Adenoid Cystic Carcinoma of the Lacrimal Gland With Late Mandibular Metastasis Presenting as an Intraoral Mass: A Case Report and Literature Review.

Adenoid cystic carcinoma (ACC) of the lacrimal gland is an uncommon but clinically aggressive malignancy, characterized by perineural invasion, local recurrence, and delayed distant metastasis. Mandibular metastasis is exceptionally rare. We report a 61-year-old man who presented with progressive right orbital swelling, proptosis, and visual loss. Imaging demonstrated a destructive lacrimal gland mass with temporal bone involvement. The patient underwent surgical debulking followed by radiotherapy, with subsequent local recurrence. Two years later, he developed a painless right posterior mandi

So What Exactly Is Progressive Collapsing Foot Deformity?

Progressive collapsing foot deformity is a new nomenclature coined by a group of expert foot and ankle orthopaedic surgeons who published numerous articles on adult flatfoot deformities. The consensus statements were published in 2020 due to concern that traditional terminologies such as "posterior tibial tendon dysfunction," "adult acquired flatfoot deformity," and numerous others could be confusing to physicians/health care professionals and do not adequately encompass the deformity. The consensus group also designed a new classification system to simplify the decision-making process that co

Cervicosacral stabilisation in alkaptonuria: seven-year surgical management of multisystem ochronosis including joint arthroplasties.

Alkaptonuria (AKU) is an ultra-rare autosomal recessive disorder caused by homogentisic acid (HGA) accumulation, leading to ochronosis and progressive joint and spinal degeneration. Misdiagnosis as ankylosing spondylitis (AS) is possible due to overlapping spinal features. We present a man in his late 40s with ochronotic arthropathy, initially diagnosed as AS. Diagnosis was confirmed by elevated urinary HGA. Over 7 years, he underwent bilateral total shoulder, hip and knee arthroplasties, and long-segment cervicosacral fusion with pedicle subtraction osteotomy. Histopathology confirmed ochronot

Keratoconus associated with Fuchs' Corneal Endothelial Dystrophy in a Young Woman.

INTRODUCTION: Keratoconus has association with various ocular diseases. Here a case of the co-occurrence of keratoconus and Fuchs' corneal endothelial dystrophy (FCED), a rare entity has been reported. CASE: A 26-year-old woman presented with a sudden onset of blurring of vision of right eye (RE) three weeks back. OBSERVATION: The RE showed features of advanced keratoconus with apical scaring due to acute hydrops. Her left eye (LE) had myopic astigmatism with Vogt's striae. Careful examination revealed corneal guttate in both eyes. The co-existence of keratoconus and FCED was confirmed by to

When to intubate in acute hypoxaemic respiratory failure? Options and opportunities for evidence-informed decision making in the intensive care unit.

The optimal timing of intubation in acute hypoxaemic respiratory failure is uncertain and became a point of controversy during the COVID-19 pandemic. Invasive mechanical ventilation is a potentially life-saving intervention but carries substantial risks, including injury to the lungs and diaphragm, pneumonia, intensive care unit-acquired muscle weakness, and haemodynamic impairment. In deciding when to intubate, clinicians must balance premature exposure to the risks of ventilation with the potential harms of unassisted breathing, including disease progression and worsening multiorgan failure.

Clinical staging of Anderson-Fabry cardiomyopathy: An operative proposal.

As a slowly progressive form of hypertrophic cardiomyopathy (HCM), Anderson-Fabry disease (FD) resembles the phenotype of the most common sarcomeric forms, although significant differences in presentation and long-term progression may help determine the correct diagnosis. A variety of electrocardiographic and imaging features of FD cardiomyopathy have been described at different times in the course of the disease, and considerable discrepancies remain regarding the assessment of disease severity by individual physicians. Therefore, we here propose a practical staging of FD cardiomyopathy, in h

Multi-Ethnic Study of Atherosclerosis (MESA): JACC Focus Seminar 5/8.

The MESA (Multi-Ethnic Study of Atherosclerosis) is a National Heart, Lung, and Blood Institute-sponsored prospective study aimed at studying the prevalence, progression, determinants, and prognostic significance of subclinical cardiovascular disease in a sex-balanced, multiethnic, community-dwelling U.S. cohort. MESA helped usher in an era of noninvasive evaluation of subclinical atherosclerosis presence, burden, and progression for the evaluation of atherosclerotic cardiovascular disease risk, beyond what could be predicted by traditional risk factors alone. Concepts developed in MESA have i

[Stage-appropriate treatment of keratoconus].

Keratoconus (KC) is a progressive cone-shaped corneal protrusion that causes paracentral thinning at the apex of the cone and typically occurs asymmetrically on both sides. After a careful anamnesis and classification of the degree of severity a targeted treatment appropriate to the stage of the disease is available. If the visual acuity is no longer sufficient, rigid gas-permeable contact lenses (CL) are fitted by a specialist. Riboflavin UVA cross-linking (CXL) is recommended in cases of progression and visual acuity that is still useful for the patient. Intracorneal ring segments (ICRS) are

[Treatment of renal artery stenosis in the year 2021].

Severe arteriosclerotic stenosis of the renal artery with at least 60-70% narrowing of the lumen can lead to various diseases: in the case of unilateral stenosis it can lead to renovascular hypertension, in the case of bilateral narrowing (or in a stenotic solitary kidney) also to an often progressive renal insufficiency (ischemic kidney disease) and/or to acute pulmonary edema (pulmonary flash edema). Renal artery stenosis may be treated by revascularization using either percutaneous (balloon angioplasty with or without stenting) or less commonly open surgical procedures, both with excellent

Systemic karyomegaly with primary pulmonary presentation.

