Management of Inflammatory Bowel Disease with History of Cancer.

Background/Objectives: Because of the chronic course of the disease, clinicians managing IBD frequently encounter patients with a prior or newly diagnosed cancer. This can be related to the specific background cancer risk in that subject, aging, familial/genetic factors, or ongoing chronic inflammation. However, a potential influence of some therapeutic agents should also be considered. This setting, in the absence of controlled trials and few open series reports available, raises issues such as correct screening, prevention, and surveillance, but also eventual modification or adaptation of th

Splenic Abscesses With Different Modalities of Management: A Case Series.

Splenic abscesses are rare but potentially fatal infections, particularly in individuals with diabetes mellitus. Management strategies vary based on the abscess size, complexity, and response to the initial treatment. We report five patients with diabetes and splenic abscesses who were managed at a tertiary care centre. All patients presented with left upper quadrant pain, fever, and leukocytosis. The diagnosis was confirmed via ultrasonography and contrast-enhanced computed tomography. One patient was managed conservatively with antibiotic therapy. Two patients underwent successful image-guid

Urinary incontinence in older adults: what you need to know.

Incontinence is a problem that can affect individuals of all ages. However, it is more frequently seen in older adults. It is a common geriatric syndrome, and its investigation should form part of a comprehensive geriatric assessment. Continence problems are usually ignored by patients and not reported to a medical professional, but with simple assessment and even simpler interventions, a significant improvement can be made for those living with incontinence. Most cases need a simple clinical assessment consisting of a good history and basic examination covering the abdomen, external genitalia

Practical Recommendations of the Obesity Management Task Force of the European Association for the Study of Obesity for the Post-Bariatric Surgery Medical Management.

Bariatric surgery is today the most effective long-term therapy for the management of patients with severe obesity, and its use is recommended by the relevant guidelines of the management of obesity in adults. Bariatric surgery is in general safe and effective, but it can cause new clinical problems and is associated with specific diagnostic, preventive and therapeutic needs. For clinicians, the acquisition of special knowledge and skills is required in order to deliver appropriate and effective care to the post-bariatric patient. In the present recommendations, the basic notions needed to pro

Management of priapism in a child with sickle cell anemia; successful outcome using epidural analgesia.

PURPOSE: To describe the successful management of priapism secondary to sickle cell anemia in a child using neuraxial analgesia provided via an epidural catheter. CLINICAL FEATURES: A seven-year-old male presented with chest crisis and priapism which, following hemoglobin electrophoresis led to a new diagnosis of sickle cell anemia. Epidural management was attempted as an alternative to surgery after failure of more conventional medical and surgical methods to treat the priapism. The patient's clinical condition improved with this intervention and together with further conservative therapy re

[Management of amyotrophic lateral sclerosis].

There is no specific treatment for patients with amyotrophic lateral sclerosis. Nevertheless several associations of patients have been created to stimulate the research to find the causes and treatments of the disease and to help the patients. In spite of the absence of a specific treatment, new symptomatic therapeutic resources have been recently introduced in order to modify the management and in some cases to improve the condition of patients with ALS. Among these resources, it is worth outlining those which are devoted to the medulla trouble such as the permanent gastric tubes. The respir

Sinonasal Cholesteatoma Presenting Within the Maxillary Sinus: A Case Report.

OBJECTIVES: Epidermal inclusion cysts are a common type of benign cutaneous cyst. When located in the mastoid cavity, they are more commonly known as cholesteatomas. However, it is extremely rare to be present within the paranasal sinuses, with 20 known cases involving the maxillary sinus. This is a case presentation of an epidermal inclusion cyst of the maxillary sinus, describing its clinical presentation, identifying pathological correlations, and highlighting the importance of early recognition and management. STUDY DESIGN: A case report. METHODS: A retrospective review of the medical re

Transport of the Open Abdomen: A Review.

OBJECTIVE: An open abdomen (OA) is a critical surgical condition in which the abdominal wall is intentionally left open to prevent or manage intra-abdominal hypertension or abdominal compartment syndrome or facilitate staged re-exploration. Although OA management is well described in surgical literature, the safe transport of patients with an OA-particularly in air medical and critical care transport settings-presents unique challenges that are less frequently addressed. METHODS: This review summarizes current evidence and best practices for the transport of patients with an OA, focusing on p

Sham surgery as an alternative procedure for unexplained pain following transvenous implantable cardioverter defibrillator implantation: case report.

BACKGROUND: Persistent pain after implantable cardioverter-defibrillator (ICD) implantation is uncommon and often unexplained after ruling out device-related or musculoskeletal causes. Psychological factors can contribute to symptom persistence, complicating treatment. In rare cases, sham surgery has been considered to address psychogenic pain. This report explores its role in managing atypical post-ICD implantation pain. CASE SUMMARY: A 76-year-old female with hypertrophic cardiomyopathy, atrial fibrillation, and a history of ICD implantation presented with persistent left parasternal pain r

Pharmacological Management of the Dentally Anxious Patient.

