Mycobacterium tuberculosis bacteremia in a cohort of hiv-infected patients hospitalized with severe sepsis in uganda–high frequency, low clinical suspicion [corrected] and derivation of a clinical prediction score.

BACKGROUND: When manifested as Mycobacterium tuberculosis (MTB) bacteremia, disseminated MTB infection clinically mimics other serious blood stream infections often hindering early diagnosis and initiation of potentially life-saving anti-tuberculosis therapy. In a cohort of hospitalized HIV-infected Ugandan patients with severe sepsis, we report the frequency, management and outcomes of patients with MTB bacteremia and propose a risk score based on clinical predictors of MTB bacteremia. METHODS: We prospectively enrolled adult patients with severe sepsis at two Ugandan hospitals and obtained

Policy reform to shift the health and human rights environment for vulnerable groups: the case of Kyrgyzstan's Instruction 417.

BACKGROUND: Police activities shape behavior and health outcomes among drug users, sex workers, and other vulnerable groups. Interventions to change the policing of drug consumption and sex work in ways that facilitate public health programming and respect for human rights have included policy reforms, education, and litigation. In 2009, the Kyrgyz government promulgated "Instruction 417," prohibiting police interference with "harm reduction" programs, re-enforcing citizen rights, addressing police occupational safety concerns, and institutionalizing police-public health collaboration. Harm reduction and law enforcement in Vietnam: influences on street policing.

BACKGROUND AND RATIONALE: The HIV epidemic in Vietnam has from its start been concentrated among injecting drug users. Vietnam instituted the 2006 HIV/AIDS Law which includes comprehensive harm reduction measures, but these are unevenly accepted and inadequately implemented. Ward police are a major determinant of risk for IDUs, required to participate in drug control practices (especially meeting quotas for detention centres) which impede support for harm reduction. We studied influences on ward level police regarding harm reduction in Hanoi to learn how to better target education and structur

Phase II study of gemcitabine for treatment of patients with advanced stage marginal zone B-cell lymphoma: Consortium for Improving Survival of Lymphoma (CISL) trial.

BACKGROUND: Therapeutic approaches to marginal zone B-cell lymphoma (MZL) continue to evolve. Localized MZL responds favorably to local treatments, including surgery and/or local radiation therapy. However, MZL manifests as a disseminated disease in one-third of the cases at diagnosis. Moreover, relapses involving distant sites after local therapy have been reported previously. Therefore, the search for effective forms of systemic therapy is a critical issue. We conducted this multi-center, phase II trial to assess the efficacy and safety of gemcitabine single chemotherapy for patients with st

CardioGAP+: bridging the gap between patient identification and therapy implementation in cardiovascular risk.

INTRODUCTION: In the face of escalating medical complexity, the development of simple strategies that allow the optimization of resources becomes more urgent than ever. This is particularly evident in cardiovascular diseases (CVD), where despite successful risk stratification and mitigation strategies, elevated mortality rates persist among socioeconomically disadvantaged groups. Efforts to prevent CVD events rely on early identification of at-risk individuals and timely implementation of risk-mitigating measures. Persistent challenges in health systems to identify and intervene with high-risk

Multicentre randomised controlled trial protocol in China comparing postless hip arthroscopy with conventional perineal post-assisted hip arthroscopy to reduce perineal complications in patients undergoing hip arthroscopy for femoroacetabular impingement: the POSTLESS-HIP trial.

INTRODUCTION: Perineal complications-including nerve injury (eg, pudendal neurapraxia), soft tissue injury and sexual or urinary dysfunction-are well-documented adverse effects of conventional hip arthroscopy using a perineal post. Postless hip arthroscopy avoids direct perineal pressure, but high-quality randomised evidence is lacking. METHODS AND ANALYSIS: This multicentre, prospective, parallel-group, randomised controlled trial will enrol 270 patients aged 15-60 years with femoroacetabular impingement syndrome requiring hip arthroscopy. Participants will be randomised 1:1 via a centralise
