary hypertension promotes reentrant arrhythmias.

BACKGROUND: Electrophysiological (EP) properties have been studied mainly in the monocrotaline model of pulmonary arterial hypertension (PAH). Findings are confounded by major extrapulmonary toxicities, which preclude the ability to draw definitive conclusions regarding the role of PAH per se in EP remodeling. OBJECTIVE: The purpose of this study was to investigate the EP substrate and arrhythmic vulnerability of a new model of PAH that avoids extracardiopulmonary toxicities. METHODS: Sprague-Dawley rats underwent left pneumonectomy (Pn) followed by injection of the vascular endothelial grow

Prediction and Prevention of Sudden Death in Young Patients (<20 years) With Hypertrophic Cardiomyopathy.

Highly reliable identification of adults with hypertrophic cardiomyopathy (HC) at risk for sudden death (SD) has been reported. A significant controversy remains, however, regarding the most reliable risk stratification methodology for children and adolescents with HC. The present study assesses the accuracy of SD prediction and prevention with prophylactic implantable cardioverter-defibrillators (ICDs) in young HC patients. The study group is comprised of 146 HC patients <20 years of age evaluated consecutively over 17 years with prospective risk stratification and ICD decision-making. We rel

Value of Genetic Testing for the Prediction of Long-Term Outcome in Patients With Hypertrophic Cardiomyopathy.

Pathogenic gene mutations are found in about 50% of patients with hypertrophic cardiomyopathy (HC). Previous studies have shown an association between sarcomere mutations and medium-term outcome. The association with long-term outcome has not been described. The aim of this cohort study was to assess the long-term outcomes of patients with genotype positive (G+) and genotype negative (G-) HC. The study population consisted of 626 patients with HC (512 probands and 114 relatives) who underwent phenotyping and genetic testing from 1985 to 2014. End points were all-cause mortality, cardiovascular

Inhibition of Oct 3/4 mitigates the cardiac progenitor-derived myocardial repair in infarcted myocardium.

BACKGROUND: Recent evidence has demonstrated that cardiac progenitor cells play an essential role in the induction of angiomyogenesis in infarcted myocardium. We and others have shown that engraftment of c-kit(+) cardiac stem cells (CSCs) into infarcted hearts led to myocardium regeneration and neovascularization, which was associated with an improvement of ventricular function. The purpose of this study is aimed at investigating the functional role of transcription factor (TF) Oct3/4 in facilitating CSCs to promote myocardium regeneration and preserve cardiac performance in the post-MI heart.

Cyanotic congenital heart disease with decreased pulmonary blood flow in children.

Of the "five T's" of cyanotic congenital heart disease--tetralogy of Fallot, TGA, TAPVC, truncus, and tricuspid valve abnormalities (tricuspid atresia, stenosis, and displacement)--the first and last are commonly associated with diminished PBF. The four features that comprise tetralogy of Fallot--right ventricular hypertrophy, VSD, overriding aorta, and subpulmonary stenosis--are all secondary to a single morphogenetic defect: failure of expansion of the subpulmonary conus. This also explains the variability in clinical presentation. When neonates need intervention, shunts are usually performe

Genetic insights into radiomic and proteomic changes under β1-blockers treatment in hypertensive heart disease and hypertrophic cardiomyopathy.

BACKGROUND: Hypertensive heart disease (HHD) and hypertrophic cardiomyopathy (HCM) are characterized by left ventricular hypertrophy and diastolic dysfunction. Despite overlapping remodeling features, their distinct mechanisms and therapeutic responses remain unclear. This study integrated genetic, imaging, and proteomic data to identify key mediators underlying β1-adrenergic receptor blockers (β1-blockers)-related therapeutic heterogeneity between HHD and HCM. METHODS: Genetic instruments for β1-blockers were derived from two genome-wide association studies and integrated with cardiac magnet

Clinical Profile and Outcomes of Patients with Chronic Kidney Disease on Chronic Hemodialysis Hospitalized for Acute Coronary Syndrome in a Tertiary Public Hospital in the Philippines.

INTRODUCTION: Acute coronary syndrome (ACS) and end-stage renal disease (ESRD) are both prevalent globally. The diagnosis and management of ACS in ESRD is difficult because the interplay of cardiovascular and renal disease is complicated. The guidelines for ACS may not be applicable to the ESRD population because the trials from which these are drawn mostly excluded ESRD patients. OBJECTIVE: To determine the clinical profile and outcomes of CKD patients on dialysis admitted for ACS in the Philippine General Hospital (PGH). METHODS: We did a retrospective cohort study and employed a retrospec

Phenotypic Selectivity of Artificial Intelligence-enhanced Electrocardiography in Cardiovascular Diagnosis and Risk Prediction.

INTRODUCTION: Artificial intelligence (AI)-enhanced electrocardiogram (ECG) models are designed to detect specific anatomical and functional cardiac abnormalities. Understanding the selectivity of their phenotypic associations is essential to inform their clinical use. Here, we sought to assess whether AI-ECG models function as condition-specific classifiers or broader cardiovascular risk markers. METHODS: We included four distinct study populations, drawn from both electronic health records (EHR) and prospective cohort studies. We deployed six image-based AI-ECG models, including five valida

Electrocardiographic Strain and Relationship with Left Ventricular Remodelling and Clinical Outcomes in Patients with Aortic Stenosis Undergoing Transcatheter Aortic Valve Implantation.

INTRODUCTION: Left ventricular (LV) remodelling and fibrosis are known to occur in patients with aortic stenosis (AS) and are linked to post-intervention outcomes. These myocardial changes may be detected upon the routine 12-lead electrocardiogram (ECG) by the presence of a LV strain pattern (LVS-ECG). Although LVS-ECG has been related to excessive cardiovascular morbidity and mortality in multiple patient populations, there is currently a dearth of data upon its impact in patients undergoing transcatheter aortic valve implantation (TAVI). The aim of the current study was to investigate the pr

Vaccine-linked chemotherapy improves cardiac structure and function in a mouse model of chronic Chagas disease.

INTRODUCTION: Chagas disease, caused by chronic infection with the protozoan parasite Trypanosoma cruzi, affects 6-7 million people worldwide. The major clinical manifestation of Chagas disease is chronic Chagasic cardiomyopathy (CCC), which encompasses a spectrum of symptoms including arrhythmias, hypertrophy, dilated cardiomyopathy, heart failure, and sudden death. Current treatment is limited to two antiparasitic drugs, benznidazole (BNZ) and nifurtimox, but both have limited efficacy to halt the progression of CCC. We developed a vaccine-linked chemotherapy strategy using our vaccine consi
