[Symptomatic true aneurysm of a venous bypass with compression of the right atrium].

A symptomatic true aneurysm of a saphenous vein graft to the right coronary artery in a female patient twelve years after bypass surgery is presented. Beating heart surgery included resection of the aneurysmatic saphenous vein graft, reconstruction of the right atrium and coronary artery bypass grafting to the right coronary artery. The symptoms of aneurysms of vein grafts, the diagnostic procedures and the surgical treatment are discussed.

Cyanosis in uncomplicated atrial septal defect with normal right cardiac and pulmonary arterial pressures.

A patient with cyanosis in an uncomplicated ostium secundum atrial septal defect without pulmonary hypertension is described. There were no anatomic abnormalities in right-sided cardiac valves or venous return and no evidence of right ventricular hypoplasia or hypertrophy; however, the diastolic pressure curve suggested a decreased compliance. We suggest that this unusual finding may be a result of intrinsically abnormal right ventricular compliance.

Young and Non-Smoking: An Unusual Case of Bronchial Carcinoid.

We report the case of a 30-year-old female, non-smoker, with no known allergies or occupational exposures, presenting with a two-year history of recurrent stridor and wheezing, recently exacerbating over the past two months. Physical examination and routine laboratory tests were largely unremarkable. Pulmonary function tests were within normal limits, with no significant bronchodilator response. Chest CT revealed a 15 × 13 mm perihilar right-sided lesion adjacent to the right pulmonary artery. Bronchoscopy identified a nearly obstructive, highly vascularized endobronchial lesion at the origin

Successful Combination Therapy of Optic Canal Decompression and Steroid Administration for Traumatic Optic Neuropathy in a 10-Year-Old Boy.

Traumatic optic neuropathy (TON) is a rare complication caused by head injury in children. TON treatment has employed conservative treatment, steroid administration, and surgical procedures; however, which treatment is preferable remains controversial. We herein present a case of a 10-year-old boy with a TON-complicated head injury after falling from a two-meter-high slide in a park. Initial head computed tomography (CT) revealed the right optic canal fracture, and the patient complained of right visual impairment. He was diagnosed with TON, and surgical right optic canal decompression was per

Uniportal video-assisted thoracoscopic surgery for complex mediastinal mature teratoma: A case report.

BACKGROUND: Mediastinal mature teratoma is the most common histological type of primary extragonadal germ cell tumor. In this report, we describe a rare case of giant mature teratoma located primarily in the anterior mediastinum and causing partial atelectasis of the upper and middle lobes of the right lung, as well as extrinsic compression of the right atrium. CASE SUMMARY: A 31-year-old male with a giant mediastinal mature teratoma presented with progressive exertional dyspnea and chest pain for 1 mo. Computed tomography of the chest indicated the diagnosis of anterior mediastinal teratoma.

Posterior Inferior Cerebellar Artery Thrombosed Aneurysm Associated with Persistent Primitive Hypoglossal Artery Successfully Treated with Condylar Fossa Approach.

A 68-year-old woman presented with generalized seizure due to the left internal carotid artery (ICA) aneurysmal compression of the ipsilateral medial temporal lobe. Computed tomography angiography (CTA) revealed multiple aneurysms of the right persistent primitive hypoglossal artery (PPHA), the right ICA, and the right anterior cerebral artery (ACA). The right PPHA originated from the ICA at the level of the C1 and C2 vertebral bodies and passed through the hypoglossal canal (HC). The PPHA aneurysm was large and thrombosed, which was located at the bifurcation of the right PPHA and the right p

Acupuncture for ischemic stroke: cerebellar activation may be a central mechanism following Deqi.

The needling sensation of Deqi during acupuncture is a key factor of influencing acupuncture outcome. Recent studies have mainly focused on the brain function effects of Deqi in a physiological state. Functional magnetic resonance imaging (fMRI) on the effects of acupuncture at Waiguan (SJ5) in pathological and physiological states is controversial. In this study, 12 patients with ischemic stroke received acupuncture at Waiguan (SJ5) and simultaneously underwent fMRI scanning of the brain, with imaging data of the activated areas obtained. Based on the patient's sensation, imaging data were al

Epistaxis during nasotracheal intubation: a comparison of nostril sides.

