Society of Dermatology Hospitalists supportive care guidelines for the management of Stevens-Johnson syndrome/toxic epidermal necrolysis in adults.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are life-threatening conditions with high morbidity and mortality. Supportive care management of SJS/TEN is highly variable. A systematic review of the literature was performed by dermatologists, ophthalmologists, intensivists, and gynecologists with expertise in SJS/TEN to generate statements for supportive care guideline development. Members of the Society of Dermatology Hospitalists with expertise in SJS/TEN were invited to participate in a modified, online Delphi-consensus. Participants were administered 9-point Likert sca

[Functional Health Pattern Model Based Care Plan for a Depression Diagnosed Patient].

Depression is the most frequent disorder among mental disorders, with frequent attacks, high suicidal risks, causing loss of labour-force, as well as to loss of serious physical and psycho-social skills, and also is economically and socially overcosting. The aim of nursing care for depression diagnosed patients is to provide security, to support their independence as much as possible, to help the patient rebuild the harmony at pre-diagnosis level, and to continue their well-being. This case applied to the hospital after an attempt to commit suicide, having continuous thoughts of suicide, guilt

Stress hyperglycemia in pediatric critical illness: the intensive care unit adds to the stress!

Stress hyperglycemia (SH) commonly occurs during critical illness in children. The historical view that SH is beneficial has been questioned in light of evidence that demonstrates the association of SH with worse outcomes. In addition to intrinsic changes in glucose metabolism and development of insulin resistance, specific intensive care unit (ICU) practices may influence the development of SH during critical illness. Mechanical ventilation, vasoactive infusions, renal replacement therapies, cardiopulmonary bypass and extracorporeal life support, therapeutic hypothermia, prolonged immobility,

Amyloid and the GI tract.

Amyloidosis is characterized by the extracellular deposition of an abnormal fibrillar protein, which disrupts tissue structure and function. Amyloid may be localized to a single organ, such as the GI tract, or be systemic where the amyloid type is defined by the respective fibril precursor protein. Among patients with systemic amyloidosis, histological involvement of the gastrointestinal (GI) tract is very common but often subclinical. The presence and pattern of GI symptoms varies substantially, not only between the different amyloid types but also within them. GI presentations are frequently

Nutritional immunomodulation of acute pancreatitis.

Despite the great advances in our understanding of the pathophysiology of acute pancreatitis, no specific therapy has emerged, and treatment remains supportive. In patients with the severe form of the disease, in which mortality remains high at 20% to 30%, the function of the upper gastrointestinal tract is disturbed due to extrinsic compression by the inflamed and swollen pancreas, and normal eating is impossible. Such patients often develop multiple organ failure, necessitating intensive-care management and artificial ventilation for weeks on end. In this setting, protein catabolism will rap

Dietary modifiers of cancer.

The foregoing review of current risk factors associated with single nutrients and nutritional status in human populations, supported by animal studies, point clearly to a possibility for manipulation of nutrients in our diet, which may be a major tool for prevention of cancer. Migrant populations exhibit changes in incidence and frequency of certain types of cancer indicating external causes for cancer with diets high on the list of suspects. The foods we eat are probably the most important risk factors that human populations encounter and they offer a major focus for hope of preventing cancer

Supportive Care Needs of Newly Diagnosed Cancer Patients in a Comprehensive Cancer Center: Identifying Care Profiles and Future Perspectives.

The prompt introduction of supportive care for patients with cancer leads to a better quality of life, potential survival benefits, and improvements in treatment safety. Considering that patients' needs vary, descriptive assessments could serve as a compass for an efficient and prompt healthcare response. The aim of this study was to identify supportive care needs in newly diagnosed patients according to cancer type. A retrospective study was conducted by collecting data from the case consultation and medical records of a comprehensive cancer center in France. Patients' needs were divided into

Nutritional supplementation for nonalcohol-related fatty liver disease: a network meta-analysis.

BACKGROUND: The prevalence of non-alcohol-related fatty liver disease (NAFLD) varies between 19% and 33% in different populations. NAFLD decreases life expectancy and increases risks of liver cirrhosis, hepatocellular carcinoma, and the requirement for liver transplantation. Uncertainty surrounds relative benefits and harms of various nutritional supplements in NAFLD. Currently no nutritional supplement is recommended for people with NAFLD. OBJECTIVES: • To assess the benefits and harms of different nutritional supplements for treatment of NAFLD through a network meta-analysis • To generate r

Effects of different dietary restriction regimens on cognitive function and pathological markers in Alzheimer's disease mouse models: A systematic review and meta-analysis.