Dental anxiety is a complex psychological problem in which patients associate negative feelings with the dental experience and is determined clinically through physiological and behavioral presentations. Self-reporting along with questionnaires and patient interviews can indicate the level of a patient's dental anxiety and guide the dentist in determining the appropriate management. Nonpharmacological methods of managing dental anxiety should be exhausted before pharmacological sedative techniques are considered. Nitrous oxide with oxygen is frequently used in the dental setting because of its

[Troyakov. Experience in using hytolysin in the integrated management of urinary tract infections and methapylactics of nephrolithiasis].

RELEVANCE: Urinary tract infection (UTI) are a risk factor for diseases leading to impairment of renal function and kidney stone disease (KSD). Growing resistance of uropathogens to antibacterial agents is a challenging issue in most countries of the world. Urolithiasis is the second most prevalent urologic condition following urinary tract infections and has a pronounced tendency to recur. Rational stone metaphylaxis leads to a significant reduction in the incidence of recurrent stones. In recent decades, there has been a markedly increasing interest in plant-based therapies in managing urolo

A new horizon for the use of non-invasive ventilation in patients with acute respiratory distress syndrome.

Non-invasive ventilation (NIV) has assumed an important role in the management of acute respiratory failure (ARF). NIV, compared with standard medical therapy, improves survival and reduces complications in selected patients with ARF. NIV represents the first-line intervention for some forms of ARF, such as chronic obstructive pulmonary disease (COPD) exacerbations and acute cardiogenic pulmonary edema. The use of NIV is also well supported for immunocompromised patients who are at high risk for infectious complications from endotracheal intubation. Selection of appropriate patients is crucial

Managing urinary incontinence: what works?

Urinary incontinence is a common condition, which, although not life-threatening, impairs the health-related quality of life of affected individuals. All women complaining of incontinence require a basic assessment and those with complex or refractory symptoms may benefit from urodynamic studies. Initial treatment includes lifestyle advice, behavioral modifications, bladder retraining and pelvic floor muscle training. For those women with persistent stress urinary incontinence following conservative therapy, surgical management might be considered. The development of the minimally invasive, re

Acute coronary syndromes: diagnosis and management, part I.

The term acute coronary syndrome (ACS) refers to any group of clinical symptoms compatible with acute myocardial ischemia and includes unstable angina (UA), non-ST-segment elevation myocardial infarction (NSTEMI), and ST-segment elevation myocardial infarction (STEMI). These high-risk manifestations of coronary atherosclerosis are important causes of the use of emergency medical care and hospitalization in the United States. A quick but thorough assessment of the patient's history and findings on physical examination, electrocardiography, radiologic studies, and cardiac biomarker tests permit

Using nominal group technique to identify barriers, facilitators, and preferences among patients seeking treatment for opioid use disorder: A needs assessment for decision making support.

BACKGROUND: The opioid crisis requires rapid scale-up of evidence-based interventions to treat opioid use disorder (OUD), of which pharmacologic therapies with methadone, buprenorphine or long-acting naltrexone are most effective. With recently-developed formulations, there are unprecedented treatment options. Even when pharmacologic treatment is accessible, however, uptake remains low, suggesting individual-level barriers. Decision aids are an evidence-based strategy that may overcome these barriers. This study aims to inform such a tool by describing and rank-ordering patients' consideration

Pediatric Heart Failure Pharmacotherapy: Transformative Insights for the Future.

This review aims to summarize the status and future directions of pediatric heart failure (HF) pharmacotherapy. Notably, managing HF in children presents unique challenges due to heterogeneous etiologies and a longstanding paucity of pediatric-specific data. While historically reliant on adult-derived evidence, current treatment strategies are evolving through an integration of novel and pediatric-focused therapies. Indeed, present pediatric HF algorithms, adapted from adult guidelines, now include four pharmacologic pillars: angiotensin-converting enzyme (ACE) inhibitors/angiotensin receptor

[Positive interferon-gamma release assay and immunosuppression: guideline-based diagnosis and treatment of tuberculosis infection].

With the growing use of immunosuppressive therapies, clinicians are increasingly faced with the question of how to manage positive interferon-gamma release assays (IGRA). A tuberculosis infection (TBI, formerly referred to as "latent tuberculosis") is clinically silent and non-contagious but can reactivate as active tuberculosis under immunosuppressive conditions. Diagnosis involves either an IGRA or a tuberculin skin test (TST), alongside microbiological and imaging studies to exclude active disease. Screening is recommended for individuals at increased risk, including those living with human