PURPOSE: It is commonly believed that for preventing epistaxis during nasotracheal intubation (NTI), the right nostril should be used. However, there is no real evidence as to which nostril should be used. In this study, we tested our hypothesis that epistaxis during NTI is more frequent and severe using the left nostril rather than the right, provided that patency appears equal on both sides of the nose. PATIENTS AND METHODS: A total of 54 patients who were scheduled for elective oral surgery, in which NTI was indicated to optimize the surgical approach, were recruited into this study. The n

[Graves ophthalmopathy with coexisting tumor of the right orbit and hyperprolactinemia--case report].

Ophthalmopathy is a typical manifestation of Graves disease. Although most commonly ophthalmopathy affects both eyes, sometimes it is localized in only one orbit; neoplastic process must be considered then. The coexistence of a tumor of the orbital area and Graves ophthalmopathy is extremely rare. We present a case of a 42-years-old female with bilateral Graves ophthalmopathy, and a coexisting tumor of the right orbit. Magnetic resonance imaging of the orbits revealed a thickening of both rectus inferior extraocular muscles typical for Graves ophthalmopathy. An irregular low-signal mass was ob

Use of the pedicle of previously harvested pectoral myocutaneous flap as a recipient for free flaps in head and neck reconstruction.

INTRODUCTION: The pectoral myocutaneous flap (PMF) is a workhorse regional reconstructive option for head and neck defects. It is commonly used for primary reconstructions due to its advantages or as a life-boat flap in the salvage of failed reconstructions of free flaps. However, it also has intrinsic drawbacks, such as perfusion problems and partial or complete flap loss. Although there are many studies about the advantages and use of PMF in the literature, the number of studies about salvage of this workhorse flap is inadequate. We aimed to present the use of the pedicle of previously perfo

Interventional Removal of Traumatic Nail from Skull Base After Endovascular Coiling of Internal Carotid Artery and Jugular Vein.

BACKGROUND: Cases of nail gun injuries to the head and neck are rare, and treatment approach varies with each case based on location and extent of injury. CASE DESCRIPTION: We present a case of a 52-year-old male who fell off a roof holding a pneumatic nail gun and accidentally shot himself with a nail through the right frontal neck region. The nail penetrated the right sternocleidomastoid muscle and extended intracranially through the right occipital condyle with its tip embedded in the right cerebellar hemisphere. The nail coursed between the right distal cervical internal jugular vein and

Tricuspid valve replacement after dislocation of a transcatheter tricuspid heart valve.

A 77-year-old female patient was referred to our department with dyspnoea, peripheral oedema and a history of right heart failure. Echocardiography revealed torrential tricuspid regurgitation. Since tricuspid-transcatheter edge-to-edge repair was not feasible and the patient refused open heart surgery, heart team decision was made to perform transcatheter tricuspid valve implantation in a borderline annulus dimension. During the procedure, migration of the valve in the right ventricle occurred. After patients' consent was obtained surgical explantation of the valve with subsequent surgical tri

Current Status of Tricuspid Valve Interventions in Asia Pacific Region.

Transcatheter tricuspid interventions are becoming increasingly more common in Asia Pacific. In the past decade, clinicians in Asia Pacific have worked with a multitude of new transcatheter tricuspid technologies. A standardized clinical algorithm to diagnose symptomatic tricuspid regurgitation to increase patient access to novel right heart therapies has not yet been identified. Anatomic diversity in the Asia Pacific patient population; disease prevalence patterns; and socioeconomic, cultural, and local health structures represent unique challenges in the treatment of these patients with righ

Hepatic haemangioma causing isolated right-sided pleural effusion and bilateral pedal oedema.