Systemic karyomegaly is a distinct disorder characterized by progressive renal failure and enlarged, bizarre renal tubular epithelial cells. We report the first case of systemic karyomegaly with primary pulmonary presentation and present the first detailed characterization of the karyomegalic cells in lung tissue. A 33-year-old woman was evaluated for chronic and progressive restrictive lung disease, ultimately necessitating single-lung transplantation. Her post-transplant course was marked by graft dysfunction, respiratory decline and renal failure culminating in her death 97 days post-transp

Potential risks of calcium channel blockers in chronic kidney disease.

Antihypertensive therapy remains the most effective strategy for slowing the progression of chronic kidney disease (CKD). However, in proteinuric nephropathies, calcium channel blockers (CCBs) are less effective than other antihypertensives unless normotension is achieved. This is because the glomerular capillaries, rather than larger vessels, are the primary site of hypertensive injury in proteinuric nephropathies. CCBs impair renal autoregulation, which protects glomerular capillaries against the transmission of systemic pressures. CCBs' renoprotective inferiority in the comparator group lik

Idiopathic neuropathy, prediabetes and the metabolic syndrome.

Peripheral neuropathy is a common problem encountered by neurologists and primary care physicians. While there are many causes for peripheral neuropathy, none can be identified in a large percentage of patients ("idiopathic neuropathy"). Despite its high prevalence, idiopathic neuropathy is poorly studied and understood. There is evolving evidence that impaired glucose tolerance (prediabetes) is associated with idiopathic neuropathy. Preliminary data from a multicenter study of diet and exercise in prediabetes (the Impaired Glucose Tolerance Neuropathy Study) suggests a diet and exercise couns

Another toxic shock syndrome. Streptococcal infection is even more dangerous than the staphylococcal form.

In the battery of diseases caused by group A streptococci, streptococcal toxic shock syndrome is among the most severe. Its nonspecific presentation and rapid progression require that the treating physician be both knowledgeable and vigilant. Certain signs and symptoms suggest that caution is in order: Pain out of proportion to physical findings may herald deep tissue involvement in a patient who at first glance appears to have cellulitis. Streptococcal infection accompanied by hypotension, confusion, or unexplained acute renal insufficiency is a clue to streptococcal toxic shock syndrome. Tre

The use of current knowledge and non-invasive testing modalities for predicting at-risk non-alcoholic steatohepatitis and assessing fibrosis.

There is ongoing recognition of the burden of non-alcoholic fatty liver disease (NAFLD) and non-alcoholic steatohepatitis (NASH), with fibrosis being the most important histological feature that is associated with progression to cirrhosis and the occurrence of major adverse liver outcomes. Liver biopsy is the gold standard applied to detect NASH and determine the stage of fibrosis, but its use is limited. There is a need for non-invasive testing (NIT) techniques to identify patients considered at-risk NASH (NASH with NAFLD activity score > 4 and ≥ F2 fibrosis). For NAFLD-associated fibrosis, s

Niche point-of-care endocrine testing - Reviews of intraoperative parathyroid hormone and cortisol monitoring.

Point-of-care (POC) testing, which provides quick test results in near-patient settings with easy-to-use devices, has grown continually in recent decades. Among near-patient and on-site tests, rapid intraoperative and intra-procedural assays are used to quickly deliver critical information and thereby improve patient outcomes. Rapid intraoperative parathyroid hormone (ioPTH) monitoring measures postoperative reduction of parathyroid hormone (PTH) to predict surgical outcome in patients with primary hyperparathyroidism, and therefore contributes to the change of parathyroidectomy to a minimally

Operative management of necrotizing pancreatitis--necrosectomy and continuous closed postoperative lavage of the lesser sac.

In patients with severe acute pancreatitis, the most important diagnostic goal is differentiation between the interstitial-edematous and the necrotizing type of acute pancreatitis. Surgical management in patients with proven necrotizing pancreatitis is indicated in patients who develop surgical acute abdomen, sepsis, shock syndrome, multisystemic organ failure syndrome, persistent or progressive despite maximum intensive care. The most appropriate procedure for surgical management of pancreatic necrosis is the careful removal of necrosis and preservation of vital pancreatic tissue. Necrosectom

Cardiac sequelae of acute head injury.

An intensive 72 hour study of the cardiovascular effects of severe diffuse brain injury was made in seven patients. No other major injury was present. Patients received optimal care very rapidly after injury and intracranial pressure, which was continuously monitored, was maintained at or just above normal by elective positive pressure ventilation techniques. Three patients showed evidence of progressive myocardial ischaemia on continuous electrocardiographic monitoring. Two of these patients died. Ventricular arrhythmias were uncommon but one of the patients with ischaemia developed ventricul

Resource-stratified machine learning framework for cognitive status classification and mild cognitive impairment to dementia progression prediction.

BACKGROUND: Widespread access to diagnosis of cognitive decline remains inadequate. Assessment tools rely on neuroimaging, biofluid markers, or lengthy neuropsychological batteries. This reliance limits their use in primary care and low-resource settings and contributes to healthcare disparities. We developed and validated a three-level, resource-stratified machine learning framework to provide scalable dementia screening and prognostic risk stratification across diverse healthcare settings. METHODS: We used data from 31,081 participants in the National Alzheimer’s Coordinating Center. We trai