Dietary restriction (DR) has emerged as a promising non-pharmacological intervention for Alzheimer's disease (AD). This systematic review and meta-analysis provides the first comprehensive comparison of five dietary restriction regimens in Alzheimer's disease mouse models. Analysis of 23 studies demonstrates that caloric restriction yields the most consistent benefits. While intermittent fasting exhibits model-dependent efficacy-improving recognition memory but exacerbating neuroinflammation in 5 ×FAD models. The fasting-mimicking diet showed the largest effect size. From a geroscience perspec

Optimizing Nutrition Delivery in Critically Ill Patients on Vasopressors.

Early enteral nutrition (EN) in critically ill patients receiving vasopressors remains controversial due to concerns about gut ischemia and feeding intolerance. Current evidence supports a cautious, individualized approach emphasizing resuscitation status, vasopressor dose, and feeding strategy. Observational and randomized trials suggest trophic EN may be safe and beneficial in patients postresuscitation on low-to-moderate norepinephrine doses, whereas full-dose EN on higher-dose vasopressors increases gastrointestinal complications. Practical recommendations include gastric-only trophic feed

Medical Management for Emphysematous Gastritis: Is It Time for a Standardized Approach?

Emphysematous gastritis (EG) is a rare form of gastritis with a high mortality rate caused by gas-forming organisms that infect the stomach wall. Due to the rarity of the condition, no established management guidelines exist. Historically, surgical exploration was favored, but more recently, the management has shifted to a non-surgical approach with antibiotics, gastric decompression, and bowel rest. Here, we detail the case of a 55-year-old male with EG in which medical management alone led to the complete resolution of his illness without complication. This case supports the efficacy of a me

Non-biopsy Strategy for the Diagnosis of Celiac Disease in Adults: A Narrative Review.

Celiac disease (CeD) diagnosis is a complicated process, requiring a multi-step procedure and a high level of clinical knowledge. Some scientific societies, mainly from Europe and North America, have proposed appropriate guidelines for the diagnosis and management of CeD. Since duodenal biopsy is particularly challenging for children, guidelines of the European Society for Pediatric Gastroenterology, Hepatology, and Nutrition, presented in 2012 and updated in 2020, have made it possible to avoid the biopsy in symptomatic pediatric patients with high levels of IgA anti-transglutaminase. Several

Fatigue, bleeding tendency and osteoporosis in an adolescent: a diagnostic conundrum.

Scurvy is a rare nutritional deficiency, particularly in developed nations. Sporadic cases are still reported, particularly among alcoholics and malnourished. Herein we present an unusual case of a previously healthy 15-year-old Caucasian girl, who was recently hospitalized for low velocity spine fractures, back pain and stiffness over several months and rash for 2 years. She was later diagnosed with scurvy and osteoporosis. Dietary modifications were instituted together with supplementary vitamin C, supportive treatment with regular dietician review and physiotherapy. Gradual clinical recover

[Toxic hepatitis induced by Polygonum multiflorum].

Toxic liver injury is a common cause of acute hepatitis. Here we report a case of 33-year old female with toxic hepatitis caused by unusual agent- extract of chinese plant Polygonum multiflorum. The patient presented with clinical signs of nausea and icterus and laboratory signs of hepatocellular damage following 2 months of readministration of Polygonum mulltiflorum pills. All other causes of hepatocellular damage were excluded. The causality between hepatocellular damage and Polygonum multiflorum ingestion was supported by early recovery after discontinuation, by international scoring system

Biodegradation of propargite by Pseudomonas putida, isolated from tea rhizosphere.

Biodegradation of miticide propargite was carried out in vitro by selected Pseudomonas strains isolated from tea rhizosphere. A total number of 13 strains were isolated and further screened based on their tolerance level to different concentrations of propargite. Five best strains were selected and further tested for their nutritional requirements. Among the different carbon sources tested glucose exhibited the highest growth promoting capacity and among nitrogen sources ammonium nitrate supported the growth to the maximum. The five selected Pseudomonas strain exhibited a range of degradation

The influence of seminal plasma on ovarian function in pigs--a novel inflammatory mechanism?

Seminal plasma is increasingly recognised as contributing to the reproductive process in roles apart from that of providing nutritive support and transport for spermatozoa. Seminal components elicit inflammatory responses in the female reproductive tract, including altered patterns of cytokine secretion, which have consequences for early embryo development and implantation. This review examines evidence, generated principally in the porcine model, for a more recently recognized role for seminal plasma in regulating the temporal kinetics of ovulation, corpus luteum development and steroid produ

Congenital chylothorax.

Congenital chylothorax is a rare condition in which chyle accumulates in the pleural space because of an intrauterine obstruction or anomalies of the thoracic duct. This paper presents a case of congenital chylothorax diagnosed antepartum echographically. The patient's history revealed a previous sibling with a similar diagnosis. The baby developed respiratory distress after delivery and the diagnosis was established by thoracentesis. Computed tomography of the chest and nuclear lymphangiography were obtained to evaluate the origin of the pleural effusion, but a congenital fistula or other pat
