Glucagon-like Peptide-1 receptor agonist use is associated with decreased risk of periprosthetic joint infection following total knee arthroplasty: A laterality-verified propensity matched comparison.

IntroductionDiabetes and obesity are associated with increased risk of complications following total knee arthroplasty (TKA). Glucagon-like … ry of the TriNetX database identified patients who underwent TKA and used GLP-1 RAs within a 1-year preoperative period. GLP-1 RA patients were 1:1 propensity matched to non-user controls for demographics and confounding comorbidities. Laterality was assessed independently to ensure ipsilateral TKA and corresponding complication. Outcomes analyzed were TKA revision, …

A 7-year minimum follow-up of the M6-L total disk replacement for low back pain with radiculopathy in young railway employees: a retrospective single-center study.

PURPOSE: To report our minimum 7-year clinical and radiographic outcomes, as well as complications of the M6-L disk prostheses in railway workers with lumbar disk herniation (LDH) causing low back pain with radiculopathy. METHODS: 97 cases underwent total disk replacement (TDR) using the M6-L disk prostheses between 2012 and 2017. Of those, 88.6% had surpassed the minimum 7-year follow-up and were included in the retrospective single-center study. Clinical and radiological data were obtained preoperatively, at 1-year and 106 (90;139) months post-operatively. Factors associated with delayed or

Assessing Left Ventricular Pathology in Patients with Ebstein Anomaly Using Cardiovascular Magnetic Resonance: Looking Past the Right Heart.

Ebstein Anomaly (EA) is a malformation of the right heart, but there is data to suggest that the left ventricle (LV) can suffer from intrinsic structural and functional abnormalities which affect surgical outcomes. The LV in patients with EA is hypertrabeculated with abnormalities in LV function and strain. In this retrospective single-center study, patients with EA who underwent pre-operative cardiac MRI (CMR) between the periods of 2014-2024 were included along with a group of healthy-age-matched controls. Left ventricular and right ventricular volume, function and strain analyses were perfo

Early Childhood Scoliosis Management by Vertical Expandable Prosthetic Titanium Rib (VEPTR): Experience of Royal Medical Services (RMS).

INTRODUCTION: Scoliosis could develop at the childhood age and progress beyond skeletal maturity. An early spinal fusion arrests growth of the spine and thorax, risking the development of secondary thoracic insufficiency syndrome. Vertical expandable prosthetic titanium rib (VEPTR) is a fusionless technique aiming to correct the deformity with preservation of growth potential. AIM: To show our experience and results regarding the use of VEPTR in children with scoliosis in regard to coronal profiles(length and deformity angle), spinal growth, and the complications we faced during the follow-up

Radiological and clinical outcomes of 3-level cervical disc arthroplasty.

OBJECTIVE: One- and two-level cervical disc arthroplasty (CDA) has been compared to anterior cervical discectomy and fusion (ACDF) in several large-scale, prospective, randomized trials that have demonstrated similar clinical outcomes. However, whether these results would be similar when treating 3-level disc herniation and/or spondylosis has remained unanswered. This study aimed to investigate the differences between 3-level CDA and ACDF. METHODS: A series of 50 patients who underwent 3-level CDA at C3-7 was retrospectively reviewed and compared with another series of 50 patients (age- and s

Factors predicting the need for cerebrospinal fluid diversion following posterior fossa tumor surgery in children.

BACKGROUND: No consensus exists regarding the management of hydrocephalus in children with posterior fossa tumors before, during or after surgery. In the present study we analyze the factors that predispose to persistent hydrocephalus and the need for a postoperative cerebrospinal fluid (CSF) diversion procedure. METHODS: Pediatric patients who underwent surgery for posterior fossa tumors with hydrocephalus at our hospital were reviewed to evaluate the need for a postoperative CSF diversion procedure. Patients having undergone CSF diversion preoperatively were excluded from the study group. T

Back pain and disability after lumbar laminectomy: is there a relationship to muscle retraction?

OBJECTIVE: Preliminary studies have suggested that prolonged retraction of the paraspinal muscle during spinal surgery may produce ischemic damage. We report the continuous measurement of intramuscular pressure (IMP) during decompressive lumbar laminectomy and its relationship to subsequent back pain and disability. METHODS: Twenty patients undergoing two-level decompressive lumbar laminectomy for lumbar canal stenosis were recruited. Back pain and disability were assessed by use of the Visual Analog Score (VAS), Oswestry Disability Index (ODI), and Short-Form 36 (SF-36) Health Survey. During

Connectivity alteration in thalamic nuclei and default mode network-related area in memory processes in mesial temporal lobe epilepsy using magnetoencephalography.

This work aimed to investigate the involvement of the thalamic nuclei in mesial temporal lobe epilepsy (MTLE) and identify the influence of interictal epileptic discharges on the neural basis of memory processing by evaluating the functional connectivity (FC) between the thalamic nuclei and default mode network-related area (DMNRA) using magnetoencephalography. Preoperative datasets of nine patients with MTLE with seizure-free status after surgery and those of nine healthy controls were analyzed. The FC between the thalamic nuclei (anterior nucleus [ANT], mediodorsal nucleus [MD], intralaminar

The perioperative management of patients maintained on medications used to manage opioid addiction.

PURPOSE OF REVIEW: The substantial increase in prescription and illicit opioid abuse observed over the last 2 decades has significantly increased the number of patients in recovery from addiction and now maintained on opioid replacement or agonist therapy. These patients present unique challenges to perioperative pain management. RECENT FINDINGS: Standard opioid-based analgesic techniques are often not sufficient when patients are maintained on medications used to manage opioid addiction. The current recommendations to support perioperative pain management plans in this population are based o

Nasal encephalocele: endoscopic excision with anesthetic consideration.

OBJECTIVE: Nasal encephalocele may presents as a nasal mass, its treatment is surgical and it should be done early in life. When removal is indicated, there are multiple surgical approaches; including lateral rhinotomy, a transnasal approach and a coronal flap approach. However, the treatment of a basal intranasal encephalocele using transnasal endoscopic approach could obviates the possible morbidity associated with other approaches. The aim of this study was to evaluate the efficacy of endoscopic removal of intranasal encephalocele, also to document the role of anesthetist in the operative a

Preoperative Blood Augmentation in Jehovah's Witness Patients Undergoing Transfusion-Free Surgery for Gastrointestinal or Hepato-biliary-pancreatic Cancer: Retrospective Cohort Study.

BACKGROUND: Patients with anemia preparing for abdominal cancer surgery in whom transfusion is not an option require careful preoperative management. Cancer-related anemia is often multifactorial. Therefore, we applied a blood augmentation protocol based on patient blood management (PBM), which optimizes the patient's own red cell mass. We aimed to evaluate the effect of this protocol on surgical outcomes in patients with gastrointestinal and hepato-biliary-pancreatic cancers requiring transfusion-free surgery by comparing outcomes between patients with and without preoperative anemia (the ane

Outcomes of radial head implants in total elbow arthroplasty.

BACKGROUND: There is no consensus on the management of the radial head in total elbow arthroplasty (TEA). In 3-part TEA designs, options include radial head retention, excision, or arthroplasty. Biomechanical studies suggest improved varus-valgus stability with radial head implants in unlinked total elbows. Unfortunately, complications with radial head implants have been common with historical designs. The aim of this study was to evaluate the clinical and radiographic outcomes of radial head implants in a current 3-part TEA and identify risk factors for mechanical failure. METHODS: We perfor
