Lung cancer is the leading cause of cancer death in the United States. Fifteen percent of patients are asymptomatic at diagnosis. Symptoms usually indicate advanced disease. Signs include chronic cough, hemoptysis, wheezing, dyspnea, vague chest pain, hoarseness, pleural effusion, cardiac arrhythmias and bone pain. Significant surgical advances have reduced operational mortality rate. Early detection and surgery can increase survival rates for some patients.

The yellow nail syndrome is the combination of slow growing yellow discoloured nails, lymphoedema and pleural effusion. The symptoms are supposed to be due to impaired lymphatic drainage. It is a rare clinical condition: through the last 30 years only about a hundred cases have been reported. The treatment is symptomatic (diuretics and pleuradesis). We review the literature and report four cases.

A 70-year-old man was admitted to our hospital because of left-sided chest pain and dyspnea. Chest X-ray film showed a tumor in the left main bronchus and left pleural effusion. On bronchoscopy, a tumor which moved with respiration was observed in the left main bronchus. We successfully performed endoscopic laser polypectomy. Pathological examination revealed non-chondromatous hamartoma.

A 15-year-old boy with nephrotic syndrome, renal insufficiency, and cutaneous anergy had severe pneumonia with pleural effusion. There was serologic evidence of respiratory syncytial virus infection, and extensive microbiologic, histologic, and serologic studies failed to identify any other etiologic agent. Respiratory syncytial virus as a possible cause of severe pneumonia in immunocompromised hosts has not been previously reported.

In the present study, the immunocytochemical properties of formalin-fixed and alcohol-fixed specimens were compared to evaluate the usefulness of alcohol-fixed cytocentrifuged preparations for routine cytologic diagnosis. Re-expansion pulmonary oedema is an uncommon and potentially life-threatening complication after rapid expansion of a collapsed lung. Symptom onset (cough, chest pain, dyspnoea) is often sudden and the clinical picture may be dramatic. Pathogenesis is complex and partially unclear. A well-timed intensive therapy allows a better prognosis. To date, only one case of re-expansion pulmonary oedema following fenestration of a giant hepatic cyst has been reported in the literature. We report four patients who either presented with or developed serous effusions during the course of monocytic leukaemia. This finding has not been documented previously. All the patients had very high peripheral blood monocyte counts when effusions developed. The effusions resolved after the patients were treated with anti-leukaemic therapy. It is therefore likely that the effusions were disease related. Two of the patients have been treated with razoxane, one with etoposide and the other with cytarabine.
