Treatment for proximal humeral fractures with percutaneous plating: our first results.

INTRODUCTION: Proximal humeral fractures are common lesions; there is no generally accepted strategy about the treatment for displaced and unstable two- to four-part fractures. We have nowadays many different surgical solutions, ranging from percutaneous pinning to shoulder arthroplasty. Percutaneous plating can be a good solution to treat some of these fractures using a minimally invasive technique and performing stable fixation that can allow early mobilization. Purpose of this paper is to evaluate the results of our first cases of percutaneous plating in proximal humeral fractures in order

Severely impacted valgus proximal humeral fractures.

BACKGROUND: The functional results associated with nonoperative treatment of severely impacted valgus fractures of the proximal part of the humerus are poor, and these injuries are difficult to treat with minimally invasive percutaneous fixation techniques. The aim of this study was to review the functional and radiographic results and complications of a new operative technique in a series of twenty-five patients. METHODS: Over a two-year period, we treated twenty-nine patients with a severely impacted valgus fracture of the proximal part of the humerus. Three patients were lost to follow-up

The femoral supracondylar nail: preliminary experience.

The treatment of supracondylar fractures of the femur with an intramedullary nail presents some theoretical advantages. Compared to plate osteosynthesis, intramedullary fixation requires less extensive dissection and is biomechanically more favorable. In the elderly patient, these characteristics seem important since bone quality, extensive procedures and bone grafting remain problematic. Since August 1994 we have treated 16 consecutive closed fractures of the distal femur (A.O. type: 7 A.1, 2 A.3, 4 C.1, and 3 C.2) with the Green-Seligson-Henry intramedullary retrograde supracondylar nail. Risk Factors of Diaphyseal Femoral Nonunions: A Systematic Review and Meta-analysis.

BACKGROUND: Intramedullary nailing of diaphyseal femur fractures often results in reliable union, with union rates ranging from 96% to 100%. However, femoral nonunions (FNUs) can be a debilitating clinical condition for patients. The purpose of this review was to identify and characterize the predictors of nonunion after femoral midshaft fractures. METHODS: A systematic search was conducted using key terms "femoral shaft non-union," "femoral diaphyseal non-union," "femoral shaft fracture," "femoral diaphysis fracture," and "femoral midshaft fracture" published before June 2024. Human studies

Simple dislocations of the elbow: evaluation and treatment.

Simple dislocations of the elbow or dislocations occurring without an associated fracture are common injuries. Evaluation of these injuries must include an assessment of the entire involved upper extremity and a complete neurovascular examination. Principles of management include a prompt, controlled reduction, a determination of postreduction stability, and an immediate rehabilitation protocol that considers the stability of the joint following reduction. For those joints that are stable throughout the arc of motion, an unrestricted range of motion protocol can be started. When instability is

Conservative management of pediatric mandibular fractures with cap splints- A systematic review.

BACKGROUND: Mandible is the second most common bone involved among pediatric facial fractures, and the open reduction and internal fixation (ORIF) technique used for its management results in faster bone remodeling and quicker functional recovery. However, it can lead to damage to the periosteum and surrounding tooth buds, and may require a second surgery for removal. Therefore, conservative methods always remain mainstay for management among pediatric patients. This systematic review evaluates the use of cap splints in managing pediatric mandibular fractures and compares this approach with ot

[Hemi-arthroplasty--primary or secondary measure for 3- and 4-fragment fractures of the proximal humerus in the elderly?].

Successful treatment of three- and four-part proximal humerus fractures is a therapeutic challenge to the surgeon, particularly in the case of elderly patients. Open reduction and internal fixation have been advocated, but have not consistently produced acceptable results. The results of humeral head replacement as a salvage procedure after non-union or failed open reduction and internal fixation are less predictable. The outcome of hemiarthroplasty (Neer II) performed for three- and four-part proximal humerus fractures in elderly patients was studied. The average patient follow-up was 42.9 mo

Acute traumatic posterior shoulder dislocation.

Posterior shoulder dislocation occurs rarely and is challenging to manage. The mechanisms of trauma are varied, which complicates diagnosis. Missed or delayed diagnosis and treatment can have serious deleterious effects on shoulder function. All cases of suspected posterior shoulder dislocation require a high level of suspicion and appropriate imaging. Identification of associated injuries, such as fractures and rotator cuff tears, is important to guide treatment. In the acute setting, most patients are treated with closed or open reduction with additional soft-tissue or bony procedures. Patie

Fractures of the humeral capitellum: Herbert screw fixation.

During the first half of 1991, five patients with displaced fractures of the humeral capitellum were managed by open reduction and fixation of the capitellar fragments with Herbert bone screws. All patients had stable, painfree elbows at follow-up, with no restriction of normal daily activities. There were no evidence of early avascular necrosis of capitellar fragments, even where soft tissue attachments of small fragments had been injured. This method of fixation allows accurate restoration of the articular surface of the humero-radial joint and may therefore contribute to the excellent funct

[Fractures of the tibial pylon: treatment options and outcomes].

PURPOSE OF THE STUDY: Fractures of a tibial pylon are serious problems in traumatology. The methods of their therapy have not been unified yet and treatment outcomes are not satisfactory. This is often due to extensive comminution of the distal tibia, bone defects or a thin coverage of soft tissue. The objective of this study is to compare the literature data and, on the basis of this and our own experience, to present guidelines for the treatment of fractures of tibial pylons. MATERIAL: In the period from 1998 to June 2002, 60 patients (average age, 42 years) underwent surgery for a fracture

Incidence of Carpal Tunnel Syndrome in Distal Radius Fractures Treated by Various Modalities in a Tertiary Care Center: A Single Center Study.

Background Distal end radius fractures (DRF), which account for 17.5% of all fractures, are the most frequent fracture seen in emergency rooms. In patients with DRFs, delayed carpal tunnel syndrome (CTS) occurs in about 20% of cases. When patients are treated with DRFs using different modalities, CTS results in poor functional outcomes. Our study aims to identify the prevalence of CTS in DRF patients receiving treatment with various modalities. Materials and methods Two hundred twenty patients with a history of DRFs who were treated by a variety of modalities at R.L. Jalappa Hospital and Resea

Finite Element Analysis of 2- and 3-Point Internal Fixation Methods for the Treatment of Zygomaticomaxillary Complex Fracture.

Open reduction followed by internal fixation (ORIF) has been regarded as the most effective technique for surgical repair of zygomaticomaxillary complex (ZMC) fractures. However, the ideal internal fixation method to achieve stable reduction remains controversial. This research aims to assess and compare the stability of the 2- and 3-point screw-plate fixation methods using finite element method (FEM). Based on computed tomography (CT), the finite element models of 2-point (ZFS + IOR) and 3-point (ZFS + IOR + ZMB) fixation for isolated displaced ZMC fracture were reconstructed. The force of 12
