This report documents a case of a fatal primary malignant neoplasm of the liver with metastases to the lymph nodes of the porta hepatis and the pubic bone. Profound, intractable hypoglycemia was seen during the course of the disease. No immunoassayable insulin was found in the blood during episodes of severe, symptomatic hypoglycemia. Acute renal failure developed in a 47-year-old man during the second course of a high-dose methotrexate sodium (MTX) treatment for chondrosarcoma of the pelvis with pulmonary metastasis. Uninterrupted peritoneal dialysis for seven days had a negligible effect on plasma MTX. Although high-dose leucovorin calcium prevented severe myelosuppression and mucositis, the patient died 12 days after receiving MTX. Renal histological studies showed severe tubulointerstitial damage consistent with MTX toxicity. Better alternatives for removing MTX in patients with renal failure are clearly needed.

Two cases of basal cell carcinoma had diffused infiltration that included not only perineural but endoneurial invasion. One patient presented with trigeminal neuralgia, and the other patient developed localized neuralgia when the tumor recurred. Nests and indian files of carcinoma cells were in the endoneurium, a region that anatomically is devoid of lymphatics. The involved nerves displayed axonal degeneration. Such intraneural invasion by basal cell carcinoma is very rare, even in the scelerosing variety of basal cell carcinoma.

Twenty-five patients subjected to pelvic node dissection for urologic malignancies underwent bilateral pedal lymphangiography preoperatively. Postoperatively, 50 samples of the nodes selectively removed from the regions surrounding the obturator nerve were radiographed. All 50 samples revealed the presence of radiopaque dye. The lymph nodes surrounding the obturator nerve represent the first point of lymphatic metastases in carcinoma of the prostate and the bladder. In two cases of typical small-cell bronchial carcinoma (one of them peripheral, small and clinically not diagnosed) destructive parenchylmal metastases in the pancreas, carcinomatous lymphoangiosis and infiltration of the larger excretory ducts by tumour cells were demonstrated. A male forestry worker presented with chest pain followed by severe continuing haemoptysis and an extensive bilateral nodular pulmonary infiltrate. From 1960 to 1972, 639 patients have undergone bilateral oophorectomy for carcinoma of the breast. A case of malignant anorectal melanoma is presented together with a short review of the literature. The lesion arises from the melanocytes normally present within the squamous epithelium of the anal canal. It presents as a small polypoid tumor and spreads rapidly into rectum, perirectal tissues, pelvic and inguinal nodes. It should be considered in the differential diagnosis of sessile lesions located or involving the anal canal.