Hepatic haemangioma (HH) is a common benign tumour of the liver and is usually asymptomatic. HH causing isolated right-sided pleural effusion and bilateral pedal oedema due to inferior vena cava (IVC) compression have never been reported in the literature. We report a 35-year-old male patient who presented with breathlessness and mass per abdomen. On examination, the patient was found to have right-sided pleural effusion, bilateral pedal oedema, hepatomegaly. Contrast-enhanced CT showed compression of the IVC by the HH. The patient was managed with right-sided intercostal drain insertion for p

Neural mechanisms of embodiment: asomatognosia due to premotor cortex damage.

BACKGROUND: Patients with asomatognosia generally describe parts of their body as missing or disappeared from corporeal awareness. This disturbance is generally attributed to damage in the right posterior parietal cortex. However, recent neuroimaging and electrophysiological studies suggest that corporeal awareness and embodiment of body parts are instead linked to the premotor cortex of both hemispheres. PATIENT: We describe a patient with asomatognosia of her left arm due to damage in the right premotor and motor cortices. The patient's pathological embodiment for her left arm was associate

Ictal urinary urge: further evidence for lateralization to the nondominant hemisphere.

PURPOSE: To determine the lateralizing value of ictal urinary urgency. METHODS: A retrospective database search was performed for patients with ictal urinary urgency admitted to the Epilepsy Monitoring Unit at the Cleveland Clinic between 1994 and 2001. RESULTS: Six patients were identified; intracarotid amytal test demonstrated left hemispheric speech dominance in five cases. The sixth patient continued to speak during right temporal seizures. EEG and imaging data supported right temporal or frontotemporal epilepsy in all six cases. Two patients were seizure free after focal right hemispher

Truncal valve repair combined with right atrial appendage valved right ventricular-to-pulmonary connection in truncus arteriosus type II with single coronary artery.

We present a challenging case of truncus arteriosus type II repair in a 2-month-old, 3.3 kg infant admitted with heart failure and cyanosis. Right ventricular-to-pulmonary arterial continuity was restored using a right atrial free-wall autograft, while a neopulmonary valve was constructed from the right atrial appendage, providing an autologous valved connection. The patient also exhibited moderate-to-severe truncal valve stenosis and mild regurgitation, which were addressed with concomitant valve repair. The patient also had a single coronary artery.

Right Upper Quadrant Pain: A Rare Presentation of Diverticulitis.

Acute diverticulitis is a prevalent medical condition with increasing incidence rates. While the sigmoid colon is the most commonly affected part of the large intestine, there have been occurrences of right-sided diverticulitis although uncommon. We present a case report highlighting the atypical presentation of diverticulitis in a 27-year-old female patient. The patient experienced right upper and epigastric pain and was ultimately diagnosed with right-sided diverticulitis, supported by her CT imaging findings. This uncommon presentation underscores the importance of considering diverse clini

Spontaneous Pneumothorax Due to Ruptured Bulla of the Azygos Lobe.

Azygos lobe is an uncommon anatomic variant that is widely recognized, but rarely associated with pneumothorax. We present a successful surgical management of a spontaneous pneumothorax resulting from rupture of a bulla in an incidentally discovered azygos lobe. The patient is a 73-year-old man who presented with the first-time occurrence of a spontaneous right pneumothorax. The patient did not tolerate nonoperative management and underwent a right thoracotomy with bullae resection in the azygos and right lower lobes for definitive management. Our treatment highlights several considerations du

Cardiac herniation after extrapleural pneumonectomy.

Cardiac herniation is a fatal complication in patients undergoing pneumonectomy with pericardial resection. A 53-year-old man underwent right-sided extrapleural pneumonectomy for malignant pleural mesothelioma. He underwent right-sided pericardial resection and reconstruction with an expanded polytetrafluoroethylene sheet. Routine chest radiography performed 18 h postoperatively revealed cardiac herniation into the right-sided thoracic cavity. The patient was immediately transferred to the operating room, and the defect was repaired. He died of tumor progression. However, cardiac herniation di
