Pseudo-hypersomnia and pre-sleep behaviour with bilateral paramedian thalamic lesions.

The sleep/wake status of three patients with bilateral lesions involving the paramedian thalamic regions was investigated. Long-term monitoring with infrared video camera and polygraphy were performed. In spite of presenting a behavioural aspect of sleep with sleep posture, eyes closed and lack of activity for 15-17 h per day, these subjects did not develop the normal non-rapid eye movement (NREM) and rapid eye movement (REM) sleep states during the daytime. The EEG indicated presence of a mixture of low amplitude, irregular, diffuse theta and alpha range frequencies during hours associated wi

[A 61-year-old man with rapidly progressing dementia and gait disturbance].

We report a 61-year-old male with rapidly progressive dementia and gait disturbance. He was well until spring 1990 as a postmaster, when there was an onset of memory disturbance and mistakes in his job. In May 1990, his wife noted slurring of his speech. In August, there was an onset of gait disturbance. He fell down frequently. In October, he was seen by a neurologist, who found moderate dementia, small step gait, retropulsion, freezing, paratonic rigidity, bradykinesia and a restriction in the vertical gaze on him. His dementia and gait disturbance progressed rapidly and in May 1991, he deve

Burnout and its causes in Finnish dentists.

In the spring of 1986 burnout and its relation to social and physical environments and the nature of work were studied using a questionnaire sent to a sample of 232 dentists aged under 62 living in the province of Uusimaa, Finland. Most male and female dentists (71 and 67%, respectively) were working in group practices and most (88%) employed an assistant. Professional problems were generally (71%) solved by consulting colleagues. Half of those responding were very satisfied with their relationship to other dental staff. All but 9% of dentists experienced problems in their physical working env

Anti-sympathetic nervous system autoantibodies. Diminished catecholamines with orthostasis.

The etiology of autonomic neuropathy in insulin-dependent diabetes mellitus (IDDM) is unknown. Previous studies have noted the presence of anti-adrenal medullary antibodies in IDDM. Recently, we have also demonstrated the presence of anti-sympathetic ganglia antibodies in IDDM. We initiated a study to evaluate whether subjects with complement-fixing anti-adrenal medullary (CF-ADM) and anti-sympathetic ganglia (CF-SG) antibodies have a decreased catecholamine response to change in posture. Seven IDDM subjects aged 19-41 yr with duration of disease 5-21 yr at the time of the posture study were e

[Report of a case of Kearns-Sayre syndrome associating with Isaacs-Mertens syndrome].

The patient is a woman who was born in 1936. Her father died of a valvular heart disease and her mother died of apoplexy. She was healthy until 1976 when she noticed stiffness of extremities and so she came to our hospital. At this time, her symptoms disappeared without any specific therapy. In 1979, she entered this hospital because of generalized edema. She was diagnosed as heart failure and treated effectively with frusemide. In 1982, stiffness and gait disturbance aggravated and entered the hospital. At this hospitalization, she was diagnosed as Kearns-Sayre syndrome. After the discharge,

M-mode echocardiography in supine and standing position in control subjects and patients with auscultatory evidence of mitral valve prolapse but negative supine echocardiography: does sensitivity improve?

Approximately 10-20% of patients with auscultatory evidence of mitral valve prolapse (MVP) have "false negative" echocardiograms. Pharmacological maneuvers do not significantly improve the sensitivity and may cause side effects. It is well known that auscultatory features of MVP change and not uncommonly can be heard only in the standing position. Therefore M-mode echo was performed in both supine and standing positions in 10 normal control subjects and 25 patients of comparable age and sex who had auscultatory evidence of MVP but negative echo in supine position. All 10 control subjects had n

Extradural hematoma: toward zero mortality. A prospective study.

This is a prospective analysis of 107 consecutive cases of extradural hematoma treated during the last 3 years at the Department of Neurosurgery of the University Hospital of Verona (Italy). The overall mortality was 5%; 89% of the patients made a good recovery or had only moderate residual disability. We regard this as meaningful progress compared to recent reports from other sources showing mortality rates of approximately 20%. The majority of our patients (57%) underwent operation within 6 hours of injury; 60% went into surgery with a Glasgow coma scale (GCS) score between 8 and 15. No deat

The role of the sphenoid bone in the connection between the temporal-mandible-tongue-hyoid system and the cervical spine.

The cranio-cervical-mandibular system is a functional complex whose understanding has evolved significantly, moving from a purely mechanical model to an integrated view that includes myofascial and postural interdependencies. Recent studies, particularly the work of Messina, have led to the definition of the Temporal-Mandible-Tongue-Hyoid (TMTH) system, a biomechanical unit in which the temporal bone acts as a fulcrum. This article aims to analyze the often-underestimated role of the sphenoid bone as a key element of anatomical and functional connection between the TMTH system and the upper ce

Unravelling neurodegeneration with cerebral calcifications: Krabbe disease masquerading as Aicardi-Goutieres syndrome.

Krabbe disease is an autosomal recessive, rare affliction characterised by progressive demyelination with neurodegeneration and peripheral nerve involvement. This report describes unique presentations in two children with cerebral calcifications. A female infant born to non-consanguineous parents presented with posturing and tightening of limbs, multiple paroxysmal events, loss of attained milestones and feeding difficulty. Second, a female infant presented with posturing since birth and sound-induced startles from a few weeks after birth. Examination in both children revealed microcephaly, bi

Bilateral vestibulopathy and disabling vertigo: A CARE case series.

INTRODUCTION: The term bilateral vestibulopathy (BV) was recently defined by the Bárány Society. Loss of otolith function was not included in their criteria. Although spontaneous progression to complete bilateral impairment of vestibular function is expected, it is unlikely that patients with advanced BV will continue to present episodes of intense vertigo. Here, following CARE case report guidelines, we report the case of patients meeting the criteria for BV and still disabled by vertigo. CASE SERIES: Three patients evaluated in our department meeting the Bárány criteria for definite BV but

Balance impairment and fragility vertebral fractures.

Postural instability and increased risk of falls are relevant problems in patients with fragility vertebral fractures (VFs). The inter-relationship between flexed posture (FP), VFs and altered balance control is complex and only partially elucidated. The purpose of the study is to identify key factors of balance impairment in osteoporotic patients with VFs. In particular, we focused on the role of FP, number and severity of VFs and their relationships with clinical and static balance instrumental measures. Secondary aim of the study is to investigate possible correlations of stabilometric para

Spontaneous Intracranial Hypotension Presenting With Frontotemporal Dementia: A Case Report.

Spontaneous intracranial hypotension (SIH) is a rare and often underdiagnosed condition, which commonly results from a cerebrospinal fluid leak. The classic clinical presentation of SIH is a postural headache and dizziness. Less frequent complications include nausea, neck stiffness, and even coma. This case report describes a 70-year-old woman with an initial complaint of postural headaches and sleep attacks, who developed a 22-month progressive history of personality and behavioral changes, cognitive decline, urinary incontinence, chorea, and dysarthria. Although no specific cerebrospinal flu

A child with acute transverse myelitis requiring permanent pacemaker implantation.

We diagnosed a 3-year-old girl with acute transverse myelitis (ATM). She presented with weakness of the limbs and developed urination difficulty and respiratory disturbance. Magnetic resonance imaging revealed a symmetric area of high signal intensity on T2-weighted images involving the lower end of the medulla oblongata to the level of the fourth thoracic vertebra. Anti-aquaporin-4 antibody was negative. She was treated with intravenous methylprednisolone pulse therapy, immunoglobulin therapy, and plasmapheresis; however, her clinical symptoms did not change. At 10 and 20days after symptom on

A Movement system impairment approach to evaluation and treatment of a person with lumbar radiculopathy: A case report.

BACKGROUND AND PURPOSE: There are several systems of classification and treatment of patients with low back pain (LBP) based on assessment of the effect of lumbar postures and movements on symptoms. The efficacy of one of these systems, The Movement System Impairment (MSI) method, has not yet been demonstrated in the literature. The purpose of this case report is to describe the approach of the MSI method for an individual with lumbar radiculopathy. CASE DESCRIPTION: A 79-year-old woman with a history of chronic LBP was referred to PT with a physician's diagnosis of sciatica. The patient was

Holmes' tremor as a delayed complication of thalamic stroke.

Movement disorders are not commonly associated with stroke. Accordingly, thalamic strokes have rarely been associated with tremor, pseudo-athetosis and dystonic postures. We present a 75-year-old man who developed a disabling tremor 1 year after a posterolateral thalamic stroke. This tremor had low frequency (3-4 Hz), did not disappear on focus and was exacerbated by maintaining a static posture and on target pursuit, which made it very difficult to perform basic functions. MRI demonstrated an old ischemic lesion at the left posterolateral thalamus. Treatment with levodopa led to symptom contr

Prevalence and risk factors of musculoskeletal disorders among Myanmar migrant workers in Thai seafood industries.

A cross-sectional descriptive study was conducted with self-administered questionnaires among 368 Myanmar migrant workers to investigate the prevalence of and to determine the risk factors for musculoskeletal disorders (MSDs) in the Thai seafood industry. The results showed the prevalence of MSDs was 45.1% occurring in the last 7 days. Marital status, number of dependents, other health problems, working hours, repetitive hand movements, awkward posture of wrists, prolonged standing, and manual handling of heavy loads were found to be associated with MSDs. Multiple logistic regression indicated

Hypothesis about an existent biomechanical cause-effect relationship between Schëuermann's kyphosis and scoliosis.

Schëuermanńs kyphosis is usually observed with a mild idiopathic scoliosis, and there is parity between these two diseases. The aim of this work is to establish a hypothesis about the existence of a biomechanical causal relationship between Schëuermann's kyphosis and scoliosis. To achieve this, a literature review was conducted. A simple mechanical model of the passive thoracolumbar subsystem was created to support part of the discussion. This mechanical model describes the passive thoracolumbar subsystem under ideal conditions of equilibrium. After giving consideration to the system under the

[Sandifer's syndrome in a 5-month-old child with suspicion of infantile spasms].

Sandifer's syndrome is a dystonic movement disorder in infants with gastroesophageal reflux (GER). It is probably misdiagnosed as epileptic seizures. We report the case of a 5-month-old infant with no past medical history admitted to a pediatric unit for suspicion of infantile spasms. She presented with dystonic movements of the upper left limb with left blepharospasm and an occasional dystonic head posture. Physical examination, EEG, brain MRI, and blood analysis were normal. Since the baby experienced regurgitations, Sandifer's syndrome was suspected and confirmed by 24-h esophageal pH monit
